disabling disorders and injuries by physical means

When you are looking for short term rehabilitation for yourself or a loved one, there are a number of things to consider. Here are some of the more important things to look for when searching for the right place:

1) Reputation: There are many facilities with stellar reputations for providing great care and good results. While you should not base your decision solely on this status, it is something to consider. And a personal referral, a recommendation from a friend or family member is also a reliable source.

2) Facility Tour and Equipment Available: If you are able, take an unannounced tour of the places you are considering. Get a feel for staff, physical surroundings and the atmosphere. Does it have the equipment you will need for rehab?

3) Is the program age-appropriate? Rehab for an 80 year old with a broken hip should be a little less intense and different than rehab for a 30 year old with a broken hip.

4) Location, Location, Location: Successful rehab is much more likely if friends and family are able to visit frequently to provide support and receive training as needed. Choosing a quality location as close to home as possible will positively impact your rehabilitation.

5) Staff: When touring a facility, talk to some of the staff–nurses, therapists, social workers etc. Ask questions and get a feel for the people who work there. A caring, dedicated, friendly staff will work hard to make your rehab successful.

Take time to choose the right rehab facility for you whether you are recovering from a stroke or from a hip replacement. Your choice of rehab is as important as your choice of a doctor.

My wish for you during the upcoming holidays is one of love and giving.

This is the season of giving… so I suggest you GIVE of yourself. There are many ways to do that. It isn’t always a pretty boxed gift…sometimes it’s a hug or a few

minutes of your time sitting and listening to someone else. Taking an elderly person out for lunch or babysitting for a single mom who

doesn’t get time to herself is another option. I’m sure you can think of many other ways to give of yourself. Put down the Candy Crush and think of someone who can use a special gift – a gift of your time. We in Central PA, America are rich in so many ways…and we can be richer with our friends by giving them a present of ourselves.

I watched a “You Tube” the other day…It was called LandFillHarmonic because in this one community they make instruments made out of trash and have created an orchestra of young people. It’s a slum built on a landfi ll and yet wonderful sounds come from their efforts. One girl said, “My life would be worthless without music.” Can you imagine? I’m guessing that those people are giving of themselves.

I would like to take this opportunity and say THANK YOU to our readers and our contributors. It’s music to my ears and a special gift from you when I hear stories about how one of our journal stories helped someone make a new decision or take a new path. That’s awesome and I’m thankful women continue to give and receive those ‘gifts’ that help them grow both professionally and personally.

It’s the Most Wonderful Time of the YearGiven the rising cost of higher

education, if you would prefer to save more for college, 529 Savings Plans have become very popular. 529 plans permit an owner to retain control of the assets inside the plan and name the benefi ciary for the account which is usually the future college student. The owner can even change the benefi ciary within the same family. The contribution limitations are limited to the annual gift amount, which is $14,000.

If your children are grown, there are other opportunities to assist them. You can open or contribute to a Roth IRA or traditional IRA in the name of a child. Since your children may be fi nancially busy saving for a house, paying for college or a new car, they may be unable to max out their retirement. Your children not only get the power of your gift later in retirement, but they may also get a tax deduction on an IRA contribution.

Many accounts allow for multiple donors to contribute to the same account. Grandma, Uncle and Dad can all contribute to the same account and make it a family holiday tradition. There are many pros, cons and restrictions to investing for a minor or an adult child. Consult with a fi nancial advisor to fi nd the best fi t for you and your whole family.

As I write this, it is the beginning of October and the leaves are changing color. Tomorrow, the forecast calls for 80 degree weather. Even as early as this, before Halloween or Thanksgiving, I spotted Christmas items at Macy’s last week. Given that this article will be published in November and December in the ‘heat’ of the holiday season, I am going to

concentrate on the gifts that keep on giving.

I know many children that have too much ‘stuff.’ The toys overfl ow the house and some sit idle after Christmas morning. My suggestion is to cool it with the toys. No, I am not suggesting becoming Scrooge. I am suggesting that you gift something that will be worth more as your children grow older rather than a toy that may be left behind rather quickly. Split your gift amount between the here and now and their future. The opportunity to add money to a child’s account once a year or more (birthdays, Christmas, etc.) can pay large dividends. Forgo some of the ‘stuff’ and invest for their college, future home or wedding instead.

Investing for a child when they are young will also teach a powerful lesson in priorities. Minors follow your example, so while providing them with a brighter future, you are also illustrating that investing and saving for the future is a priority.

