TITLE V OPERATING PERMIT R. R. DONNELLEY LEAR FACILITY

I. Introduction and General Description of Operations This permit pertains to the operations of the Reno Plant of R. R. Donnelley & Sons Company which is located at 14100 Lear Blvd., Reno, Nevada 89506. The Reno Plant prints magazines, catalogs, tabloids, newspaper inserts and other printed material using the gravure printing process (SIC # 2754 / NAICS# 323111) for distribution throughout the United States. Other process support equipment located at this source includes boilers, gravure cylinder manufacturing (including chrome and copper plating equipment), paper cutting and pneumatic byproduct paper dust collection and handling systems, ink and solvent storage tanks, as well as other related product finishing equipment. Emissions from the gravure printing process result from the evaporation of gravure ink solvent, which is primarily toluene. Toluene is both a volatile organic compound (VOC) and a listed hazardous air pollutant (HAP). The ink solvent may also contain trace quantities of other listed HAPs such as Ethyl Benzene and Xylene. Both stack and in plant fugitive emissions from operations using gravure solvent are captured and collected for recovery in carbon bed adsorbers for subsequent reuse or resale to ink supplier(s). The gravure printing operations are subject to the requirements of the Printing and Publishing MACT standard promulgated by EPA as 40 CFR 63 Subpart KK. The MACT compliance date for this source was May 30, 1999. Four boilers provide steam for use in desorbing the gravure ink solvent from the carbon bed solvent recovery system, for the gravure press dryers and for plant space heating during the cool weather seasons. These boilers use natural gas as a primary fuel with No. 2 fuel used as a backup fuel. Boiler No. 4 is subject to the requirements of the Small Industrial Commercial Institutional Steam Generating Unit NSPS standard of 40 CFR 60 Subpart Dc. The NSPS compliance date for this boiler was at start up. The four boiler operations are also subject to the requirements of the Boiler MACT standard promulgated by EPA as 40 CFR 63 Subpart DDDDD. The MACT compliance date for this source was January 31, 2016. Manufacturing operations at this facility also include the chrome-plating of gravure cylinders. The facility’s chrome plating tanks are subject to the requirements of the chrome MACT standard promulgated by EPA at 40 CFR 63 Subpart N. The compliance date for this equipment was January 25, 1997. Two (2) cyclones are used to collect and convey by-product paper from trim and paper shredder (hogger) operations to the baler for subsequent off site recycling of the paper. These operations result in emissions of particulate matter (paper dust). A complete list of equipment (except insignificant sources) from which emissions are allowed by this permit is given in Section III “SIGNIFICANT EQUIPMENT UNITS”. Emissions listed in Section IV "Facility Emissions Limitations" constitute best engineering estimates of the potential emissions of equipment subject to regulation as set forth in the permit application. Attachment A lists insignificant sources.

V. Rotogravure Printing

A) Limitations The permittee shall meet all applicable requirements of 40 CFR 60 Subparts A and QQ, and 63 Subparts KK, including the following:

1) Rotogravure Presses, General a) Volatile organic compound emissions from the production rotogravure presses shall not exceed more than 16

percent of the total mass of VOCs used during any one performance averaging period. [40 CFR 60.432]

b) The permittee shall operate an activated carbon adsorption system to control emissions from the rotogravure press operations. [PSD Permit # NSR-4-7-2, NV 84-01]

c) All rotogravure presses shall be vented to the carbon adsorption system which must capture enough of the solvents from the presses to enable the facility to meet the requirements of 40 CFR 63, Subpart KK. [40 CFR 63.824]

d) The carbon adsorption system will be maintained in good working condition in accordance with designed operating specifications. [40 CFR 60.11]

e) The permittee, for the publication rotogravure source, shall limit emissions of organic HAP to no more than eight percent of the total volatile matter used each month. The emission limitation may be achieved by overall control of at least 92 percent of organic HAP used, by substitution of non-HAP materials for organic HAP, or by a combination of capture and control technologies and substitution of materials. [40 CFR 63.824]

f) The permittee shall meet the following operation and maintenance requirements: (i) At all times, including periods of startup, shutdown, and malfunction, the permittee shall operate and

maintain the facility, including associated air pollution control equipment, in a manner consistent with good air pollution control practices for minimizing emissions at least to the levels required by all relevant standards.

(ii) Malfunctions shall be corrected as soon as practicable after their occurrence. (iii) Determination of whether acceptable operation and maintenance procedures are being used will be based on

information available to the Administrator which may include, but is not limited to review of operation and maintenance procedures, review of operation and maintenance records, and inspection of the source.

2) Emissions Limits, NR625-N629: a) The permittee shall not discharge or cause the discharge into the atmosphere hydrocarbons in excess of 1365

lbs/hour or 16 percent of the total mass of hydrocarbons used by rotogravure presses NR625 – NR629 based on a 30 day calendar average. [PSD Permit NSR4-7-2 NV-84-01 Condition IX]

b) The permittee shall limit the operations of rotogravure presses NR625-NR629 such that none of these presses operate for more than 6400 hours per year. Only hours where the press is applying ink to product and the press is creating emissions shall be counted as “in operation” and applied to the 6400-hour limitation. Maintenance, repair and replacement operations shall not be counted into the 6400 hours limitation. [PSD Permit NSR4-7-2 NV-84-01 Condition IX.D (1)]

3) Emissions Limits, NR624 and NR630:

a) The permittee shall limit the operations of NR624 such that net emissions increase due to this press shall not exceed 54.5 tons per year, which is based on limiting press emissions to no more than 39.0 tons per year, with all banked creditable emissions decreases taken into account.

b) The permittee shall limit the operations of NR630 such that net emissions increase due to this press shall not exceed 45.4 tons per year, which is based on limiting press emissions to no more than 39.0 tons per year, with all banked creditable emissions decreases taken into account. [40 CFR 52.21]

c) The permittee shall limit the operations of presses NR624 and NR630 such that emissions do not exceed the limitations in this condition for all rolling 12 month periods.

