In thepast fewweeks, I travelled tothree different places where I haveengaged inprofessionaldevelopmentactivities.Firstly, Iwas invitedby theMacauHealthBureauas theExternalExaminerfortheassessmentofdoctorsingeneralpracticefortheirpromotions.IwenttoameetingwiththeOfficialsoftheBureauinMacautodiscussfinalizingthecriteria forassessment.Macauhasadifferent systemof theenrollmentandtrainingofmedicalpractitioners.Tosit for theexamination, thesedoctorsneedat least5yearsofexperiencesingeneralpracticeundertheBureau.Theyhave topresent thecurriculumvitaesimilar toourBasicvocational traininglogbook which should list in detailsthedurationofexposures in variousmedicalservices,both inhospitalandgeneral practice. Three examinerswould be appointed by the Macaugovernmentwhichwillbegazetted intheannouncementof theassessment.Candidatesare requiredtosubmitallrelevant documents within a statedperiod of time, so that these threeexaminers can assess and mark thedocuments.Markingcriteriaareclearlylaiddowninthegazetteandexaminerswillhave to follow thesecriteria.Thesecondpartwillbeanoralexaminationwhich wi l l last for s ixty to ninetyminutes.Thethreeexaminerswill taketurnstoaskquestionseitherasatestofthecandidate’smedicalknowledgeortoclarifyanymattersarising fromthelogbook.Markswillbegivenaccording
to thecandidate’s responseto the quest ions ra i sed.In order to get a pass, thesumofmarks fromthe twosegmentsshould be higher than 50%, whichmeanspromotiontoahighermedicalgrade as well as salary increments.
This issomethingquitedifferent fromour current system of assessmentof postgraduate qualification whichonly tests knowledge and skills, butdoes not take into considerationpracticeexperienceandothermedicaleducationalactivities. Isthereanythingwe can learn from this, especiallyfor those general practitioners whohave started private practice beforeformal vocat ional t ra in ing beingavailable to them in Hong Kong?

Arewereadytotakeupthechallengetoassistourcountryinthehealthcarereformbasedonapropersystemoffamilydoctorsbasedinthecommunity?Thedevelopmentofaproperandappropriatetrainingmodeltakingintoaccountsregional,geographicalandculturaldifferencesisoftheutmostimportance.StandardsettingandassessmentmusttakeintoconsiderationofthelevelofserviceandresourcesavailableinalargecountrywithhugepopulationlikeChina.Wemustbepragmaticbeforewecanachievearealisticgoalstepbystep.Ihopeouryoungergenerationoffellowscantakeupthismostworthwhilechallengetocontributetoourcountry.

HKCFP Research Fellowship 2011
Introduction
The HKCFP Research Fellowship was established by the Hong Kong College of Family
Physicians to promote research in family medicine. The grant is up to the value of HK$100,000.
It provides the successful candidate with protected time to develop research skills. Applicants
are expected to have regular contact with a nominated supervisor with research doctorate degree or equivalent.
Eligibility
Applicants for the HKCFP Research Fellowship must be active fellows, active full members or
associate members of the HKCFP. New and emerging researchers are particularly encouraged to
apply.
Selection criteria
Applications will be judged on*:
• training potential of applicants
• relevance to family medicine and community health
• quality
• value for money
• completeness (incomplete or late applications will not be assessed further).
* Please note that new researchers and those at an early stage
of their research careers are encouraged to apply.

The HKCFP Award for the Best Research of 2010
The Research Committee of the Hong Kong College of Family Physicians has an Award for the
Best Research of the Year 2010. All members and fellows of the College are invited to participate
and submit their research papers to the Research Committee for selection. The Award would be
presented at the Conferment Ceremony in 2011.
Entry and assessment criteria are listed below:
Entry Criteria:
1. The principal investigator has to be a member or fellow of the Hong Kong College of
Family Physicians.
2. The research must be original work of the investigator(s).
3. The research should be done in Hong Kong.
4. The research must have been completed.
5. The paper should be presented under the standard headings of Abstract, Introduction,
Methodology, Results, Discussion and Conclusion. References should be listed in full
at the end in Harvard or Vancouver format.
Assessment Criteria:
1. How relevant are the topic and finding to Family Practice?
2. How original is the research?
3. How well designed is the methodology?
4. How well are the results analysed and presented?
5. How appropriate are the discussion and conclusion(s) drawn?
6. How useful are the results for patient care in Family Practice?
7. How much effort is required?
Each research project submitted will be assessed according to the seven criteria listed above by
a selection panel. Each criterion may attract a different weighting to be decided by the selection