Gifting to a minor can be done through a Uniform Gift to Minor’s Account (UGMA). An adult (not necessarily the donor) must be named the custodian on this account and will retain investing authority and oversee account use for the child until the minor reaches the age of majority, which is age 21 in Pennsylvania. The downside is that the money is used at the “child’s” discretion once they reach the age of 21 and take over the account.

If the child is likely to go to a private school, there is another good option. A Coverdell Education Savings Account (ESA) can be used for qualifi ed expenses for elementary, secondary or higher education school expenses. The maximum annual contribution is $2,000 and the ability to contribute is limited by the parents’ adjusted gross income (AGI).

By Kelly William Biggs, M.D.October is Breast Cancer Awareness Month. As the Radiologist for the

Breast Cancer and Women’s Health Institute of Central Pennsylvania, I have a passion for fi ghting breast cancer. It is a fundamental mission of radiologists nationwide to detect breast cancers as early as possible.

The screening mammogram, the annual ritual in which women undergo a low dose X-ray of each breast, is a simple, fast, and relatively inexpensive tool that physicians have used for decades to save lives. Even though it is not new technology, no other means come close to being its equal in detection of cancer. Mammography is associated with a 30 percent reduction in breast cancer deaths.

I’m writing to champion screening mammograms because breast cancer is sadly way too common. Chances are you know someone with breast cancer. Around 300,000 women in the U.S. are diagnosed with breast cancer each year, and about 40,000 will die annually from breast cancer.

I also want to emphasize a very important point. The American Cancer Society, among several other esteemed national organizations, recommends that women who are at average risk for breast cancer begin annual screening mammography at age 40.

Currently, there is a lot of confusion about mammography, most stemming from the US Preventive Services Task Force (USPSTF). This advisory body, although not consisting of a single breast surgeon, oncologist, or radiologist--that is, anyone who is an expert in breast cancer detection and treatment—decided in 2009 that it would be best for women in their 40s to skip mammography, and for older women to undergo screening every two years instead of one.

These recommendations are hard to accept, especially when one in six of women diagnosed with breast cancer is in her 40s. A recent Harvard Medical School study determined that half of all breast cancer deaths occurred in women under age 50, and over 70% of these deaths occurred among women who did not participate in screening. Other studies suggest that getting a mammogram every other year instead of yearly may result in a 30% increase in missed cancers.

Are You Due For Your Screening Mammogram?These 2009 USPSTF recommendations have been discounted by

breast cancer specialists, and thankfully insurance carriers including Medicare continue to reimburse based on the American Cancer Society recommendations. But erroneous or misleading information is still out there. As recently as this past summer a national nightly news healthcare spokesman espoused the USPSTF recommendations as though they were valid. Perhaps even more alarming, Wikipedia’s mammography article referenced by so many uses a highly questionable European pamphlet as its

primary resource—a pamphlet that for all practical purposes equates breast cancer to athlete’s foot in severity of disease. I hope that most readers who encounter that write-up know how dangerous its arguments are.

Despite the success screening mammography has achieved, it is not perfect. Mammograms can miss breast cancers, and the sensitivity of mammography is reduced for women with very dense breast tissue. I highly recommend that women be vigilant about responding to changes they experience in their bodies. Talk to your doctor about healthy habits that can reduce your risk of breast cancer. While it is diffi cult to fi nd strong scientifi c data to support the monthly self breast assessment, I personally support it. Many women have detected lumps or tumors through the self breast assessment. Some women with a strong family history of breast cancer or with other risk factors may benefi t from supplemental screening with breast MRI. Although solid standards have not been set, some women may also benefi t from supplemental surveillance with breast ultrasound.

Be vigilant, but keep in mind that mammography continues to be the mainstay. All the great new or newer technologies, whether they are MRI, 3-D imaging, automated ultrasound, etc., remain just adjuncts to our core screening standard, the mammogram.

What can/can’t I bring to the event?This event is designed specifically towards the wedding market. Anything you used for

your ceremony, reception, rehersal...anything purchased for your wedding/wedding events is welcome. You should be very conscience of the items you bring and the condition they are in! If you are renting a table and selling wedding items you may also try to sell your wedding gown. Items that are not wedding related, wedding themed or irrelevant to weddings (i.e. shampoo, laptops, car chargers, toothbrushes, jeans, shorts, etc.) will not be permitted. Organizer of the event has the right to remove table from the event and ask that all items are removed from event.