d) The operator shall construct and maintain emissions control enclosures around both press NR624 and press NR630. The presses are at separate locations in the facility, and the enclosures are separate as well. The enclosures shall be rigid and stable and meet all the terms and requirements of a “permanent total enclosure” as described and defined in Federal EPA Method 204 (40 CFR 51, Appendix M). All emissions captured in the enclosures shall be vented to the existing carbon adsorption system, and subject to MACT control requirements. [District Regulation 030.700]

(Note that presses NR624 and NR 630 were each added as minor modifications to a major source permit. The emissions limit therefore references the major source limitations of 40 CFR 52.21, in that the presses must not exceed that limitation in order to remain minor.) Emissions Offsets - The operator shall maintain all emissions decreases used in the netting calculations as proposed in the application for Authority to Construct which was issued by the AQMD January 26, 2005. [40CFR 52.21].

B) Monitoring 1) The permittee will install and operate an emissions monitoring system to track the volatile organic compound

concentration of both the input and exhaust gases from the carbon bed adsorption solvent recovery system. The primary purpose of this device is to monitor the carbon adsorption system for breakthrough. The monitor shall be an NDIR, or FID type or equivalent design as approved by the Control Officer.

2) The emissions monitoring system will be calibrated at least annually and more often if necessary to insure accurate readings. It is both understood and allowable that the monitors shall operate in a semi-continuous mode, cycling back and forth between several intake points in sequence in order to monitor the overall efficiency of the solvent recovery system.

3) The location of inlet gas collection probe shall be downstream of the all press dryer collection ducts. The exhaust probe shall be at least 3 stack diameters distance upstream from the exhaust exit point or some equivalent point as approved by the Control Officer.

4) Presses NR624 and NR630 will use the capture efficiency across the carbon adsorption system as measured by the emissions monitoring system to calculate their emissions. Emissions from the presses must be determined based on the liquid-liquid mass balance efficiency for the carbon bed adsorption solvent recovery system as specified in 40 CFR 63.824.

5) The amount of volatile organics used in and recovered from gravure operations and the resultant overall solvent recovery efficiency and emissions calculated for each calendar month. [District Regulation 030.700]

C) Testing 1) The VOC content of all organic solvent based printing products (i.e. inks and cleaners…) shall be determined using

methods specified in 40CFR 63.827(c)(1-3). The VOC content analysis shall speciate both benzene and toluene for each organic solvent based product used in the rotogravure presses.

2) The operator shall demonstrate compliance with the requirements of 40 CFR 63.824 - Standards for Publication Rotogravure Printing. The procedure to be used shall be the one described in 40 CFR 63.824 (b)(1)(i). Because compliance with this standard will be demonstrated by means of a liquid-liquid materials balance, no capture efficiency performance test will be required.

D) Record Keeping 1) The permittee shall record all measurements needed to demonstrate compliance with 40 CFR 63.829, such as

a) continuous emission monitor data, b) control device and capture system operating parameter data,

2) All applicable record keeping provisions of 40 CFR 63 Subpart KK apply to the operations of all presses. 3) The operator will maintain records of all ink and other materials consumed in the operations of presses, and the VOC

and HAP content of those materials. 4) The amount of volatile organics used in and recovered from gravure operations and the resultant overall solvent

recovery efficiency and emissions calculated for each calendar month.

5) The volatile matter (VOC) content of all volatile materials shall be determined using methods specified in 40CFR 63.827(c)(1)-(3). The VOC information shall include the specific contents of toluene for each solvent containing product used in the rotogravure press systems.

6) The organic HAP content of these materials shall be calculated using methods specified in 40CFR 63.827(b)(1)(i)-(iii). The calculations of volatile matter and HAP as required under 40CFR 63.827(b)(1) and 40CFR 63.827(c) shall consider all inks, coatings, varnishes, adhesives primers, solvents thinners, reducers, thinners and other materials considered for the purposes of meeting the requirements of 40 CFR 63.824.

7) The permittee shall use the quantities of inks and other materials consumed by each press NR624 and NR630, the VOC and HAP content of those materials, and the control efficiency of the carbon absorption system as determined by the monitor required in condition V (B) to calculate emissions from the presses.

8) Emissions from all presses shall be calculated monthly and annual emissions shall be determined each month using a 12 month rolling average.

9) All solvent emissions from the press systems shall be considered as both HAPs and VOC. 10) These records shall be maintained in accordance with the requirements of 40 CFR 63.10(b). [PSD Permit NSR 4-7-2 NV-84-01, 40 CFR 52.21, 40 CFR 63.824 and 40 CFR 63.827]

E) Reporting 1) The permittee shall furnish the District with a written report of gravure solvent use and recovery for every calendar

month. [PSD Permit NSR 4-7-2 NV-84-01, 40 CFR 63.824-830]

2) The permittee shall furnish a written report of solvent use and recovery to the EPA administrator quarterly. [40 CFR 60 Subpart QQ; 40CFR 63.827(b)(1) and 40CFR 63.827(c)(1); PSD Permit # NSR4-7-2 NV 84]

3) The operator shall demonstrate compliance with the requirements of 40 CFR 63.824. The procedure to be used shall be the one described in 40 CFR 63.824 (b)(1)(i). Because compliance with this standard will be demonstrated by means of a liquid-liquid materials balance, no capture efficiency performance test will be required.

4) All Statements of compliance must be made in writing to both the EPA Administrator as well as to the Control Officer. Failure to submit a statement of compliance in a timely manner may result in citation, penalties, suspension or revocation of the Part 70 permit. [40 CFR 70.6 (c)(1); Clean Air Act (2014 Revisions) Section 182 (a)(3)(b) and Section 184 (b)(2)].

