Message from Dr. Nicola Mercer Medical Officer of Health & Chief Executive Officer

In 2011 we undertook the first year of our five year strategic plan which states that

“our primary focus is on the health and well-being of our communities.”

Public Health is made up of many dedicated staff and healthcare professionals who ensure our schools, daycares, nursing homes, workplaces, restaurants, tattoo parlours and swimming pools are safe public spaces.

At the same time we work to encourage individual health and wellness through health promotion, and disease and injury prevention.

As Ontarians become increasingly aware of the challenges facing the current healthcare system they are also aware of public health’s potential to alleviate some of those challenges. In fact, the recent Drummond Report noted that investing in health promotion programs is a way to help support the overall healthcare system.

Public Health is a leader in gathering research to understand the unique needs of each community. We work with local community groups to help everyone achieve their highest level of health regardless of income, education or social situation.

To have a sustainable healthcare system we have to reduce the demands on our current system by ensuring we live healthier lives in safer communities. Wellington-Dufferin-Guelph Public Health is helping build a healthier future.

Message from Amanda Rayburn Chair of the Board of Health

I am proud of the work of Public Health in 2011 and know that we will continue to be a

positive influence in the health of our neighbours.

As a business leader and community activist from the Orangeville area, I share a commitment to improving the health of the residents of our region.

You may not be aware of it, but WDG Public Health touches all of our daily lives as is evidenced in this Annual Report. The staff who work out of Fergus, Guelph, Orangeville, Shelburne and Mount Forest are dedicated to improving the health of everyone, including babies, children, teens, adults and seniors.

While moving forward with the daily work of public health, we’ve been preparing for the future health needs of our communities. It was essential in 2011 to address our physical space limitations for serving the public. We have identified locations in Orangeville and Guelph for new facilities and will begin construction in 2012. We are also renovating our clinics at the Shelldale Centre in Guelph to better serve our clients.

We will continue to meet the current needs of our clients as we position ourselves to meet the projected growth requirements of our region. Though this is a time of transition for WDG Public Health, it is also a time for renewal. We are committed to excellence in our work and healthy outcomes for individuals, families and our communities.

Public Health is... Using Evidence-Informed Practices

Wellington-Dufferin-Guelph Public Health ensures its programs and services are making

a difference by informing its practices with current, relevant and reliable evidence.

WDG Public Health is also committed to continuous improvement and evaluation of the effectiveness of its services.

In 2011, Public Health conducted research to assist with the evaluation of programs for health promotion and protection, reproductive health, child health, priority populations, healthy eating, physical activity and road safety.

The organizational knowledge that WDG Public Health is generating about public health services and their effectiveness is substantial. Employees have access to Public Health’s Evidence Warehouse which contains local and up-to-date research, statistics and reports. This evidence allows Public Health staff to identify best and promising practices and adapt to the changing needs of public health in the community.

Public Health is... Working to Ensure Everyone Has Equal Access to Good Health

Health starts in our homes, schools, workplaces

and communities.

. . . People in less-advantaged situations – whether measured by income, education, socio-economic status, gender, race, or other forms of social inequality and exclusion – have poorer health outcomes.... [What is needed is] improved data collection to adequately identify and assess disparities in exposure to risk factors and the burden of disease and for increased targeting of resources for health promotion initiatives for disadvantaged populations.

Taking action to prevent chronic disease: recommendations for a healthier Ontario, Cancer Care Ontario, Public Health Ontario

We often go to the doctor to get our health back, but the conditions we live and work in are impacting our health long before we ever call for a doctor’s appointment.

Health and well-being depend on social, economic and environmental factors as well as individual choices, living conditions and genetics. Simply put, the wealthier you are, the healthier you are. A 2011 study by the Public Health Agency of Canada confirms that more than half of all healthcare expenditures are linked to 20 percent of Canadians – those who live with the lowest income.

In 2011, WDG Public Health collaborated with the Region of Waterloo Public Health and Grey Bruce Public Health to produce Addressing Social Determinants of Health in the Waterloo-Wellington Local Health Integration Network (LHIN) Area. Funded by the LHIN, the study is accompanied by A Call to Action which challenges community groups to work together to address social inequities that contribute to poor health.

WDG Public Health has expanded the analysis throughout the entire Wellington-Dufferin-Guelph area to better understand how to work with community partners to implement programs and advocate for change on a neighbourhood basis. This evidence will inform Public Health and community leaders on what practices, programs and services can improve the health equity of everyone.

