Not surprisingly, insulin manipulation is the most common way in which young people with type 1 diabetes seek to control their weight; however, many of these will not qualify as having an eating disorder. Where an eat- ing disorder is diagnosed, bulimia or EDNOS (eating disorder not otherwise specified) rather than anorexia nervosa are the most common. 2 Unfortunately, the success rates for treatment of eat- ing disorders are lower in people with diabetes com- pared with non-diabetic clients. 3 Referral can often be delayed and accessing a specialist service may be a chal- lenge in itself. These disorders are, by nature, secretive and this paper by Susan Hart and colleagues highlights the difficulty of identifying them in the diabetic clinic at an early stage. The paper offers a valuable insight into the challenges faced by the in-patient eating disorder team trying to manage these patients with type 1 dia- betes on a day-to-day basis. It also offers some useful management strategies. It is clearly essential that the two specialist teams (eating disorder and diabetes) work closely together and keep communicating. The ‘control’ needed for diabetes management will be at odds with the need for patients to be less restrictive with their food intake as part of their eating disorder treatment pro- gramme. Eating disorder staff may not be regularly deal- ing with diabetes management and likewise the diabetes care team may not fully appreciate the manipulation of food and insulin which occurs in this client group. The prevalence of eating disorders in patients with type 1 diabetes is significant and this implies that these patients are under the noses not only of those working in hospitals, as outlined in the paper, but also of com- munity teams. Naturally, understanding and empathy will improve care offered by specialist teams, but those not working within this context will also need to have an awareness of the kinds of issues this paper raises. The next few years will witness the introduction of a series of novel technologies to augment existing thera- pies for the management of type 1 and type 2 diabetes. Over the years much has been made of advances in insulin technology; however, with the possible exception of reduced hypo incidence the benefits offered to dia- betes care may have been more limited than has been suggested. Novel approaches to caring for people with diabetes, and perhaps equally importantly preventing the pandemic of type 2 diabetes the world is witnessing, are urgently needed. In his excellent article, Professor Nauck reviews GLP-1 and the incretin mimetics. This novel therapeutic class will become available in the USA later in 2005 and most probably in Europe in 2007. In offering the poten- tial to intervene in the natural history of type 2 diabetes at an early stage these agents open up exciting possibili- ties. For instance, the prospect of gaining glycaemic con- trol without the weight gain often seen at present is attrac- tive, particularly when associated with no risk of hypogly- caemia. The potential for future molecular manipulation to render the therapy long acting also brings the prospect of weekly or even monthly dosing regimens. No new therapy comes without caveats however. As a gut hormone which reduces gastric motility, the effects of GLP-1 therapy on the significant group of people with diabetes who have sub-clinical gastroparesis are unknown. The effects of altered gastric emptying on the pharmacokinetics of other concomitant therapies are also uncertain.