VI. Ink and Solvent Storage Tanks

1) **All equipment associated with delivery, storage, transfer and loading operations shall be maintained in a leak free and vapor tight condition and in good working order. During transfer and loading it shall be permissible to vent the equipment to prevent over pressurizing the system.

2) **All working loss emissions from the bulk storage tank and tank farm ink and solvent storage tanks shall be vented into the Carbon Adsorption system. [District Regulation 030.217]

3) Only water which has been processed in the “pre-treatment” plant for removal of plating system metals; or comes from the water softener regeneration systems of the boilers may be as feed input into the waste water evaporator for treatment. [District Minor Source Permit dated Sept. 21, 2008]

VII. Boiler Operations

A) Limitations 1) Each existing boiler must comply with 40 CFR 63 Subpart DDDDD no later than January 31, 2016, except as provided in

40 CFR 63.6(i). [40 CFR 63.7495(b)] 2) The permittee must meet the notification requirements in 40 CFR 63.7545 according to the schedule in 40 CFR 63.7545

and in subpart A of 40 CFR part 63. Some of the notifications must be submitted before you are required to comply with the emission limits and work practice standards in 40 CFR 63 Subpart DDDDD. [40 CFR 63.7495(d)]

3) At all times, the permittee must operate and maintain each affected source (as defined in 40 CFR 63.7490), including associated air pollution control equipment and monitoring equipment, in a manner consistent with safety and good air pollution control practices for minimizing emissions and according to manufacturer’s specifications. Determination of whether such operation and maintenance procedures are being used will be based on information available to the administrator that may include, but is not limited to, monitoring results, review of operation and maintenance procedures, review of operation and maintenance records, and inspection of the source. [40 CFR 63.7500(a)(3)]

4) The permittee shall not discharge or cause the discharge into the atmosphere sulfur oxides in excess of 51 lbs/hour (based on a 2 hour average) from boilers Nos. 1 - 3. The permittee shall not discharge or cause the discharge into the atmosphere sulfur oxides in excess of 14.5 lbs/hour from boiler 4.

5) Each boiler is subject to the subcategory requirements in 40 CFR 63 Subpart DDDDD for units that are designed to burn gas 1 fuels and/or light liquid fuels, as defined in 40 CFR 63.7575. Each boiler must be incompliance with the requirements of the applicable subcategory when that fuel is being burned. [40 CFR 63.7499(l) and (u) and 40 CFR 63.7495(i)]

6) Boilers and process heaters burning natural gas are not subject to the emission limits in Tables 1, 2 and 4, or 11 through 13 to 40 CFR 63 Subpart DDDDD. [40 CFR 63.7500(e)]

7) When burning No. 2, CO emissions must not exceed 130 ppm by volume on a dry basis corrected to 3 percent oxygen

(or 0.13 lb/MMBtu of steam output or 1.4 lb/MWh), except during startup and shutdown, based on a 1 hour minimum

sample time. [40 CFR 63.7500(a)(1), Table 1, Item 16 to 40 CFR 63 Subpart DDDDD]

8) When burning No. 2, filterable PM shall not exceed 0.0011 lb/MMBtu of heat input (or 0.0012 lb/MMBtu of steam

output or 0.016 lb/MWh, except during startup and shutdown, as determined by collecting a minimum of 3 dscm per

test run. [40 CFR 63.7500(a)(1), Table 1, Item 16 to 40 CFR 63 Subpart DDDDD]

9) When burning No. 2, maintain the 30-day rolling average operating load of each unit such that is does not exceed 110

percent of the highest hourly average operating load recorded during the performance test. [40 CFR 63.7500(a)(2) and

Item 7, Table 4 to 40 CFR 63 Subpart DDDDD]

10) For boilers Nos. 1-3, the permittee shall not burn more than 2.5 million gallons of No. 2 fuel oil in any 12 month period.

For Boiler No. 4, the permittee shall not burn more than 1.0 million gallons of oil in any 12 month period.

[PSD Permit NSR4-7-2 NV-84-01] 11) The General Provisions in 40 CFR 63.1 through 63.15 applies as specified in Table 10 to Subpart DDDDD of part 63.

[40 CFR 63.7565] 12) For each new boiler, an initial 5-year tune-up must be completed no later than 61 months after the initial startup of

the affected unit. [40 CFR 63.7510(g), 40 CFR 63.7515(d)]

B) Fuel Switching 1) The operator must notify the Air Quality Management Division whenever the primary fuel is switched from gas to oil

or from oil to gas. The notification must be made by the end of the next business day following the change. [District Regulation 030.217]

2) The fuel switch notification must identify: a) The name of the owner or operator of the affected source, as defined in 40 CFR 63.7490,

b) The location of the source, the boiler(s) and process heater(s) that have switched fuels, or were physically changed, and

c) The date of the fuel switch and the date of the notice. d) The currently applicable subcategory (gas 1 fuel or liquid fuel) under 40 CFR 63 Subpart DDDDD.

[40 CFR 63.7545(h)] e) Fuel oil sulfur content. [PSD permit NSR 4-7-2 NV-84-01]

[PSD Permit NSR 4-7-2 NV-84-01 Condition IX] 3) If the permittee switches fuel, you must be in compliance with the applicable requirements for the new fuel on the

effective date of the fuel switch. [40 CFR 63.7495(i)] 4) The permittee must conduct new performance tests within 60 days of burning the new fuel type. [40CFR63.7515(h)] 5) If the permittee switches fuels after the initial compliance date, you must demonstrate compliance within 60 days of

the effective date of the switch, unless you had previously conducted your compliance demonstration for the new fuel within the previous 12 months. [40 CFR 63.7510(k)]

6) If the permittee uses an alternative fuel other than natural gas, you must keep records of the total hours per calendar year that alternative fuel is burned and the total hours per calendar year that the unit operated during periods of gas curtailment or gas supply emergencies. [40 CFR 63.7555(h)]

7) The permittee shall record hours of operation for all boilers, total fuel use by boilers and sulfur content of the fuel oil, for each calendar month that fuel oil is stored on site.

