This is a sample of the Lender Record Information (Form 582) application. The sample lender name, Black Pearl Mortgages (LE), will be replaced with your company name when you login and edit your Form 582. The sample blank form

is annotated with helpful notes in to guide you in filling the form.


Note: Based on the answer selected, some questions may require comments.

The section cannot be verified if required comments are not entered.


Note: In Roles field, all mandatory roles should be covered by the

contacts reported. A maximum of four roles can be entered per

Contact with the exception of Owner/Principal role that can be added

as extra role for reporting ownership Interest on “Ownership Interest”

section without having to reenter the contact details later.

“Verify this response” only becomes enabled once all the mandatory

roles have been covered through the reported contacts.


Note: If there are no approved affiliates to report, just click “Verify this response”

Note: If “Yes” is selected then any individuals (i.e. Contacts) already reported with Owner/Principal role are automatically shown in the list. Edit

and enter the ownership interest for individuals where the ownership percentage value is > 5%.

You may click “Add another ownership interest” to report any new individuals or firms. This will open the form as shown on next page.

Note: Selecting the checkbox above will update the address for all the servicer 9-digit accounts

of the same “Servicing Type” i.e. Subserviced servicers in the example above.

respective policy, procedure, or process.
