rectication o" PSC*/&S* deciencie% -ill be reduced/eli$inated re%ulting in
operational efcienc( o" CO+PA12 .

3. The Third Ofcer %hould under%tand that there $ight be a gap bet-een the
actual %tatu% o" the LSA/&&A and the re'uire$ent% a% per the Record o"
appro!ed cargo %hip %a"et( e'uip$ent certicate B5<.
*t i% thi% gap -hich need% to be identied and rectied in the ne0t #5 da(% to
# $onth %o that the LSA/&&A on board can be $aintained to top %tandard.
5. The Third Ofcer $u%t take the LSA/&&A plan and the Record o" appro!ed
cargo %hip %a"et( e'uip$ent certicate and co$pare the actual LSA/&&A on
board.
a. The gap% -hich are identied $u%t be recorded6 co$pared -ith deli!er(
challan%/in!oice% and then di%cu%%ed -ith Chie" Ofcer and +a%ter.
b. Once it i% %ure the identied gap% either %hortage or e0ce%% or -rong
e'uip$ent< do e0i%t the%e $u%t be reported to ofce.
c. 8hen the indi!idual e'uip$ent i% checked it $u%t be co$pared -ith
$anu"acturer $anual6 SOLAS6 and LSA/&&A code to en%ure correct ite$%
are on board.
d. Once the gap% are di%cu%%ed and a nal %tatu% on the$ agreed then
during the ne0t !i%it b( Cla%% %ur!e(or $u%t be !eried and endor%ed b(Cla%% %ur!e(or on the LSA/&&A plan and Record o" appro!ed cargo %hip
%a"et( e'uip$ent certicate %o that the%e docu$ent% are $o%t updated
and late%t "ro$ -hich "uture check% on board can be undertaken.
=. 7eadline to co$plete the check% and re%tore the correct %tatu% o" LSA/&&A
-ithin #5 da(% to # $onth ti$e. 7uring thi% period dail( report% $u%t be
%ub$itted to CO+PA12 and Chie" Ofcer and +a%ter $u%t !eri"( the report

be"ore %ending. *" an( e0ten%ion to deadline i% re'uired then %a$e can be
re'ue%ted but on a ca%e b( ca%e ba%i%.

F. *t $u%t be en%ured that the detail% and $aintenance %chedule o" the LSA/&&A
ite$% a% per &lag State re'uire$ent% $u%t be entered in the %hip?% S++S
progra$ to en%ure proper record keeping and "ollo- up.
#.The LSA/&&A $anual%6 79E,@A Red Book< and the LSA/&&A chart $u%t be
properl( lled and $aintained.
##.Other %ta) $u%t be trained and retrained b( taking up LSA/&&A ite$% a"ter
drill% or a"ter %a"et( $eeting%. Training o" %ta) i% !er( i$portant becau%e it
-ill en%ure that proper care o" LSA/&&A i% undertaken on board and their
$i%u%e i% pre!ented.
#,.8hen the ti$e "or %hore ba%ed $aintenance co$e% it $u%t be realiDed that it
i% !er( critical ti$e during -hich i" the LSA/&&A ite$% are not care"ull(
re$o!ed -hen being %ent "or %er!icing and placed back a"ter return "ro$
%er!ice there i% high chance that again the( $ight be -rongl( placed in
po%ition or returned not properl( %er!iced. Third Ofcer $u%t care"ull( look
a"ter the entire operation and en%ure that LSA/&&A %tatu% i% -ell re%tored a"ter
the %hore %er!icing.
#.8hen %hore %er!icing i% co$pleted6 the Third Ofcer together -ith $a%ter
!eri"( that all ite$% are returned properl( and %er!icing ha% been done. The
%hore %er!ice certicate% are correct and no error% e0i%t. *" there i% an( doubt
on the certicate %a$e $u%t be !eried -ith %er!ice pro!ider and ofce
i$$ediatel( prior the !e%%el lea!e% the port %o that re%pon%ibilit( can be
e%tabli%hed -ith the %er!ice co$pan(.
#3.*" an( ite$ i% replaced a"ter the %er!ice due to it% e0pir( or "ailure then thedetail% o" the ne- ite$ $u%t be care"ull( entered in the LSA/&&A plan and

LSA/&&A plan and Record o" appro!ed cargo %hip %a"et( e'uip$ent certicate

#5.8hen re'ui%ition% are to be rai%ed "or LSA/&&A ite$% it $u%t be en%ured that
the re'ui%ition i% ba%ed on the actual ite$ re'uired detail% to be taken "ro$LSA/&&A plan and Record o" appro!ed cargo %hip %a"et( e'uip$ent and the
ite$ on %ite<. 7o not onl( rel( on pre!iou% re'ui%ition -hich can be incorrect.
Check the ite$% i$$ediatel( -hen recei!ed and i" %till unu%able due to
-rong %uppl( b( %upplier then it $u%t be returned and ofce updated
i$$ediatel(.
&. Li%t o" all LSA/&&A ite$% on -hich training %hould be i$parted to attendee% in
CO+PA12 Training Centre. .5 da( #. da( training %hould be i$parted to