8) If there is a period of natural gas curtailment or supply interruption, the permittee may burn No. 2 provided all curtailment provisions are meet according to 40 CFR 63.7545(f)(1) through (5).

C) Work Practices 1) By no later than January 31, 2016, each existing boiler must complete a one-time energy assessment, according to the

procedures specified in Table 3, Item 4 to 40 CFR 63 Subpart DDDD. [40 CFR 63.7510(e)] 2) The permittee shall meet the following requirements for tune-ups in order to demonstrate continuous compliance,

including the initial tune-up: a) Each required tune-up shall comply with the procedures in 40 CFR 63.7540(a)(10)(i) through (vi). You must

conduct the tune-up while burning the type of fuel that provided the majority of the heat input to the boiler or process heat over the 12 months prior to the tune-up. [40 CFR 63.7540(a)(10)-(12)]

b) If an affected unit is not operating on the required date for a tune-up, the tune-up must be conducted within 30 calendar days of startup.[40 CFR 63.7540(a)(13)]

c) For an affected unit that has not operated since the previous compliance demonstration and more than one year has passed since the previous compliance demonstration, you must complete a subsequent tune-up within 30 calendar days of startup. [40 CFR 63.7515(g)]

d) For each affected unit that has a continuous oxygen trim system that maintains an optimum air to fuel ratio, you must conduct a tune-up of the boiler every 5 years. You may delay the burner inspection specified in 40 CFR 63.7540(a)(10)(i) until the next scheduled or unscheduled unit shutdown, but you must inspect each burner at least once every 72 months. If an oxygen trim system is utilized on a unit without emission standards to reduce the tune-up frequency to once every 5 years, set the oxygen level no lower than the oxygen concentration measured during the most recent tune-up. Each 5-year tune-up must be conducted no more than 61 months after the previous tune-up. [40 CFR 63.7500(d) and (e), 40 CFR 63.7540(a)(12), 40 CFR 63.7515(d)]

3) For each new boiler, an initial 5-year tune-up must be completed no later than 61 months after the initial startup of the affected unit. [40 CFR 63.7510(g), 40 CFR 63.7515(d)]

4) When burning No. 2, during startup and shutdown the permittee shall follow the work practice standards in Items 5 and 6, as applicable. This requirement is met by operating any required continuous monitoring system (CMS) during startup and shutdown and by burning clean fuels during startup and shutdown, which includes natural gas or No. 2. [40 CFR 63.7500(f) and 40 CFR 63.7530(h)].

D) Initial Compliance Demonstration 1) The permittee must submit a Notification of Compliance Status containing the results of the initial compliance

demonstration according to 40 CFR 63.7545(e) within 60 days of the compliance date (date of startup). [40 CFR 63.7530(e) and (f)]

2) The following initial compliance demonstration requirements become applicable upon switching from burning natural gas to No. 2, as required by VII. B) Fuel Switching: a) Each boiler must demonstrate initial compliance with each emission limit by conducting initial performance tests

and establishing operating limits, according to 40 CFR 63.7520(b) and (c), and Tables 5 and 7 to 40 CFR 63 Subpart DDDDD. You must also install, operate, and maintain a CMS for monitoring boiler operating load according to 40 CFR 63.7525(d) and Item 5 to Table 7 to 40 CFR 63 Subpart DDDDD. [40 CFR 63.7510(a) and 40 CFR 63.7530(a)]

b) You must establish each site-specific operating limit in Table 4 to this subpart that applies to you according to the requirements in 40 CFR 63.7530, and Item 5 in Table 7 to 40 CFR 63 Subpart DDDDD and 40 CFR 63.7510(a)(4). The requirement to install an oxygen analyzer does not apply to a boiler that installs a continuous oxygen trim system. The continuous oxygen trim system must be set with the oxygen level no lower than the lowest average oxygen concentration measures during the most recent CO performance test as the operating limit according to Table 7 of Subpart DDDDD of part 63. [40 CFR 63.7510(a)(3) and (4), 40 CFR 63.7530(b), and 40 CFR 7525(a)(7)]

c) Your initial compliance demonstration for CO is to conduct a performance test for CO according to Table 5 to this subpart or conduct a performance evaluation of your continuous CO monitor, if applicable, according to 40 CFR 63.7525(a). [40 CFR 63.7510(c)]

d) Your initial compliance demonstration for PM is to conduct a performance test in accordance with 40 CFR 63.7520 and Table 5 to this subpart. [40 CFR 63.7510(d)]

E) Testing and Monitoring 1) When combusting No. 2 or natural gas, you do not need to conduct further performance tests (stack tests or fuel

analyses) if the pollutants measured during the initial compliance performance tests meet the emission limits in Tables 1 or 2 of 40 CFR 63 Subpart DDDDD, providing you demonstrate ongoing compliance with the emissions limits by monitoring and recording the type of fuel combusted on a monthly basis. [40 CFR 63.7515(h)]

2) The permittee shall install, operate, and maintain an oxygen trim system (as defined in 40 CFR 63.7575). The system shall be operated with the oxygen level set no lower than the lowest hourly average oxygen concentration measured during the most recent CO performance test as the operating limit for oxygen. [40 CFR 63.7525(a)(7)]

3) When burning No. 2, performance testing shall follow the stack test and procedures in 40 CFR 63.7520. Specifically, performance testing for filterable PM and CO shall be conducted according to Items 1 and 5, respectively, in Table 5 to 40 CFR 63 Subpart DDDDD. [40 CFR 63.7520]