Public Health is... Developing Community Partnerships

Wellington-Dufferin-Guelph Public Health has strategic working relationships with partners

regarding health issues that impact the community. This team approach helps reduce or

eliminate challenges and barriers to health.

Children and Youth PartnershipsWDG Public Health collaborated with community service providers in a Wellington-Dufferin-Guelph Report Card Coalition to examine and report on the health, development and overall well-being of children and youth in our region. The coalition released its second report card The Well-Being of Children Ages 7 to 13: A Report Card for Wellington-Dufferin-Guelph. The Report Card provides key information for planning programs for children and youth in our communities.

Public Health is a member of the Dufferin Coalition for Kids (DuCK) which works with community partners to plan services for children from birth to 18 and their families. In 2011, DuCK obtained a grant from the Ministry of Children and Youth Services for educational sessions for professionals to promote collaboration and best ways to provide integrated services and support.

Healthy Communities PartnershipIn an effort to make a healthier choice an easier choice, Public Health participates in a Healthy Communities Partnership

which focused on creating policies in six key areas: healthy eating, physical activity, sports and recreation, injury prevention, mental health and tobacco/alcohol/substance misuse. The group hosted a workshop for 38 community partners about the policy development process and the need for meaningful engagement with local political leaders. The Healthy Communities Partnership works in collaboration with many groups including the Guelph Wellington Food Roundtable, WDG “in motion” and Headwaters Communities in Action.

Active Transportation PartnershipIn the summer of 2011, Public Health joined The Wellington County Active Transportation Committee with partners from all seven municipalities of the county. A partnership is currently working on an Active Transportation Plan to connect communities in Wellington County via walking and cycling routes for health, environmental, economic and social benefits. Extensive community consultation has occurred including a public survey of 650 people and a workshop that was attended by 30 local stakeholders. The partnership will report to municipal councils in 2012.

Public Health is... Working to Prevent Disease and Injury

Wellington-Dufferin-Guelph Public Health develops programs to reduce the burden

of preventable chronic diseases and injuries in the community. By addressing health

inequities that contribute to chronic disease, WDG Public Health improves the health

of the entire population.

In 2011, Public Health supported many individuals and families to reduce the frequency, severity and impact of preventable disease, injury and substance misuse.

Public Health Is Addressing Childhood ObesityObesity has long-term consequences for the individual and increases the demands on the healthcare system. Public Health is addressing the serious problem of childhood obesity with a physical activity program designed to ensure that healthy children become healthy adults. Working with community partners, Public Health delivers a program called “in motion Certified!” to children in elementary schools and childcare centres.

21 Elementary schools and 6 childcare centres joined the “in motion Certified!” physical activity program.

45 Schools participated in a physical activity program that encourages 60 minutes of daily exercise.

Public Health Is Helping Clients With NutritionPublic Health is helping food bank clients in Dufferin by providing workshops to assist with the challenge of meeting nutritional needs on a budget. The workshops are offered by trained volunteer Community Food Advisors with support from Public Health nutritionists.

Clients who attended workshops in 2011 learned about budgeting, cooking with food bank items, and serving low-cost healthy meals and snacks.

community groups.

Public Health Is Helping Schools With NutritionPublic Health is supporting school administrators, teachers and parents to provide healthier food options for children at school. The new Ministry of Education standards that came into effect on September 1, 2011 affect what food and beverages are being sold in schools. Public Health also provides nutrition information for teachers and students to use in the classroom.

37 Schools received nutrition information about the

Public Health Is Reducing the Harmful Effects of TobaccoPublic Health is working with communities to reduce exposure to smoking which is known to be the leading cause of preventable death. Public Health conducted a survey in Orangeville that showed there was widespread support for a ban on smoking in public places. As a result, a new smoking bylaw in the Town of Orangeville will mean that children and adults will not be exposed to the harmful effects of second-hand smoke at playgrounds and around town-owned and operated buildings.

Public Health Is Educating Students About Drinking and DrivingAlcohol is linked to motor vehicle collisions and increasing awareness in youth is an important strategy of Public Health. Public Health has designed a program to reduce the rates of death and injury in young drivers. In 2011, students at three high schools participated in the program which involves navigating an indoor course on a PlasmaCar while wearing goggles that simulate impairment. Participants, many not yet old enough to drive, saw first-hand the dangers of distracted and impaired driving.

Public Health Is Preventing Falls in Older Adults A fall for a senior can be debilitating and result in a drastic change of life. Public Health is partnering with local agencies to provide in-home exercise programs designed to reduce the incidence of falls in isolated older adults in Wellington County.

