TREATMENT AUTHORIZATION I authorize the physician, school nurse, or other health professional to render necessary medical care to my child named above. NETWORK APPROPRIATE USE AGREEMENT

We are pleased to offer campers of St. Margaret’s Summer Camp access to our computer network. To gain access to the internet, all students under the age of 18 must obtain parental permission and must sign and return this form to camp. Access to the internet will enable campers to explore thousands of libraries, databases, and bulletin boards while exchanging messages with other users throughout the world. Families should be aware that some material accessible via the internet may contain items that are illegal, defamatory, inaccurate, or potentially offensive to some people. Internet access is regulated by a network security appliance. This blocks access to sites that are known to be inappropriate for our campers. While our intent is to make internet access available to further educational goals and objectives, campers may find ways to access other materials as well. We believe that the benefits to campers from the access to the internet, in the form of information and communication resources and opportunities for collaboration, exceed any disadvantages. But ultimately, parents/guardians of minors are responsible for setting and conveying the standards that their child should follow when using media and information sources. ST. MARGARET’S SUMMER CAMP TECHNOLOGY CODE OF CONDUCT A. PRIVILEGES

Each member of the St. Margaret’s Summer Camp has the opportunity to access the St. Margaret’s network to facilitate personal growth in technology, information seeking skills, electronic research skills, and communication skills.

Each member of the St. Margaret’s Summer Camp has the privilege to access and retrieve information from the internet. This includes access to the World Wide Web.

Each member of the St. Margaret’s Summer Camp has the privilege to access network groups, list serve subscriptions, and real-time two-way communication over the internet.

B. RESPONSIBILITIES

As a member of the St. Margaret’s Summer Camp:

You will use the network and individual computer workstations or other electronic devices as educational resources and take responsibility for all material sent or received under your accounts.

You will monitor and control all material sent or received via the internet accessed under your accounts.

You will immediately contact a staff member when your account security or integrity has been compromised.

Parents and campers should also be familiar with and abide by other policies, as applicable (such as the Cell Phone Policy and Use Agreement).

PERSONAL SAFETY

You will not post personal contact information about yourself to other people. Personal contact information includes your address, telephone, camp address, etc.

You will not agree to meet with someone you have met online without your parent/guardian’s approval.

You will promptly disclose to a staff member any message you receive that is inappropriate or makes you feel uncomfortable.

ILLEGAL ACTIVITIES

You will not attempt to gain unauthorized access to St. Margaret’s computer network or any other computer system through the St. Margaret’s computer network or go beyond your authorized access. This includes attempting to log in through another person’s account or access another person’s files. These actions are illegal, even if only for the purpose of browsing.

You will not deliberate attempts to disrupt the computer system or destroy data by spreading computer viruses or by any other means. These actions are illegal.

You will not use St. Margaret’s computer network to access material that is profane or obscene, that advocates illegal acts, or that advocates violence or discrimination toward other people (hate literature).

You will not use the St. Margaret’s computer network to engage in conduct that endangers the health, safety, or reputation of yourself or other members of the camp.

SYSTEM SECURITY

You are responsible for your own individual accounts and should take all reasonable precautions to prevent others from being able to use your accounts. Under no condition should you provide your passwords to another person.

You will immediately notify a staff member if you have identified a possible security problem.

INAPPROPRIATE LANGUAGE

Restrictions against Inappropriate Language apply to public messages, private messages, and material posted on Web pages.

You will not use obscene, profane, lewd, vulgar, rude, inflammatory, threatening, or disrespectful language.

You will not engage in personal attacks, including prejudicial or discriminatory attacks.

You will not harass another person. Harassment is persistently acting in a manner that distresses or annoys another person. If you are told by another person to stop sending him/her messages, you must stop.

You will not knowingly or recklessly post false or defamatory information about a person or an organization.

RESPECT FOR PRIVACY

You will not repost a message that was sent to you privately without permission of the person who sent you that message.

You will not post private information about another person.

PLAGIARISM AND COPYRIGHT INFRINGEMENT

You will not plagiarize and will carefully document material taken from the internet or other electronic resources. These sources should be treated just as any other conventional printed source and referenced appropriately if used in your written works. Do not plagiarize these sources.

You will respect copyright software and will not load such software on any of the camp’s computers via a network or any other sources. No copyrighted software may be copied and/or removed from the camp’s computers under any circumstances.

By signing this contract, I understand that the network manager may review all audits (computer generated records of your network use) created by the Novell software or any other audit trail created by the access to the network. The network manager will be responsible for determining and investigating inappropriate usage of the network and is responsible for informing camp staff and campers when appropriate.