4) When burning No. 2, the permittee must have a continuous monitoring system (CMS) used to monitored boiler operating load for each affected boiler. For each CMS, the permittee must develop a site-specific monitoring plan according to 40 CFR 63.7505(d). The CMS must be installed, operated, and maintained according 40 CFR 63.7525(d)(1) through (5) by the compliance date (upon startup). [40 CFR 63.7505(d)]

5) Monitoring data shall be collected and obtained according to 40 CFR 63.7535(a) through (d). [40 CFR 63.7535]

F) Recordkeeping 1) Records shall be maintained in a form suitable and readily available for AQMD review. [40 CFR 63.7560(a), 40 CFR

63.10(b)(1)] 2) The permittee must keep records for 5 years following the date of each occurrence, measurement, maintenance,

corrective action, report, or record. You must keep each record on site, or they must be accessible from on site, for at least 2 years after the date of each occurrence, measurement, maintenance, corrective action, report, or record. You can keep the records off site for the remaining 3 years. [40 CFR 63.7560(b)-(c), 40 CFR 63.10(b)(1)]

3) The permittee must keep copies of each notification and report that is submitted to AQMD or the administrator for compliance purposes with 40 CFR 63 Subpart DDDDD, including all documentation supporting any Initial Notification or Notification of Compliance Status or semiannual compliance report that you submitted, according to the requirements in 40 CFR 63.10(b)(2)(xiv). [40 CFR 63.7555(a)(1)]

4) The permittee must keep records of performance tests, fuel analyses, or other compliance demonstrations and performance evaluations as required in 40 CFR 63.10(b)(2)(viii). [40 CFR 63.7555(a)(2)]

5) For each CMS, the permittee must keep records according to 40 CFR 63.7555(b)(1) through (5). [40 CFR 63.7555(b)] 6) When burning No. 2, the permittee must keep the records of all monitoring data required by Item 10 in Table 8 to 40

CFR 63 Subpart DDDDD to show continuous compliance with each emission limit and operating limit. [40 CFR 63.7555(d)]

7) The permittee must keep records according to 40 CFR 63.7555(d), including records of: a) Monthly fuel use by boilers, including the type(s) of fuel and amount(s) used. [40 CFR 63.7555(d)(1)] b) The occurrence and duration of each malfunction of the boiler, or of the associated air pollution control and

monitoring equipment.[40 CFR 63.7555(d)(6)] c) Actions taken during periods of malfunction to minimize emissions in accordance with the general duty to

minimize emissions in 40 CFR 63.7500(a)(3), including corrective actions to restore the malfunctioning boiler, air pollution control, or monitoring equipment to its normal or usual manner of operation. [40 CFR 63.7555(d)(7)]

d) The calendar date, time, occurrence and duration of each startup and shutdown. [40 CFR 63.7555(d)(9)] e) The type(s) and amount(s) of fuels used during each startup and shutdown.[40 CFR 63.7555(d)(10)]

8) If you use an alternative fuel other than natural gas during a period of natural gas curtailment or supply interruption, you must keep records of the total hours per calendar year that alternative fuel is burned and the total hours per calendar year that the unit operated during periods of gas curtailment or gas supply emergencies. [40 CFR 63.7555(h)]

G) Reporting 1) The permittee must submit to the Administrator all of the notifications in 40 CFR 63.7(b) and (c), 63.8(e), (f)(4) and (6),

and 63.9(b) through (h) that apply to you by the dates specified. [40 CFR 63.7545(a)] 2) The permittee must submit each report in Table 9 to Subpart DDDDD of part 63 that applies to you. [40 CFR

63.7550(a)] 3) For the initial compliance demonstration for each boiler, the permittee must submit the Notification of Compliance

Status, before the close of business on the 60th day following the completion of all initial compliance demonstrations for all boilers at the facility according to 40 CFR 63.10(d)(2). The Notification of Compliance Status report must contain all the information specified in paragraphs 40 CFR 63.7545(e)(1) and (8) and must be submitted within 60 days of the compliance date specified at 40 CFR 63.7495(b). You must include with the Notification of Compliance Status a signed certification that either the energy assessment was completed according to Table 3 to Subpart DDDDD of 40 CFR part 63, and that the assessment is an accurate depiction of your facility at the time of the assessment, or that the maximum number of on-site technical hours specified in the definition of energy assessment applicable to the facility has been expended. [40 CFR 63.7545(e), 40 CFR 63.7530(e) and (f)]

4) The permittee must submit compliance reports as specified in 40 CFR 63.7550(b)(1) through (5). When burning natural gas and the units are subject only to a requirement to conduct an annual, biennial, or 5-year tune-up, and not subject to emission limits or operating limits, you may submit only an annual, biennial, or 5-year compliance report, as applicable, instead of a semi-annual compliance report. [40 CFR 63.7550(b)]

5) When switching to burning No. 2, the permittee shall submit a notification of compliance status according to 40 CFR

63.7545(e). [40 CFR 63.7545(e)]

6) If the permittee is required to conduct a performance test, a Notification of Intent to conduct a performance test must

be submitted 60 days before the performance test is schedule to begin. [40 CFR 63.7545(b).

7) For the initial compliance demonstration for each boiler utilizing No. 2 fuel, the permittee must submit all

performance test results in the Notification of Compliance Status according to 40 CFR 63.9(h)(2)(ii).

8) The permittee must report the results of performance tests and the associated fuel analyses within 60 days after the

completion of the performance tests. This report must also verify that the operating limits for each boiler or process

heater have not changed or provide documentation of revised operating limits established according to 40 CFR

63.7530 and Table 7 40 CFR 63 Subpart DDDDD, as applicable. The reports for all subsequent performance tests must

include all applicable information required in 40 CFR 63.7550. [40 CFR 63.7515(f)]

9) If a unit burning natural gas burns an alternative fuel during a period of natural gas curtailment or supply interruption,

you must submit a notification according to 40 CFR 63.7545(f)(1) through (5) within 48 hours of the declaration of

each period of natural gas curtailment or supply interruption, as defined in 40 CFR 63.7575.

