March is here and it is one of my
favorite times of year…March
Madness…The NCAA Basketball
Tournament. It is one of the least
productive times in US workplaces and
it is estimated that billions of dollars
of productivity is lost.
Workers become distracted with their brackets and
scores and as a result, there is a decreased output of
work. This can happen to us at any time of the year.
Whether it is Facebook or ski trips or other events,
there is a risk of losing focus early in the year. Don’t
let this happen to you. Focus on what is important and
be productive.
March Madness is bound to occur in our State
Legislature as well. The session starts on Tuesday
March 7, 2017 and will last for 60 consecutive days
unless 3/5th of both the House and Senate agree on an
extension.
There are several issues that concern Health Care. PIP
may be on the chopping block without a plan to
replace it. There is also a bill on Direct Provider
Contracting. Another deals with a one year limit on
refunds requested by insurance companies from
approved health insurance claims. There is a
telemedicine bill and a few others. If you are not
aware of these, I suggest you check with the FCA for
updates as to the progress of this and other important
legislation.
It can be madness when our legislators start discussing
these measures and the outcomes of the new laws
passed as well as those measures that do not make the
cut.
Don’t let March be a time of madness for you. Keep
focused on your goals and priorities. Keep updated on
what is happening in our legislative process.
And don’t forget…Happy St. Patrick’s Day!!! Everyone
is Irish for a day!!!

CONFIDENCE AND
CONSEQUENCE
The instructor projected an x-ray
image onto the screen and asked the
class if it was normal. But before
anyone in the class could answer, the
instructor pointed to a lytic lesion in
the pelvis and asked if anyone had
seen it. I thought to myself, “Oh yeah, I saw that”.
Yet, I did not raise my hand because the level of
confidence I had in my answer was so low even though
my response had no significant consequence (other than
embarrassment). In regards to radiology film
interpretation and in clinical practice, confidence (a
state of being certain) can be developed and improved.
The confidence that one has in response to a question is
directly related to the consequences of that response. I
had no responsibility for the consequence of my
response when the instructor asked if anyone saw any-
thing on that projected image. If the instructor had said
that you need to write down your answer, but do not
have to hand it in, the value of that response would
increase because I had to commit, even if no one would
see what was written. But, if I were asked to hand in
my response and it would be read out loud, my level of
confidence would be shaken because there would have
been a greater value to what was written. If the
question was on a final examination, the consequence
of my response would have even more value and I
would have to have a reasonable degree of confidence
in my response.

Every time I interpret an image I need to have a high
level of confidence (certainty) in what I say since the
consequence of my impression has such a high value.
The clinician has the same responsibility since the
patient’s record is the testimony of that clinician and the
information contained in that record can have
considerable value and consequence. Confidence
in the content of that file is vital.
Can the level of confidence in a clinical setting be
improved? I think so. One of the best ways to improve
is through education. The reason I did not have the
confidence to verbalize a response or even raise my
hand in that radiology class was due to my relative
ignorance to the discipline of radiology. As my
educational level increased through additional courses
and residency, my level of confidence improved until
ultimately I was comfortable enough to sign my first
report. And the education never ends. After decades in
practice I need continuing education to stay proficient
in radiology. The clinician undergoes the same
evolution. If you want to reach higher levels of
confidence in your discipline never stop learning.
The other factor involved in improving your level of
confidence is through experience. Experience does not
necessarily equate with time. Many physicians in
practice for twenty years do not have twenty years of
experience, but rather one year twenty times. Having
interpreted hundreds of thousands of images, I continue
to feel more certain of my opinions. But I also realize
that I can never achieve 100% certainty. The clinician
gains experience by careful examination of each patient
and fine tuning treatment protocols. As proficiency
improves, so does the level of confidence. Missing an
abnormality on an x-ray or on the physical examination
of a patient can cause a loss in confidence. However, a
humbling experience like that can also serve as a
challenge to do better.
One needs to be careful of over confidence since it can
lead to carelessness and medical error. Try to improve
with each patient encounter, by learning from the
experience. Do not become an academic sluggard.
Keep yourself challenged and realize the value of what
you do and the consequence of it. Confidence will
develop from that.

Ulnar neuropathy, commonly referred to as
“Tennis Elbow,” is a relatively common condition
seen in practice and can often be confused with a
cervical radiculopathy and is often misdiagnosed.
Several sites of entrapment are possible which
include the cubital tunnel and at Guyon’s canal in
the wrist. Pain and paresthesias typically radiate
distally from the elbow, into digits 4-5 of the hand.
Since the symptoms for all three entrapment loca-
tions are similar, electrodiagnostic testing can be
helpful in pinpointing the exact location of the lesion
and identifying its severity.
The ulnar nerve is a continuation of C8 and
T1 fibers from the medial cord of the brachial
plexus. This is why it can often be confused with a
C8/T1 radiculopathy. Ulnar nerve lesions at the
elbow are much more common than ulnar lesions at
the wrist. Some common etiologies include bone
deformities, chronic subluxation, cubital tunnel
syndrome, and repetitive movements. There are
several common hand postures that arise with an
ulnar neuropathy. Benediction posture, Wartenberg’s
sign, and Froment’s sign are three common
findings.
The physical exam and patient presentation
are very important for differentiating an ulnar lesion
at the elbow vs. Guyon’s canal. Observing and
testing the intrinsic hand muscles gives valuable
information as to the location and severity of the
lesion. Cozen’s and Mill’s are two orthopedic tests
which can be used to replicate the patient’s symp-
toms. In situations where the physical exam cannot
specifically pinpoint the exact location, or a cervical
radiculopathy is suspected, electrodiagnostic testing
is highly recommended.
Treatment should start with non-operative
options. These include activity modification and an
elbow night extension splint. The splint is mainly
worn at night to prevent waking. It limits the elbow
from bending and helps protect the nerve from
being squeezed at night. Kinesio-taping and bracing
can also be used in cases where the entrapment is
activity related. Active Release Technique has also
been proven to be extremely helpful in resolving
symptoms and reducing musculature entrapment.
Fortunately, this condition can be successfully man-
aged with conservative care, although in situations
where symptoms become progressive or weakness
and/or atrophy occur, a surgical consult with an
orthopedic surgeon is highly recommended.