Further, I understand that the network manager or designee has the right to review any information stored on the fileserver without approval or consent from the user. The network manager or designee does not have the right to review information stored on an individually owned electronic device (i.e., a personal laptop computer or tablet), unless inappropriate use of the device and/or network is suspected. The user will provide such device, whenever possible, with an opportunity to be present with the network manager if a review/audit of an individual electronic device or storage device is necessary. Notice of such an audit may be given as shortly as immediately prior to the audit. No prior approval or consent is needed from the user before an audit is conducted under circumstances in which inappropriate use is suspected.

CELL PHONE POLICY & USE AGREEMENT

Having ready access to a cell phone will always be a family’s decision, even if the camper has the opportunity to have her cell phone at camp under St. Margaret’s guidelines. To have her cell phone, the camper and her parent(s)/guardian(s) must sign this acceptable use agreement and the cell phone must be registered with the camp office during check in. This agreement sets out the policy for use as explained below. Designated times are scheduled where campers may use their phones. Campers may use their phones:

Monday – Friday 1:00pm – 2:00pm during Down time

Monday – Friday 9:00pm – 9:45pm during Down time

Saturday and Sunday as scheduled between camp activities Cell phones must be turned in each night before Lights out. All cell phones will be stored in the camp office located in the residence hall during non use hours. A camper’s cell phone privilege may be suspended if she does not remain in good social standing with the camp. A staff member may confiscate a cell phone if he or she believes it is interfering with our community. The cell phone’s return will be contingent upon a conversation with the Camp Coordinator or her counselor. The camp reserves the right to review items stored on any cell phone, including text messages, and images when inappropriate use is suspected. If you bring a cell phone to camp, please consider the impact on our community and your camp experience. Phone use should be consistent with our camp expectations which are based on cooperation with a consideration for all members of our camp. Your use should show respect for the person receiving your message, for those in your company, and for those sharing common spaces with you. If you have any doubt about how you are about to use your phone, ask yourself: Is my anticipated use of the phone reasonable, responsible, and right? I have read and understand the above cell phone policy. I understand that should I be seen, heard, or found using my phone in any manner where or when I should not be using it, I will lose my cell phone privilege as outlined above. “Use” of a cell phone includes but is not limited to placing or receiving a call; sending, receiving, or reading a text message; taking

St. Margaret's Summer Camps 2017 Water Activities Waiver and Release of Liability

In consideration of my child being allowed to participate in any way in St. Margaret's Summer Camps, the undersigned acknowledges, appreciates, and agrees that:

1. The risk of injury from activities involved in this program are significant, including potential for permanent paralysis and death, and while particular rules and equipment, and personal discipline many reduce this risk, the risk of serious injury does exist; and,

2. I knowingly and freely assume all such risk, both known and unknown, even if arising from the negligence of the releases or other and assume full responsibility for my child’s participation; and,

3. I willingly agree hereby release and hold harmless St. Margaret's Summer Camps, their staff, other agencies, sponsors, and other participants, and if owners and leasers of premises and equipment used to conduct the activities, with respect to any and all injury, disability, death, and loss or damage to person or property, whether arising from the negligence of the releases or otherwise.

4. I understand this waiver can and may be used for all water events that my child participates in this summer.

I have read this Water Activities Waiver and Release of Liability agreement and fully understand its terms. I also understand that I have given up substantial rights by signing it, and sign it freely and voluntarily without inducement. St. Margaret’s Summer Camp Course Descriptions Art Join professional artists in our studio to draw, paint, and create! This course offers the freedom to explore artistic interest through medium, technique, interpretation, and reflection. Basketball This all-skills course will motivate campers to strengthen their existing fundamentals as well as learn more about basic team concepts and systems. Skill-based drills for ball control, shooting, rebounding, and footwork in addition to 3 vs. 3 and 5 vs. 5 wave competitive drills will be used in this high pace training environment. Campers will apply these skills to game situations during daily tournament style competition while also learning the importance of team play. Bay Studies* The Chesapeake Bay is a national treasure: the biggest, most fertile, and most wildlife-rich coastal body of water in the United States. Our own Rappahannock River – here at St. Margaret’s – is the healthiest, most pristine large river in the Chesapeake tributary system. This class will introduce students to the ecology and natural history of these special ecosystems. The great American outdoors will be our classroom as much as the indoors, with a series of field studies to beautiful habitats by boat, kayak, paddleboard, foot, and perhaps even bicycle. The classroom component will be hands-on and inquiry-driven, centered on a sequence of engaging laboratory investigations where campers carry out experiments of their own design and cultivate scientific process skills such as random sampling, statistical analysis, and graphing with computer software. *Bay Studies is only offered during Session 1 (June 18 – July 1) and at the additional cost of $150. This course must be taken as your Activity #1 and Activity #2 as you will receive school credit. Coding A future in coding begins here. This introduction to programming starts with the building blocks of a computer—the hardware. Then, it's all about the 1s and 0s, discovering how coded instructions are executed directly by a computer's CPU. Building on this knowledge, campers will learn object-oriented programming concepts with JavaScript. They'll dive into fun challenges throughout the session while developing real-world programming skills. Math Small groups, personal attention, and a collaborative approach to learning will ensure that every camper masters fundamental concepts in their level of math. When signing up for this course, please list the last math course completed (Spring 2017). Music & Drama Drama at camp is all about collaboration, the creative process, and having fun. Improvisation and the exploration of expressive performance are what guide this course – from theater games to poetry to skits, and of course, music! This course will conclude with a short performance for all our campers to enjoy.