HONG KONG CHECK 2011CONTINUOUS HOME EVALUATION OF
CLINICAL KNOWLEDGE
Published by the Royal Australian College of General Practitioners for the last 25 years,Check is one of the most successful continuous medical education journals in the world.
The Hong Kong College of Family Physicians, in collaboration with the Royal Australian College of General Practitioners, runs a Hong Kong version of the Check program.
It comes in case presentation format and examines a different medical topic every other month.
Now the Hong Kong Check for 2011 (6 issues) is open for subscription. Those who wish to subscribe please fill in the form below and mail it back to the Editor with payments. The subscription rates will be $550 for members and $850 for non-members.
Members who subscribe to the Hong Kong Check 2011 will be awarded CME credit points for satisfactory completion (at least 75% of the questions) of the HKCFP “Check on Check” program issued at the end of the year.

Assessment of Family Physician
I read with interest the President’s Message in the HKCFP – Family Physician Links Issue 75, September 2010 – Dr. Gene Tsoi discussed the captioned, with reference to the Practice Assessment, Clinical Audit and Consultation Skill Assessment CSA; “…most of the cases seen [at CSA] by examiners are stable and well controlled diabetes, hypertension …”
Some examiners are aware of such phenomenon, the causes of which I shall not dwell on here. With the current CSA format, we use a specific tool, a specific criterion referencing rating system. However, the limitation being we can only have 2 hours [120 minutes] examination time; and for all practical purposes [definitely not by intention] only 6-7 cases can be rated.
Over the past 2-3 years there have been proposals to address the aforementioned undesirable issues. (A) The new CSA will have 6 hours of live video recording, which are triple the current examinable time. (B) A minimal of 18-24 cases [not yet finalized] will be recorded – which tally to three to four times the current CSA clinical loading. (C) Examiners will have the authority to choose “non-chronic cases”, as they feel fit, to better assess the real life clinical challenges to family physicians.
By the way, there is also new proposed criterion for Clinical Audit CA, hoping to address more on impact factor / outcome changes. There should also be a new Research Segment - which I shall leave for others to deliberate.
I certainly hope that President, members of College and members of public would be pleased to note certain Teams and Committees’ members are constantly working to improve the quality of family medicine practice.

conservedfortreatingpatientswithseveremood
disorders,therebyreducingtheoccurrenceof
suicideanddomesticviolence.
The Mental Wellness Centre aims at earlyidentif ication of mental disorders, earlyin te r vent ion fo r pat ient s w i th ment a ldisturbances,andpromotingpublicawarenesson importanceofmentalhealth.Forpatientsattendingoneofthe23GOPCsinKowloonWestCluster,weperformmentalhealthscreeningbyusingtheWHO-5Wellbeing Indexscreeningform.PositivelyscreenedpatientswhomeetourcriteriaorthosepresentingwithsymptomsofmildmentaldisturbancearethenreferredtotheMentalWellnessCentrefor individualandgroupinterventions.Patientsareassessedbythecasemanager,eitheranadvancedpracticenurseoranalliedhealthprofessional.Afteraninitialassessment,briefpsychologicalsupportandcounsellingwillbeprovidedaccordingly.Problemsolvingtechniques,relaxationexercises,stressmanagementtechniques,waystotackleinsomnia, lifestyleredesignandpromotionofpositivethinkingandlifeattitudeintheformatof individualorgroupcounsellingareoffereddependingonpatients’needsandpreferences.PatientshavingmoreseverementaldisturbanceareassessedbyClinicalPsychologistandFamilyMedicinespecialistforfurtherinterventionandmanagement.Weeklyteammeetingsareheldforcasediscussionandmonitoringofpatientprogress.

Inan8-monthperiod(August2009-March2010),morethan500patientsattendedourMentalWellnessCentre,around300patientscompletedtheserviceandwerealreadydischargedinanaverageoflessthan4-5individualattendances.70%ofthemweredischargedwithouttheneedof further follow-ups.Mostof themsufferedfrommildmooddisturbance,mildanxietyandmildadjustmentdisorders.ThemeanWHO-5scoringwas8.9(cut-off:13)beforereceivinginterventions,while themeanpostWHO-5scoringwas14.5ondischarge.
Family physicians play important roles inpreventionandearlyintervention.Hypertension,
diabetes,osteoporosis screeningandmany
otherhealthscreeningshavebeenwell-accepted
andtherearestructuredscreeningprograms
inprimarycareclinicsandinthecommunityfor
thesephysicaldiseases.Mentalwellbeingisan
importantaspectofbeinghealthyasdefined
bytheWHO.TheMentalWellnessCentreisa
pilotmulti-disciplinarymentalhealthprojectin
GOPCsforearlydetectionandinterventionof
mentaldisorders.Withincreasingresourcesfrom
thegovernment formentalhealthproblems,
wehopenewmentalhealthservicescanbeset