What if I only want to try to sell my wedding gown? Can I?Yes! This will be listed under the Bridal Gown Consignment section. If you would like for us to try to

sell your gown- you will need to drop your gown off at Diamonds & Lace Bridal starting Thursday Nov. 7 until 5pm and by Saturday Nov. 9 at 5pm. Gowns must be clearly tagged with your name, designer, size, asking price, color, phone and email. You will be required to fill out a disclaimer when dropping off your gown. No gowns will be excepted prior to Nov. 7th. If we are able to consign your dress, The Bridal Swap charges 20% of what your gown sold for. And you keep 80% of the sales! This is a very inexpensive way to consign your gown! Most consignments take much much more in commission!

How should I price my items that I have to sell?Look at it this way. The items you have to sell have been used.

Though they may have only been used once...they still have been. So regardless that you paid full retail...when the item was new....you shouldn’t expect to receive the full retail price back. Unless you can state that it is NEW WITH TAGS. To start, you should at least start with marking them half for what you paid for them. You want the “Brides to Be” to feel encouraged to buy from you because of your qualilty item and your price. As the day goes on and you feel like you want to offer deeper discounts...go for it!! It’s your table with your items!! You’re in control! Just don’t overprice items that brides to be can get somewhere else for the same price.

A working mother of two children, Lynne Force, 41, of Salladasburg, is always on the go. When her monthly periods became so painful that she sometimes felt like crawling under her desk at work, the surgical technologist went to Dr. Angela Huggler at Susquehanna Health OB/GYN.

Lynne’s condition was caused by scarring resulting from a cone biopsy, a test she had to check for cancerous cells on her cervix. The scarring at the opening of her uterus made it contract like she was in labor. Dr. Huggler recommended a hysterectomy to remove Lynne’s cervix and uterus. The impact of this complex surgery would be minimized with the assistance of robotic technology through Susquehanna Health’s new da Vinci® Surgical System.

Lynne has some experience with traditional open abdominal surgery—that’s how her appendix was removed, and she delivered one of her children by Cesarean section. Each open procedure took about six weeks for recovery and a long incision that needed to be guarded against infection. Recalling the pain and lack of energy and appetite that dragged on for several weeks following those surgeries, Lynne opted for the robotic assisted procedure.

“I have heard a little about it [robotic assisted surgery], and I thought it was great. I knew it would get me back to work sooner,” explains Lynne.

An alternative to both traditional open and conventional laparoscopic surgery, robotic-assisted surgery became available at Susquehanna Health in May 2013. A microscopic camera and surgical tools are inserted through three tiny incisions in the same manner as

are back at work before a patient who had a traditional hysterectomy would even be out of the hospital.”

Lynne was back on the job just two weeks after her surgery. She is pleased with her results and no longer experiences monthly pain and bleeding. She says the preparation and post-operative recovery were much like her other surgeries, but she noticed a difference in her overall recovery right away.

“This procedure was definitely less painful, and I bounced back a lot quicker with my appetite and my activity,” says Lynne. “Dr. Huggler is very experienced and very talented. The whole robotics team was just wonderful.”

The technology has radically changed very delicate, complex gynecologic surgeries including hysterectomy and oophrectomy (removal of the ovaries). It can also be used for removal of ovarian cysts and removal of fibroids from the uterine wall.

Dr. Huggler says the visualization combined with the control and dexterity afforded by the robotic instruments enables surgeons to be very precise and avoid other delicate internal structures like muscles and nerves. Following surgery, patients have less internal and external scarring, less pain and can generally recover more quickly.

“Years ago, women could be hospitalized three days before a hysterectomy and remain for as many as three to five days following the procedure,” explains Dr. Huggler. “Today women arrive on the day of surgery and can be discharged the same day. Some of my patients

My name is Jackie. I live in State College, Pennsylvania and I’m a professor at a Uni-versity. I learned about Blair Plastic Sur-gery from a friend because I wanted to take care of my skin. Before this, I never, ever

did anything; didn’t use skin creams or even wash my face half the time. The (skin care) staff helped me as they taught me about my skin and the prod-ucts I needed to use. I also had a series of three Matrix week-end peels done with Dr. Fanny. They were incredible and really improved my skin. It (Matrix) ended up being a very easy procedure. I’ve learned some very good practices and now I get microdermabrasions every 6 weeks. This really maintains

my skin in good condition.

I had been thinking about a facelift since I started coming here (Blair Plastic Surgery) 6 years ago. I had a very saggy, crêpie neck. I didn’t mind getting older, but I just didn’t want to look haggard, and it seemed that everything was starting to sag on my face. When I looked in the mirror I would say, “This is not who I am. I don’t feel like that person.” I felt so much more youthful than what I looked like in the mirror, it just wasn’t me. It looked like somebody much older and tired and I felt like I had a lot more pep than that.