Next Steps This course will help campers prepare for the total college process. During this course, campers spend time learning about a variety of college campuses: big/small, public/private, urban/rural, coed/all-women’s, research/liberal arts, highly selective and more welcoming. Each Next Stepper will participate in several on-campus workshops: assessing college and career interests, debunking common college myths, creating an academic resume and sample college application, mastering the college essay, sharpening SAT test taking skills, and participating in mock admission interviews and committee review. Reading & Writing for College This course gives campers a deeper appreciation and greater command of the written and spoken word—which are absolutely critical for success in college and career. Reading & Writing for College allows students to gain a solid foundation in reading, writing, critical thinking, and oral expression. Literature in a variety of genres by both traditional and contemporary writers invites students to explore universal themes. When signing up for this course, please list the last English course completed (Spring 2017). Soccer This all-skills course will motivate campers to strengthen their existing fundamentals. Campers will be given the opportunity to receive group and individual instruction. Each day is filled with a variety of drills and small-sided games to promote correct soccer techniques and teamwork. Skill-based drills for ball control, passing, and shooting will be used in this high pace training environment. Softball This all-skills course will motivate campers to strengthen their existing offensive and defensive fundamentals. Campers will be challenged each day with new drills and techniques. We will cover fundamental skill work during this course, but the majority of our time will be spent on developing more advanced skills to bring your hitting, fielding, and/or pitching to the next level. Spanish This course focuses on the fundamental elements of the Spanish language within a cultural context. Emphasis is on the progressive development of all four language skills: listening, speaking, reading, and writing. When signing up for this course, please list the last Spanish course completed (Spring 2017). Volleyball This all-skills course will motivate campers to strengthen their existing fundamentals as well as learn more about basic team concepts and systems. Skill-based drills for serving, passing, blocking, and hitting in addition to 3 vs. 3 and 6 vs. 6 wave competitive drills will be used in this high pace training environment. Campers will apply these skills to game situations during daily tournament style competition while also learning the importance of team play. Water Sports Does wake-boarding, waterskiing, tubing, knee-boarding, and fishing peak your interest? If so, Water Sports is the activity for you! St. Margaret’s is a mere 20 yards from the water, and Water Sports takes full advantage of all our perfect location. This course gives campers the chance to enjoy all the fun recreational activities the river has to offer.

CAMP INFORMATION 2017 Thank you for enrolling your daughter in the St. Margaret’s Summer Camp. The following information should answer a number of questions concerning Summer Camp. CAMP OVERVIEW St. Margaret’s Summer Camp offers academic and non-academic activities in a traditional camp environment where girls learn what it means to belong to a community, believe in themselves, and become leaders. Embracing the mission and philosophy of St. Margaret’s School, this overnight summer camp for girls 12-18 is designed to meet the needs and interests of each individual. On the banks of the Rappahannock River, campers will learn how to balance working hard and playing hard through the discovery of new talents, sharpening of skills, and building lifelong friendships. REGISTRATION AND PAYMENT Enrollment begins on Monday, December 19, 2016, at 8:00am. Please be aware that we will not be able to confirm your registration until all paperwork is properly completed. We require a $500 deposit to secure a spot for St. Margaret’s Summer Camp. The remaining balance is due no later than April 1, 2017. Acceptable forms of payment include cash, check, VISA or MasterCard. All payments are non-refundable. Register in person at St. Margaret’s School (444 Water Lane, Tappahannock, Virginia 22560) Monday – Friday 8:00am – 4:00pm or visit the school website to register online at www.sms.org/camp. CANCELLATIONS A partial refund will be given if your daughter is sent home due to illness with verification from the camper’s physician. Refunds are not issued in cases when the camper is homesick, sent home for negative behavior, or elects to voluntarily withdraw from camp after arrival. HOURS OF OPERATION Session #1: Sunday, June 18: Check In 1:00pm – 3:00pm and Saturday, July 1: Check Out 9:00am – 12:00noon Session #2: Sunday, July 16: Check In 1:00pm – 3:00pm and Saturday, July 29: Check Out 9:00am – 12:00noon CAMPER INFORMATION We will need a copy of your daughter’s birth certificate and physical exam before your registration can be complete. Have your doctor use the Health Information Form provided. Physicals must take place within the last 12 months. NO CAMPER WILL BE ADMITTED WITHOUT A COMPLETE HEALTH INFORMATION FORM.

