In the early nineties wide-diameter implants were introduced to the mar-ket. Initially, due to lack of matching prosthetic components, the implants were restored with standard diameter abutments. Long term radiographic follow-up of these “platform-switched” implants demonstrated unexpected positive effects on crestal bone main -tenance. These positive effects have been described in numerous studiesover the years and the “platform-switching” is a commonly accepted concept.

A Meta-Analysis published by M.A. Atieh et al. came to the conclusion: “The marginal bone loss around platform-switched implants was signifi cantly less than around platform-matched implants.” 1

How do I achieve improved preservation of crestal bone?

Numerous factors affect the bone level and the health status of the soft tissue surrounding an implant.

Some factors are design-related and can have a crucial infl uence on the success of the treatment. The use of diameter-reduced abutments, the so called “Platform- Switching” (PS) is a very valid example of this.

Gaining soft tissue through platform-switching

To achieve a successful restoration with XiVE platform-switching it is essential to not only use the fi nal abutment at a reduced diameter, but also to start with a diameter-reduced cover screw and to continue with the appropriate prosthetic components.

The platform-switching concept is designed to increase the soft tissue volume, which contributes to long-term esthetic outcomes.

Step-by-step

Removal of the TempBase/ submerged healing

The implants are always supplied with the matching diameter TempBase. If submerged healing is planned, the retaining screw of the TempBase is loosened with the hex screwdriver 0.9 mm and then removed. Afterwards, the cover screw PS D 3.4 is mounted and tightened at max. 14 Ncm. This is followed by wound closure.

Platform-Switch on XiVE®

Use of the gingiva former

To shape the gingiva, a suitable gingiva former PS (slim) is used according to the thickness of the soft tissue, regardless of whether transgingival or submerged healing is planned. Tightening is per- formed using the 0.9 mm hex driver at 14 Ncm.

Impression using the PickUp technique

The exact transfer of the implant position is also performed with special PS com-ponents. For the PickUp technique, the transfer coping PickUp PS is used. The transfer post reposition PS is available for the repositioning technique.

The final resto ration

Determine the gingiva height for the right selection of PS abutment, i.e. XiVE EstheticBase PS D 3.4, is performed using the known methods (see Surgical Manual). The consequent use of PS components shifts the transition between implant and abutment to central and creates an improved emergence profile.

Please note:This procedure is also possible with the implant diameters D 4.5 and D 5.5. Observe the respective use of the appropriate diameter-reduced abutments. The Platform-Switch components are made to be used with XiVE implants only.