I was not growing gracefully with age and I wanted to have a some-thing done but I felt that there was never the time. I finally got a sab-batical and was able to get the opportunity to do it (facelift). What I found out was that I could have probably done it sooner because it re-ally took a much shorter time to heal than I had ever expected. And I also had my eyes (upper lids) done! The first week of course, you have to take care of yourself but within 2 weeks I was rested and ready! It was not a hard surgery at all.

Dr. Louton was incredible. He was very patient with me, very kind and considerate. Dr. Louton treated me as a professional adult and someone he respected. He prepared me and answered all my questions. And I had a good many questions! He was very good humored about it.

About 3 weeks after the surgery, I had to go a meeting and saw some of my old students and some of the faculty I had been working with and it was very interesting, I felt sure that they would say, “Did you do something?” but what they said was “You look so great! The sab-batical must have really relaxed you.” Later on, I met a former student

at a wedding who said how good I looked and how rested I looked so I confided in her. She was shocked, “I would have never dreamed! It looks so natural.” So it was still me, but it was a less tired looking me.

I’m 66 years old and I feel great! It’s a huge difference for me when I look in the mir-ror, but it’s also a natural look and I’m so happy about that. I didn’t think it was possible to look this much better! I didn’t really want to look young, I just wanted to look better and I feel like that’s what happened. I am really, really happy with what I had done. It’s made a big dif-ference in my life. Everyone that works at Blair Plastic Sur-gery has been fantastic to me. Marvin Kohler, 54, creates unique art from rebar, or reinforcing bar, using only one arm/hand. His passion for this new skill came as a surprise to his family and friends, after recovering from a stroke. The Kohler’s sell his pieces for others to enjoy, and to create funds for the materials Marvin needs to keep creating. Marvin Kohler, 54, can’t speak; he uses an electronic communication device to type words, or his wife Crystine speaks for him. He also has no use of his right arm. However, despite diffi cult physical limitations due to a stroke, Marvin is an artist. His medium is rebar (short for reinforcing bar), a steel bar commonly used as a tension device in reinforced concrete and masonry structures.

“Before the stroke, if Marvin had a creative side, nobody knew it,” says Crystine, his wife of 25 years.

Crystine recounts the moments leading up to Marvin’s stroke, four years ago: “Marvin drove a truck for a living. The Friday night before his stroke, he had gotten home late and was tired. Saturday morning, I took a group of 23 family members to Dutch Wonderland. Marvin opted to stay home. That afternoon, one of Marvin’s friends called me and said, ‘I was just talking to Marvin on the phone and I think that he had a stroke; you had better come home.’”

Marvin’s friend had recognized Marvin’s slurred speech as a sign of stroke. Later, Crystine found out that somehow after talking to his friend, Marvin had managed to call an emergency number and an ambulance arrived at his house and took him to the Lewistown Hospital helipad to be life-fl ighted to Geisinger in Danville.

“After the stroke, Marvin could not walk, he could not use his right arm, and he could swallow,” says Crystine.

When he was discharged from Geisinger, he went to HealthSouth Nittany Valley Rehabilitation Hospital, where he had three hours of therapy (physical, occupational and speech) a day. He was an inpatient for four weeks. Marvin left HealthSouth with a wheelchair and a cane,

Life After Stroke Awakens a Man’s New Passion and continued outpatient therapy at HealthSouth’s Lewistown Clinic. “Now, he doesn’t even use his cane,” Crystine adds. “We have a lot of gratitude for HealthSouth and the dedicated staff who helped Marvin with his rehabilitation.”

But life after a stroke was not easy for Marvin. “For the fi rst four years, he sat in a chair and did nothing. I even got him a dog, which I had hoped would help with his depression.”

Then something changed. One day this past July, Crystine came home to fi nd a plant hanger in the yard; she didn’t know how it got there. “The next day, another one showed up,” says Crystine. “I asked Marvin if he made it, and nodded that he did.” Marvin had used the rebar material that was in his garage to make the plant hanger.

Since then, Marvin creates practically all day, every day, according to Crystine. “He draws it and then he makes it. And he creates it all with one hand.”

“Recently one of our grandson’s told him, ‘I want a choo-choo’ and two days later it was done,” she adds. Marvin also made an angel in honor of a son who passed 23 years ago, and a grandson who died at birth.