CAMPER HEALTH AND SAFETY At St. Margaret’s Summer Camp, camper safety is our first priority. All staff members undergo intensive training on how to prevent and handle a variety of emergency situations. If an emergency does occur, the safety of campers will be secured and the appropriate authorities and parents/guardians notified. It is essential that we have accurate information for the parents/guardians as well as a separate emergency contact. If your camper becomes ill during camp, you or (in case you cannot be reached) your emergency contact will be notified. If your camper needs to be treated immediately, she will be taken to our school physician in Tappahannock. Health insurance coverage is the sole responsibility of the camper’s parent/guardian. MEDICATION The infirmary provides some non-prescription medications, such as Tylenol and cough drops, to be administered as needed. Medications from home must be given to the Camp Coordinator upon arrival so that we can properly store them. They need to be in the original container with the label intact and child’s name. All medications are locked safely in our infirmary and dispensed by our Camp Coordinator. TRANSPORTATION OPTIONS On the first and last day of each camp session, St. Margaret’s will provide transportation to and from Richmond and Washington, D.C. Parents should contact the Camp Coordinator to arrange transportation at least three weeks prior to the first day of camp. There is an additional cost for transportation to and from camp: Richmond International Airport (RIC) and Richmond Amtrak Staples Mill Road (RVR) is $150 round-trip, Washington Dulles International Airport (IAD) and Reagan Washington National Airport (DCA) is $250 round-trip. This charge will be added to your balance. Scheduling Related to Travel Campers should plan arrivals no later than 1:00pm in Washington, D.C. and no later than 3:00pm in Richmond. Campers should plan departures no later than 3:00pm in Washington, D.C. and no later than 1:00pm in Richmond. CHECK IN Check in is from 1:00pm – 3:00pm the first day of camp. Our buildings will not open until 1:00pm. Staff will assist you with parking. The check in process will take about 15 minutes and will include room assignments and completion of any missing paperwork. Rooms are assigned by grade so be sure that information is up to date and accurate at the time of check in. We encourage you to help your daughter get settled in before the Opening Assembly at 4:00pm. Eat a good lunch before coming to camp. We will serve campers dinner at 5:00pm, so they need to be content through the afternoon.

TYPICAL DAY AT CAMP Ask anyone who has been to camp and they’ll tell you that there isn’t a “typical” day at camp. Each day brings new and exciting experiences. But, there is a general pattern you can expect. 7:30am: Good morning 8:00am: Breakfast 9:00am: Activity #1 12:00noon: Lunch 1:00pm: Down time 2:00pm: Activity #2 5:00pm: Dinner 6:00pm: Activity #3 8:00pm: Color wars 9:00pm: Down time 10:00pm: Lights out Part of your camper’s registration is to choose the activities she wants to do for each of the activity blocks. Our website has all activities listed with a brief description of each. Please contact our Camp Coordinator with any questions regarding these options. Don’t be fooled by “Down time.” This is when some of the best friendships are built. There has been significant research that shows this unstructured time is vital to girls’ social development. Campers must be in their residence hall during this time. They can hang out in their rooms or the hall community space. Blue and Grey are not only St. Margaret’s colors, they also represent one of our oldest and most spirited traditions. When you come to St. Margaret’s, you will become one, or the other, for life. “Color wars” is a designated time where our girls have the opportunity to be a part of a team, showing their spirit and supporting teammates in this friendly camp rivalry. At the end of camp, the winning team will be declared. Camp is like one big family and to make it run smoothly, we ask that everyone help out. Each day we have room clean up and basic camp clean up. We want campers to understand that part of living in a cooperative community means that everyone has to do their part. TUTORING Tutoring services are available to campers in exchange for payment in full of the additional tutoring fee(s). Please see the Tutoring Agreement for more information.