Thecaseswesawtherewere just likewhatwesaw ingeneraloutpatientclinics.Thereweredifferenttypesofcases;mostofthemwererelatedtohygieneproblemssuchas fluandgastroenteritis.TBwasmorecommonthere.Somepatientspresentedwithchroniccough,andsomepresentedwithTB lymphnodes.Theyalsosuffered frommanydentalproblemsas theyhadnotoothbrushes.Luckily,alocaldentistjoinedustoprovidesomebasicdentalservicessuchasextractionandfilling.Though therewereonlya few typesofbasic simpledentalequipments, theywereadequatetoaccomplishmanydifferentdentaltreatments.Formanyofthecases,theyneededfurtherinvestigationsanddrugtreatments.However,somepeoplecouldnotaffordtheexpensesanddiedasaconsequence.Somealarmingcaseswereencounteredsuchassevereweight loss,generalizedweaknesssuchthat thepatientcouldnot riseupfrombed,achildpresentedwithchronicbleedingfromthehardpalate,awomanshoweda largemassontheskinnear themouth……Whatpeopleneeded therewasfurthercheckups,investigationsandtreatments.
SuchproblemswouldneverhappeninHongKong;everypersoninHongKongcanseedoctorsandenjoyhealthcareservicesirrespectiveoftheirwealth.Somepatientsin Hong Kong even misuse our system, repeatedlyrequestingunnecessaryinvestigations.Ifwecanrelocatesomeoftheresourcesfromwealthiersocietiestopoorercountriesmorelivescouldbesaved.Ifeltsogratefultolive inawell-developedsociety.WeshouldthankGodandtreasurewhatwehave.
People inMyanmararehappy. Indeed, theyappeartobehappier thanweare inHongKong.Every timewegavesmallgifts to themorperformedsmall favours,theywereveryhappyandmuchsatisfied.Althoughtheyarephysicallypoorandlive insimplehouseswith littlefurnituresandtornclothings, theyarementally richerthanus.
Actually,lifecanbesimple!!
Thiswasmyfirstmedicalmissiontrip.Thetripwasverymeaningfuland it impactedmy interpretationon themeaningof life. Iwould liketosharethisbeautiful tripwithyouall.
Thedaybeforestartingthemedicalservices,wevisitedaschool fordeaf-mutechildren. Itprovidedschoolingforchildrenfrompre-schooltothe10thgrade.Ittaughtspeechreading,signlanguage,fingerspelling,reading,writingandcalculation.Somesimplevocationaltrainingsweregivensuchassewing,tailoring,knitting,stitching,andbasiccomputertraining.
Thebuildingwasold, thefloorandthewindowswerebroken, therewasnoair-conditioning, the lightwasdim……howeverdespite thepoorenvironment thechildrenpaidextremeattentionandtheteacherswereverypatient.Isawoneteachershowingalittleboyhowto readspeechbypickingup theboy’ssmall fingersandtouchingtheirown lips,allowingthemtofeel themovementandvibrationofthelips.IsawtheLOVEfromthat teacher.Childrenhere learnedhappily,growingupwithhealthymindssothattheycanliveandearnontheirown, independently.Weplayedgameswiththemandtheylaughedhappilyandsmiledsweetly.Theywerefilledwithhappinesswhenwegavethemsmallgiftssuchasballoonsandcolourpens.ThiswasnoteasilyfoundinHongKongkids.
ThenwehadfourdaysofmedicalservicesinMyanmar.Thefirstdaywestayedatthevillage,thesecondattheYMCAclinic,thethirddaywewenttoavolunteercenterandonthefourthdaywestayedatanotherfarmvillage.
At thebeginningof serviceeachday,wegave themhealth talks, such as teaching them how to washtheirhandsproperly,how tobrush their teeth,howtomaintaingoodhygieneandappropriateposturewithmanualwork.Thereafter,westartedourmedicalservices,givingpatientsdrugsandhealthadvice.Someofourvolunteerworkersplayedwith themandweallhadalotoffun.