10) A compliance report for units burning natural gas and only subject to the requirements of a tune-up must contain the

information in 40 CFR 63.7550(c)(5)(i) –(iii), (xiv) and (xvii). All other compliance reports must meet the requirements of 40

CFR 63.7550(c)(5)(i)-(xviii), as applicable. [40 CFR 63.7550(c)]

11) The permittee must submit compliance reports as specified in 40 CFR 63.7550(b)(1) through (5). For units that are subject only to a requirement to conduct an annual, biennial, or 5-year tune-up and not subject to emission limits or operating limits, you may submit only an annual, biennial, or 5-year compliance report, as applicable, instead of a semi-annual compliance report. [40 CFR 63.7550(b)]

12) You must submit each report in Table 9 to 40 CFR 63 Subpart DDDDD that applies to you. [40 CFR 63.7550(a)]

13) A compliance report for a facility subject to the requirements of a tune-up must contain the information in 40 CFR 63.7550(c)(5)(i) –(iii), (xiv) and (xvii). [40 CFR 63.7550(c)(1)]

14) Reports shall be submitted according to the procedures specified in 40 CFR 63.7550(h)(1) through (3) of this section. [40 CFR 63.7550(h)]

15) Deviations from emission or operating limits that occur shall be included in compliance reports according to 40 CFR

63.7550(d)(1)-(3) and (e)(1)-(9), as applicable. [40 CFR 63.7550(d) and (e) and District Regulation 020.075]

VIII. Rotogravure Cylinder Manufacture – Chrome Electroplating

A) Limitations 1) **Solvent containers used in the process must be covered whenever practical to minimize emissions.[District

Regulation 040.085] 2) Chromium emissions from the chrome plating tanks shall be controlled by a composite mesh-pad system. The

composite mesh-pad system shall be provided with adequate access for inspection. [40 CFR 63.342] 3) The composite mesh-pad system will be in operation when the plating tank is operating.

[PSD Permit # NSR4-7-2 NV 84] 4) The total emissions from the operation of the hard chromium plating system shall not exceed 0.015 mg/dscm (6.6 ×

10−6 gr/dscf) for all open surface hard chromium electroplating tanks that are existing affected sources and are located at small, hard chromium electroplating facilities. [40 CFR 63.342(c)(1)(ii)]

5) The total annual consumption of solvents in cylinder preparation operations shall be limited to no more than 13,000 gallons. [40 CFR 52.21]

6) The facility shall have and keep an operation and maintenance plan. [40 CFR 63.342(f)]

B) Monitoring 1) The pressure drop across the mesh and filter control system shall be between 0.4 and 4.4 inches of water (gauge). [40

CFR 52.21] 2) The owner of a composite mesh-pad system may repeat the performance test and establish as a new site-specific

operating parameter the pressure drop across the composite mesh-pad system according to the requirements in paragraphs (c)(1)(i) or (ii) of this section. To establish a new site-specific operating parameter for pressure drop, the owner or operator shall satisfy the requirements specified in paragraphs (c)(1)(iii)(A) through (D) of 40 CFR 63.343.

3) The operator shall track the solvent consumption of the cylinder preparation process and limit operations. [40 CFR 52.21]

C) Record Keeping 1) The operator shall record the pressure drop across the mesh and filter control system daily. [40 CFR 52.21]

D) Reporting 1) Pressure drop readings will be reported to the AQMD annually. [in 40 CFR 63 Subpart N]

IX. By-product paper collection systems

1) Emissions from the two (2) by-product paper collection systems and their cyclone based pollution control devices shall not exceed the applicable process weight based emission limitations of 12.5 pounds per hour. [District Regulation 040.020]

2) ** Emissions of particulate matter from the pneumatic paper collection systems shall be determined based on the records of annual throughput and an emission factor of 1.78 lb/ton of by-product paper processed through the cyclones. [District Regulation 040.010]

3) Visual observations of opacity will be performed and recorded not less than once per month. [District Regulation 040.010]

4) Visual emissions observations of opacity will be reported for monthly peak and period average on a Semi-Annual basis. [PSD Permit # NSR4-7-2, NV 84-01, 40]

X. Compliance Reporting & Certification of Compliance

A) Compliance Plan The Permittee is in compliance at the time of the issuance of this permit, the compliance plan consists of continued adherence to the requirements of this permit and those requirements set forth in applicable regulations and statutes. The permittee shall submit a statement of compliance as required in section 030.970(A). The permittee shall also submit a compliance schedule, certification and any other information as required to comply with the applicable provisions of 40 CFR 68. For the purposes of the compliance certification reports, the permittee shall use, at a minimum, the information obtained from the monitoring requirements of section VI of this permit. [District Regulation 030.970(A)]

B) Compliance Schedule The Permittee is in compliance at the time of the issuance of this permit, no schedule is required to attain compliance. [District Regulation 030.970(B)]

C) Semi-Annual Emissions Monitoring Report and Certification Statement In order to demonstrate compliance with the provisions of 40 CFR 70.6(a) (3), the permittee shall submit a report to the Control Officer of any required monitoring at least every 6 months. These reports shall be submitted by August 1 for the period of January 1 - June 30 and on February 1 for the period of July 1 - December 31. All instances of non-compliance with the permit shall be clearly identified. At a minimum, the report shall include the following:

1) A report of any start-up, shutdown or malfunction as required in 40 CFR 63.830(b) (5) of the Printing and Publishing MACT [40 CFR 63 Subpart KK]

2) A summary report of any emission exceedance as required in 40 CFR 63.830(b)(6) of the Printing and Publishing MACT [40 CFR 63 Subpart KK]

3) A summary report of liquid – liquid mass balance emissions results as calculated per 40 CFR 63.824(b)(1)(i)

4) A report of pressure drop observations across the Chrome plating exhaust emissions control system as require in 40 CFR 63 Subpart N.