WEEKENDS AT CAMP Weekends at camp vary from the daily schedule. On Saturday, we will take all campers off campus for an all-day field trip. Session 1(June 18 – July 1) will go to Kings Dominion in Doswell; Session 2 (July 16 – July 29) will go to Water Country USA in Williamsburg. On Sunday, we will enjoy a relaxing day on the river. Activities will include fishing, swimming, kayaking, paddle boarding, and boat rides. Land activities will include volleyball, corn hole, and other yard games. We will end our Sunday with a cookout and movie in the pool. MEALS AND NUTRITION Breakfast, lunch, and dinner are served for all campers and counselors at regular times in our dining room. When the girls come for their first meal, they will draw a number. This number correlates to a table. We pick new tables each week as it is a great way to mix up the campers so that they get to meet even more people! Our menus are made up of delicious recipes planned with hungry growing girls in mind. The food is primarily made in-house and includes fresh vegetables and fruit whenever possible. There is always a sandwich station available at each meal and a salad bar at lunch and dinner. Sample Menu for a Day Breakfast: Scrambled eggs, sausage biscuits, and fresh fruit Lunch: Chicken tenders, onion rings, and steamed broccoli Dinner: Hamburgers, baked beans, and french fries Dessert: Chocolate Cake If your camper is a vegetarian or has allergies that prevent her from eating certain foods (such as wheat, peanuts, milk, etc.), please let us know prior to the start of camp. We will make every effort possible to accommodate these situations but ask that you also provide some food to increase your camper’s comfort at meals. Campers should not remove food, beverages, silverware, or dishware from the dining room. The on-campus store will be open at various times for campers to purchase snacks between meals. LOST AND FOUND Please label EVERYTHING! This helps us return items that are found. Make sure your daughter leaves with everything she brought to camp. All items left behind without a clear indication of to whom they belong will be donated to a local charity after August 29, 2017. St. Margaret’s Summer Camp is not responsible for any lost or damaged items. PACKING FOR CAMP A Packing Checklist is available to assist your daughter in packing for camp. It would be helpful to pack a separate backpack in addition to your suitcase or duffle bag. This will come in handy each day to carry certain belongings.

Please remember that clothes and gear can get dirty, wet, well-worn, damaged, and even lost at camp. DO NOT send anything of great value or sentimental attachment. St. Margaret’s Summer Camp is not responsible for any lost or damaged items. St. Margaret’s Summer Camp has a zero tolerance for weapons, alcohol, tobacco, e-cigarettes, vaporizer pens, or any other illegal substance. If this policy is violated, campers will be sent home and no refund will be given. LAUNDRY Washing machines and dryers for campers to use are located in the basement of their residence hall. Campers can do their laundry free of charge but are expected to bring their own laundry detergent. FAMILY CODE OF CONDUCT ST. MARGARET’S EXPECTS ALL CAMPERS, PARENTS, COUNSELORS, GUESTS, AND OTHER MEMBERS OF CAMP TO BE RESPECTFUL AND CONSIDERATE TO EVERYONE AT ALL TIMES.

Appropriate behavior promotes inclusion rather than exclusion, collaborative problem-solving, seeing mistakes as opportunities

for growth, and striving for proper conflict resolution. Camper conduct violations will be handled by the Camp Coordinator depending on the severity and frequency. Each infraction will be reviewed and dealt with fairly and effectively. Communication and cooperation between parents and St. Margaret’s is essential to a community code of conduct. All infractions and consequences will be handled with appropriate confidentiality. Campers who misbehave will be corrected. Opportunities to learn from mistakes include a loss of privileges, a parent conference, suspension, and expulsion. Continued poor conduct following a parent conference may result in ineligibility for participation in camps at St. Margaret’s. We believe that a positive and constructive working relationship between the camp staff and the camper’s parents/guardians is essential to the fulfillment of St. Margaret’s mission. Therefore, St. Margaret’s reserves the right not to continue enrollment or not to re-enroll the camper if the camp reasonably concludes that the actions of the parent/guardian make such a positive and constructive relationship impossible or seriously interfere with the camp’s accomplishment of its educational purposes. St. Margaret’s decision in these matters shall be final. VISITOR POLICY Campers are not allowed to have visitors during camp. Having to readjust to camp following a visit is difficult, especially for a homesick camper. Plan to have your daughter be your “tour guide” on closing day when she can share her favorite places with you.