Public Health Is Supporting Workplace Wellness The first initiative of a new Public Health workplace wellness program is a smoking cessation program offered to employees of workplaces in the community. The program has been designed to reduce chronic disease associated with the use of tobacco. With funding from the Waterloo Wellington Cancer Prevention and Early Detection Network, WDG Public Health is offering nicotine replacement therapy and on-site counseling.

Public Health Is Involved in Road Safety

WDG Public Health and Guelph Police Service are partnering in an effort to improve the

safety of roads in Guelph. Public Health created a Community Road Safety Survey which

identified that many residents are passionate about road safety. In September 2011,

Public Health was one of 34 representatives across a variety of sectors that met to

discuss road safety and assess interest in future collaboration on the topic of road safety.

1,816 Families received a telephone call after their baby’s birth.

254 Families were referred to the Healthy Babies Healthy Children home visiting program with a total of 2,456 visits made in 2011.

Public Health is... Supporting Your Family’s Health

In collaboration with community partners, Wellington-

Dufferin-Guelph Public Health supports individuals and

families to live healthy and safe lives.

A wide range of reproductive health programs are designed to support with pregnancy and the birth of a healthy baby. Child health and dental programs enable all children to develop to their full potential and achieve optimal health.

Public Health Is Online for Prenatal SupportWDG Public Health launched its online prenatal program for pregnant women and their partners. The program makes it easier for women on bed-rest or with schedules that do not allow them to attend prenatal education classes. Public Health was able to provide prenatal support to 35 pregnant women who would otherwise have been unable to attend prenatal classes.

Public Health Is Screening Women for High-Risk PregnanciesPrebirth Clinics provide an opportunity for women and their partners to meet one-on-one with a Public Health nurse as early as 12 weeks into pregnancy to speak about a healthy pregnancy, parenting, and community services and supports.

At Prebirth Clinics, Public Health nurses also screen clients for risk factors that could affect a woman’s health and baby’s development. Appropriate referrals are offered to provide additional support to pregnant women and their families.

The Canada Prenatal Nutrition Program aims to improve the health of mothers and infants, reduce the incidence of unhealthy birth weights, and promote and support breastfeeding by building partnerships and strengthening community support for pregnant women.

In collaboration with the County of Dufferin, Public Health conducted five workshops for Ontario Early Years Centre (OEYC) staff from Wellington, Dufferin and Guelph who work with families who have young children. The workshops provided current, research-based information on a broad range of topics including positive parenting, nutrition, mental health, immunization and safety. The group continues to be supported by a Public Health nurse.

Public Health Is Supporting BreastfeedingPublic Health produced a video Breastfeed Anytime, Anywhere and a documentary Celebration of Stories used in hospitals featuring local women and Public Health nurses. The video was picked up by YouTube and North American television stations.

538 Adults and 139 teenagers received support through the Canada Prenatal Nutrition Program.

210 Couples attended breastfeeding classes.

1,334 Clients attended Prebirth Clinics at five locations.

Public Health is... Supporting Growing Children

Wellington-Dufferin-Guelph Public Health’s program Healthy Babies Healthy Children offers

education and support for families in our communities.

Public Health nurses also provide professional advice and support on many parenting topics. WDG Public Health received 4,100 calls on KIDS LINE (1-800-265-7293 ext.3616), a phone service for families who need professional advice.

Soon after birth, a family receives a phone call from a Public Health nurse to assess their comfort with newborn care and breastfeeding, and to assess maternal health and well-being. Some families receive a home visit after birth for additional support.

Long-term visiting is provided for families facing complex challenges such as a child with special needs, isolation, insecure access to food or housing, mental illness or family violence.

Public Health is the lead agency for a locally delivered preschool speech and language program called Wee Talk. The services include language assessment, speech therapy, supporting parents with speech at home and helping with transition to school.

Public Health provides oral health screenings for children in elementary schools to ensure good dental health. Children are screened for urgent dental conditions and many receive preventative dental services like fluoride applications. Public Health refers children for financial assistance through two provincially funded programs: Healthy Smiles Ontario and Children in Need of Treatment.

Public Health is... Preventing Infectious Diseases

WDG Public Health inspectors and nurses are

working in the community to keep the public safe

from vaccine preventable diseases, sexually

transmitted and blood-borne infections, tuberculosis,

rabies and other infectious diseases.

Public Health receives phone calls on the Outbreaks and Infectious Disease Reporting Line, investigates and confirms cases of infectious disease and controls outbreaks.