5) A summary report of all opacity observation results required by condition VI. B. (2). 6) A certification of the truth, accuracy and completeness of the monitoring report statement by a responsible official,

consistent with 40 CFR 70.5(d).

D) Annual Emissions Report and Certification Statement In order to demonstrate compliance with the provisions of this permit, the permittee shall submit an Annual Emissions Report and Certification Statement by February 15th of each year describing the emissions of regulated pollutants for the previous calendar year, pursuant to Washoe County Regulations 030.219 and 030.970. The compliance statement shall address each term or condition of operation on an item by item basis. The compliance statement shall contain the following:

1) Identification of each term or condition of operation that is the basis for certification. 2) The compliance status of the facility with respect to each term or condition of operation and a statement that the

operator will continue to comply with such requirements. 3) Whether the compliance with each term or condition of operation was continuous or intermittent. 4) The method(s) used for determining the compliance status of the source including a description of monitoring, record

keeping and reporting requirements, and test methods. 5) For any term or condition of operation for which the source is not in compliance, the source must submit a compliance

schedule and a narrative description of how the source will achieve compliance with such requirements as described in 030.970 (B) of these regulations.

6) For any source for which the Control Officer has established requirements for progress reports on compliance schedules, the source must provide these progress reports in a timely manner and address dates milestones and other activities that have been specified by the Control Officer. The progress report shall also include the dates on which any milestones were achieved and an explanation of any dates or milestones that were not or are not expected to be met, why they were not met and any corrective actions to be taken as a result.

7) Any other specific information needed to determine compliance as required by the Control Officer. 8) A certification of the truth, accuracy and completeness of the compliance statement by a responsible official. 9) Any additional requirements as may be specified pursuant to sections 114(a) and 504(b) of the Act.

All Statements of compliance must be made in writing to both the EPA Administrator as well as to the Control Officer. Failure to submit a statement of compliance in a timely manner may result in citation, penalties, suspension or revocation of the Part 70 permit. [40 CFR 70.6 (c)(1); Clean Air Act (2014 Revisions) Section 182 (a)(3)(b) and Section 184 (b)(2)].

XI. General Obligations of Permittee [District Regulation 030.960]

1) The permittee shall comply with all terms and conditions of this permit. It shall not be a defense for a permittee in an

enforcement action that it would have been necessary to halt or reduce the permitted activity in order to maintain compliance with the conditions of this permit. In addition to compliance with the terms of the permit, the source shall be operated in a manner consistent with the operations described in the application for permit. [40 CFR 70.6 (a)(6)(i) and (ii)]

2) Any permit noncompliance constitutes a violation and is grounds for enforcement actions; for permit termination, revocation and reissuance, or modification; or for denial of a permit renewal application. Each day of non-compliance constitutes a separate violation. [40 CFR 70.6 (a)(6)(i)]

3) Upon change of control or ownership, the applicant shall notify any succeeding owner or operator of this facility of the

existence of this permit in writing, a copy of the notification shall be provided to the District. The terms and conditions of this permit shall be binding on all subsequent owners or operators. A request for an Administrative amendment shall be sought for changes in ownership or address of the facility. [40 CFR 70.7(d)(1)(iv); PSD Permit NSR 4-7-2 NV-84-01]

4) The permittee is authorized to make physical or operational changes consistent with the conditions of this permit

without applying for or obtaining a new permit or an amendment to this permit, provided that the emissions from the unit or group of units will not exceed the applicable regulations and/or permit limitations and prior notice (if required by District Regulation 030.950) is provided to the District. [40 CFR 70.4 (b) (12), Clean Air Act Section 502(b)(10)]

5) The permittee shall monitor for and record, any section 502(b)(10) change to the source, which is defined as a change

that would contravene an express permit term as described in section 030.950 (B). Notice must be given to the Air Quality Division and the EPA for any such change seven days in advance of enactment. The changes cannot be modifications under Title I of the CAA, and no permit shield can cover 502 (b) (10) changes. [40 CFR 70.4 (b) (12), Clean Air Act Section 502(b)(10)]

6) No permit revision shall be required for increases in emissions allowed under any approved economic incentives, marketable permits, emissions trading, or other similar programs or processes for changes that are provided for elsewhere in this permit and that are authorized by the applicable requirement. The permittee has requested and has been granted the right for trading emissions increases and decreases within the facility to the extent the applicable requirements provide for such trading and have been approved as tradable by the Control Officer. [40 CFR 70.6 (a)(8)]

7) The permittee must promptly report any event which results in any excess emissions or any other deviation from the

requirements of this permit, including those due to emergency upset conditions, in accordance with District regulation 020.075 and 020.076. The reporting timeframes shall be as specified in 020.075. [40 CFR 70.6 (a)(3)(iii)(B)]

8) Any application form, report, or compliance certification submitted pursuant to District Regulations shall contain

certification by a responsible official of truth, accuracy, and completeness and state that based on information and belief formed after reasonable inquiry, the statements and information in the document are true, accurate, and complete. See Attachment B. [40 CFR 70.5 (d) and 70.6(a)(3)(iii)(A)]

9) This permit may be terminated, suspended, reopened, amended for cause. Any request from the permittee for

revisions to the permit, planned non-compliance or other planned changes shall not stay the conditions of the permit. This permit must be reopened and revised if any of the following occur [40 CFR 70.6 (a)(6)(iii) and 70.7(f)(1)(i)-(iv)] a) Additional requirements become applicable to the equipment subject to this permit and three or more years

remain before the permit expires; b) Additional requirements become applicable to an affected source under the acid rain program; c) The Control Officer or the EPA Administrator determines that the permit contains a material mistake, inaccurate

statements were made in establishing the emission standards, other terms or conditions of the permit, or the permit must be revised or revoked to ensure compliance with the applicable requirements or District regulations.

d) The EPA Administrator finds that cause exists pursuant to District Regulation 030.960(D)(1)(a) or (b), or in response to a public petition.

