1) Completing the bottom of the 1st page of the patient form (“Registration Number”) before you send it to Van-derbilt On the bottom of page 1 of the patient questionnaire, there is a section called “Registration Number”. (See image to right.) It consists of three components—The surgeon’s initials, the pa-tient’s social security number, and your site code. Often times the surgeon’s initials and site code are pre-labeled. However, the middle section needs to have the patient’s social security number. In order to pay your patient the $20 subject participa-tion fee, Vanderbilt payroll (the folks who cut the check) re-quire it. They won’t send a check without it. So, we ask each site to complete the bottom section of the 1st page of the pa-tient questionnaire. In the case that a patient does not have a social security num-ber (i.e. non US citizen, child), just put a note saying “no SSN on file”, so we’ll know. In those cases, we’ll send them a VISA $20 gift card. It is more paperwork on our end, but at least your subject gets paid. 2) Crossing out a page that doesn’t apply Answer: Don’t Reason: We use an optical scanning software package, and the application will assume an answer is checked if it sees any line (errant or not) going thru the box that’s associated with it. So on the pages where a line inadvertently goes thru some of the boxes, the software will pick it up and erroneously put that answer into the database under that question. Vanderbilt can override that (and delete it), but it takes a bit of manual ma-nipulation. Alternative Solutions: Put a post-it note with a “X” on it, let-ting you know that this doesn’t need to be populated. Then it can easily be removed. Or, put an “X” at the top of the page where it doesn’t cross any of the variable boxes.