To keep the community safe, Public Health is continuously educating professionals and the public about disease prevention. In 2011, Public Health provided more than 40 workshops for healthcare workers, operators of personal service settings, group home staff and residents, daycare staff, high school students, parents and volunteers on a variety of topics including: outbreak management, disinfection and cleaning, bed bugs and common germs.

1,030 Calls were received and followed up on the disease

reporting line.

161 Infection control workshops were given to healthcare workers.

423 Confirmed infectious diseases were investigated.

3,076 HPV vaccinations were given to grade eight females.

4,758 Doses of Hepatitis B vaccine were given to grade seven students.

681 Follow-ups were completed for diagnosed sexually transmitted infections.

40 Intestinal outbreaks, 17 respiratory outbreaks and 10 influenza outbreaks in long-term care and retirement homes were investigated.

19 Investigations of public complaints were followed up regarding infection control in personal service settings.

125 Clients were assessed and treated in a tuberculosis clinic.

105,000 Needles were distributed through the Harm Reduction program.

Wellington-Dufferin-Guelph Public Health reduces the burden of illness from health

hazards in the physical environment including air quality, extreme heat and diseases

like West Nile Virus.

In conjunction with the Ministry of the Environment, WDG Public Health surveys and monitors soil and air quality. Public Health also ensures the public has access to safe food and water.

Public Health is... Prepared for Public Emergencies

Wellington-Dufferin-Guelph Public Health collaborates with partners in all municipalities

to ensure an appropriate and effective response to any potential threats to public health.

WDG Public Health has a comprehensive emergency preparedness plan that includes ongoing communication, training and emergency exercises.

Public Health Is Responding to Public EmergenciesPublic Health and partnering agencies responded to the emergent threat of forest fires that impacted all 204 residents of Sandy Lake, a First Nations community in northern Ontario in July 2011. Staff of WDG Public Health assisted with the initiation of an emergency response that included hosting the First Nations community in the County of Wellington. Public Health also provided advice about infection control and food safety practices in the Evacuation Centre.

Following the evacuation, a report from the Ministry of Health and Long-Term Care included recommendations from WDG Public Health about the role of Public Health nurses in evacuation centres.

Public Health is... Preparing to Protect and Support the Health of Growing and Changing Communities

In 2011, Wellington-Dufferin-Guelph Public Health implemented the first year of a new

five-year strategic Plan called Moving Ahead.

WDG Public Health is planning for significant growth in the population of our service area, including meeting the needs of an aging population.

Our challenge is to build the additional infrastructure to provide the programming required to meet those needs.

WDG Public Health conducted independent space needs assessments to fulfill our responsibilities as a modern healthcare provider and as the lead agency to respond to any potential public health emergency. We inspected both municipally owned and commercial properties to consider the option of renting, but it was clear that the most cost effective way to proceed would be to build new facilities that would meet our space requirements for the foreseeable future.

In Guelph, Public Health’s new facilities will be centrally located on Chancellor’s Way near the University of Guelph.

Clients will benefit from a centralized location easily accessible from anywhere in the city. The building will be designed to accommodate a large number of people in the event of expanded immunization clinics or an emergency. The Guelph facility is scheduled to open at the end of 2013.

In Orangeville, Public Health’s new facilities will be fully accessible and take into account the heritage landscape of the downtown area. The Orangeville facility is slated for completion at the end of 2012.

Renovations are being made to Public Health clinics at the Shelldale Centre in Guelph to expand the services provided at this location. These renovations will be completed by the summer of 2012.

A 2009 study by Douglas Manuel and others revealed that British Columbia, which spends almost three times as much per capita on public health as Ontario, is the leading province in terms of overall population health and health behaviours (including quitting smoking, engaging in regular physical activity, choosing a healthy diet and maintaining a healthy body weight). This apparent correlation between public health spending and health outcomes needs to be further explored through research to determine the benefit-cost ratios.

Don Drummond, Commission on the Reform of Ontario’s Public Services (The Drummond Report)

2011 Financial Summary

Below is a synopsis of the organization’s finances at the end of 2011.

RevenueThe funding of Wellington-Dufferin-Guelph Public Health is varied and complex. The largest portions of funding come from the province of Ontario, the Ministry of Health and Long-Term Care (which now includes the former Ministry of Health Promotion and Sport) and the Ministry of Children and Youth. The counties of Wellington and Dufferin, and the City of Guelph share the next largest portion of funding. Funding is also received from the federal government through Health Canada and through grants from community partners. The total revenue in 2011 was $21,987,011.