10) The provisions of this permit are severable, and should any one or more be determined to be illegal or unenforceable,

the validity of the other provisions shall not be affected. The rights and obligations of the permittee shall be construed and enforced as if the permit did not contain the particular provision held to be invalid and the applicable requirements underlying these provisions shall remain in force. [40 CFR 70.6 (a)(5)]

11) This permit does not convey any property rights of any sort, or any exclusive privilege. This permit does not release the

permittee from the duty to comply with any requirements of any applicable statute, ordinance, code or law, including any Federal requirements, statues of the State of Nevada, County of Washoe, city ordinances or other legal requirements. [40 CFR 70.6 (a)(6)(iv)]

12) Failure to pay fines, fees or other approved charges from the District may result in enforcement action or termination

of the permit or both. All fines, fees or other approved charges are due and payable within 30 days of receipt of the invoice by the permittee. [40 CFR 70.6 (a)(7)]

13) The permittee shall submit a compliance statement signed by a responsible official due each year on the anniversary

of the date of issuance. The statement shall conform to the provisions of District Regulation 030.970(A). [40 CFR 70.6 (c)(3)]

14) Defenses to Enforcement Actions -

a) Need to Halt Activity - It shall not be a defense for a permittee in an enforcement action that it would have been necessary to halt or reduce activity to maintain compliance. [40 CFR 70.6 (a)(6)(ii)]

b) Emergency Upset, Breakdown or Scheduled Maintenance - An emergency upset or breakdown condition shall be an affirmative defense to an enforcement action only if the conditions set forth in District Regulation 020.075 are met. [40 CFR 70.6 (g)(3)(i) – (iv)]

15) **This permit shall be posted on or near the equipment listed above. This permit shall be readily available for

inspection at all times while the equipment is operating. [Regulation 030.240]

16) Any records of operation which will affect the potential of the source to emit air pollutants, such as fuel or products consumed, products produced, hours of operation, solvents or inks used in source operation must be maintained for a period of at least 5 years and made available to the control officer upon request. [40 CFR 70.6 (a)(3)(ii)(B)]

17) The permittee shall furnish, in a timely manner, any information or records requested by the Air Quality Division

pertaining to emissions compliance, production records, equipment and facility operations, or other air quality related information. [40 CFR 70.6 (a)(6)(v)]

18) Any request for confidential designation of records must be made in writing to Control Officer as specified in District

Regulation 020.055. [40 CFR 70.6 (a)(6)(v)]

19) The operator shall prepare and submit a risk management plan in compliance with the applicable sections of 40 CFR 68 on or before the date such plan is required. [40 CFR 68 Subpart A)]

20) Terms not otherwise defined in the permit shall have the meanings assigned to such terms in the referenced

regulations. [40 CFR 70.2 ]

21) **The permittee shall not knowingly falsify or render inaccurate any monitoring device or method of record required to be maintained or followed by this permit. [District Regulation 030.216]

22) The permittee shall comply with the provisions of District regulations 030.105 and 030.107 and 40 CFR Part 61,

Subpart M (Asbestos) during any renovation or demolition activity at this facility. [40 CFR Part 61, Subpart M (Asbestos)]

23) The permittee shall comply with the standards for recycling and emissions reduction pursuant to 40 CFR Part 82,

Subpart F Recycling and Emissions Reduction. [40 CFR Part 82, Subpart F]

24) Upon presentation of proper credentials and other documents as may be required by law and in accordance with constitutional limitations, the permittee shall allow the District, the Regional Administrator or any authorized representatives to perform the following [40 CFR 70.6 (c)(2)]: a) Enter upon the premises where an actual or potential source is located or where any records are kept as required

to demonstrate compliance with the terms and conditions of this permit and to have access to and copy, at reasonable times, these records;

b) Inspect at any time while the plant is open for business or in production any facilities, equipment (including monitoring and control equipment), practices or operations regulated or required under this permit;

c) Sample or monitor any substances or parameters at any location and obtain and remove samples of any discharge or emission of pollutants. The permittee may conduct parallel sampling for verification purposes.

XII. Permit Term This permit shall be valid for a period of five (5) years from the date of issuance. [40 CFR 70.6(a)(1) and District Regulation 030.910]

XIII. Permit Expiration and Renewal The right to operate terminates on the permit expiration date. If a written application for renewal of the permit is submitted at least six (6) months (but not more than 12 months) prior to expiration, the permittee may continue to operate equipment in accordance with the conditions of the previous Title V permit until final action is taken by the District on the renewal application. [District Regulation 030.9201]

XIV. Permit Modifications This permit may be modified to reflect changes in operation, procedure, ownership or other provisions as necessary. Any modification other than normal repair and maintenance, or a change which is not allowed under the operation of this permit or District regulation 030.950, shall require a revised or new Permit. If a new or revised permit is required, the operator shall make application to the Control Officer and follow the procedures as specified in District Regulation 030.950. The statute of limitations for construction of any equipment under the 1985 PSD Permit NSR 4-7-2 NV 84-01 has expired. No new equipment may be installed under the provisions of that permit. This restriction does not include normal repair and maintenance of existing equipment installed previous to the issuance of this permit as allowed under federal or District rules. Installation or construction of any new presses, including any offset presses, would require that Donnelly seek and obtain new pre-construction permits, and revision of the Title V permit, as applicable, depending on the nature of the proposed modification. [40 CFR 70.7(d) and (e) and District Regulation 030.950]