“Thanks to his art, Marvin has a passion for living again,” says his wife. “I could not ask for anything better.”

The decision to separate is hard, but the reality of what separation means can be harder. One of the most important aspects of separating is privacy. Here are fi ve tips I have for protecting your privacy:

1. If you are staying in your home, change the locks as soon as your spouse moves out. Reprogram the alarm system to a new code and inform them to not let your spouse reprogram it again. Get the garage door opener back and reprogram the garage code. Make sure that you have all the keys to the vehicles that you drive.

2. If you move, change all necessary addresses at post offi ce or if mailbox security is a concern consider a PO Box. Also, be sure to contact all the credit card companies, phone carriers, banks, etc. with your change of address.

3. Immediately change ALL your passwords; your email, shopping sites, business, retirement and bank accounts, credit cards, etc. Don’t forget to change the password on electronic devices. If your husband is the cell phone owner or manages the account, you should consider switching to a new plan or consulting your carrier to see how private you can make the information. Not only calls made, but texts as well, may be open to access by the account holder. This is also the time to consider whether or not your spouse has put spyware on your equipment or GPS on your vehicle. Also, be sure to change your privacy settings if you use social media like Facebook or Twitter.

4. Have separate bank accounts and use a different banking institution.

Get your own credit card. If there is a problem with your spouse abusing credit cards, be sure to formally remove your name from all of the joint accounts and clarify that you will not be liable for future charges. And run a credit report on yourself to make sure there aren’t any nasty surprises.

5. Even your private home may not be so private. Kids can stumble across divorce information. Your spouse comes into the house to pick them up for a visit. Designate a spot to keep your divorce fi les and put them in a locked fi ling cabinet. Never leave computers on with your email account open.

Change is in the AirAs the balmy days of summer fade into the blustery days ahead it is a good time to refl ect on the past year and begin to plan for the coming year. It is also a good time

to prepare your home for the winter and see where you can save money while staying warmer.

Interest rates and prices have started a long gradual climbs that will continue to trend up in the years ahead. Conditions continue to be good and many of the properties out on the market at this time of the year are willing to negotiate more than in the summer. If you are fi nancially able to purchase real estate this could be an excellent time to take advantage of conditions.

Keep in mind there are changes in the mortgage and insurance industries that may impact availability and conditions for mortgages in 2014. Generally speaking, as a buyer you need to be prepared for an increased need for documentation and fi scal responsibility. Availability of mortgages should remain at the levels they are now, or even increase, but as a borrower you will have to provide more verifi cation of income, etc… and do a good job as avoiding spontaneous purchases (do not open up a new credit card account!).

Perhaps the biggest change will be how properties with newly designated fl ood plains are being handled by insurance companies.

Properties that will require fl ood insurance are expected to see a marked increase in the premiums and costs for new owners that could negatively impact the salability of the land. Talk to your local Realtor for further information on how this could affect you.

Finally, take a few minutes and evaluate your home to see what you can do to make it more effi cient for the winter. There are hundreds of suggestions ranging from weather-stripping doorways to adding extra heat sources. The investment of some time now can save you lots of money in the future. If you want to get a thorough evaluation of your property consider having an energy audit done by a certifi ed professional.

Directing Education for Susquehanna Valley LPN StudentsLEWISBURG – Carol Barbarich, MSN, RN, is the director of the Susquehanna LPN Career Center, which became part of the CSIU in 1983. She has worked with the program since 1974, when it was with Danville’s Geisinger School of Nursing. She now operates the LPN center to assure educational quality, school stability, and operational compliance with the state board of nursing and national nursing education accreditation standards. � e center has two part-time training programs and one full-time program each year, graduating from 70 to 100 LPNs annually. Carol received her master’s degree in March 2006. She’s been married to Jim for nearly 37 years and has two grown children and two grand-children.

She guides young students to make wise decisions as they transition into the workforce, higher education, or military service. Her passion is volunteering as an “Awareness Ambassa-dor” with � e Sjogren’s Syndrome Foundation to help those su� ering from the disorder. Sjogren’s (pronounced show-grins) is a chronic autoim-mune in� ammatory disease that damages moisture-producing glands. Signs are dry mouth, dry eye, joint/muscle aches, and fatigue. It can a� ect other organs and can also cause lymphoma. As many as 4 million Americans have the disorder; nine out of ten are women. Tennis star Venus Wil-liams su� ers from Sjogren’s as do Sherry’s mother and brother. Sherry is available to medical and community groups to educate about Sjogren’s.