What’s happening with Rosiglitazone?
Recentlytheuseof theantidiabeticagentrosiglitazonehasbeensubjecttocontroversy.On
29September,2010, theHospitalAuthorityannouncedthat twopreparations,Avandiaand
Avandamet,whichcontainrosiglitazoneasanactivecomponent,wouldberemovedfromits
drugformulary.Thereareapproximately2,000patientscurrentlyreceivingrosiglitazoneinpublic
hospitalsandout-patientclinics. Theyarerecommendedtocontinuethemedicationuntilthe
nextfollow-upappointmenttoensureoptimalglycaemiccontrol.ThedecisionoftheHAis in
accordancewiththerecommendationfromtheCommitteeonMedicinalProductsforHuman
UseoftheEuropeanMedicinesAgency.1
Sowhatdoesthecurrentevidencesayaboutrosiglitazone?Twoimportantstudiesoffercaveats
onitspharmacovigilanceinclinicalpractice.
Grahamandcolleaguesstudied the riskofseriouscardiovascularharmby rosiglitazoneas
comparedwithpioglitazone,anotherthiazolidinedionemarketedintheUS.2Theyconducteda
nation-wide,retrospectivecohortstudyof227,571Medicarebeneficiariesaged>65yearswho
initiatedtreatmentwitheitherdrugfromJuly2009-June2009andwhowerefollowed-upforup
to3yearsaftermedication initiation. ItwasfoundfromCoxproportionalhazardmodelsthat
rosiglitazonewasassociatedwithstroke(Hazardratio[HR]1.27,95%C.I.1.12-1.45),heartfailure(HR
1.25,95%C.I.1.16-1.34),all-causemortality(HR1.14,95%C.I.1.05-1.24)andacompositeofacute
myocardialinfarction,stroke,heartfailureanddeath(HR1.18,95%C.I.1.27-2.08)whencompared
withpioglitazone. Its largesamplesizeandtherobuststudymethodologyforanalysisofthis
inceptioncohortwereimpressiveandgivethestudycredibility,despiteitsretrospectivenature.
Anotherstudywhichaddscautiontorosiglitazone’susestemsfromanupdatedmeta-analysis
of randomizedcontrolledtrials reviewingtheeffectsof rosiglitazonetherapyonmyocardial
infarction(MI)andmortality.3Thisincludedatotalof56trials,or35,531patientsreceivingeither
a rosiglitazoneoracontrolmedication. Itwas foundthat rosiglitazonetherapysignificantly
increased the riskofMI (odds ratio [OR]1.28,95%C.I.1.02-1.63),whichpoints towardsan
unfavourablebenefit toriskratioforrosiglitazone. Thesetwostudieswerepublishedinvery
highimpactinternationaljournals.
However,futurestudieswillnotsimplyendhere,asneweroralhypoglycaemicagentscontinue
tobemarketed,andevaluatedfortheirefficacyandsafetyprofiles.

The21stDermatologyInterestGroupmeetingwasheldon4thSeptember,2010.OurspeakerwasDr.LoKuenKongwhowasjustretiredfromConsultantDermatologistandHeadofSocialHygieneServiceof theGovernment.OursponsorwasGaldermaHongKongLtd.Thetopicwas“SunandSkin”.
Thereareessentially three typesofUltravioletradiation(UVR)inSunlight:
1. UVC(100-290nm)hastheshortestwavelengthandusuallycannotpenetrate theozone layerandreachtheearth’ssurface.
2.UVB (290-320nm)has longerwavelength,ableto reach skin sur face, causing tanning andacutesunburn(300nmUVBis1000timesmoreerythrogenicthan360nmUVA).
3.UVA(320-400nm)haseven longerwavelength,able to reach deep into skin, releasing freeradicals, causingDNAdamageand inducingpremalignant lesions / cancersof the skin. Itcanalso leadtophotoagingbybreakingdowncollagen and elastin (as distinguished fromchronological ageing, thewrinklesaredeepandcoarse,and thereare increasedmottledpigmentation, xerosis, telangiectasia, skinatrophy,solarkeratosisandelastosis).
Toprotectagainstsunlight,thefollowingpracticaltipscanbegiventoourpatients:
1. avoidsun’speakUVhours-between10amand4pm
2.seekshadeswheneverpossible
3.wear long-sleeveshirts,hats,sunglasses;bringumbrellas
4.wearbroadspectrumsunscreendailyyearroundespeciallyon face. (N.B.UVAraysarepresentyear roundandcanpenetratethroughglassesandwindows).Reapply sunscreenpreferablyeverytwohours.
Beneficialeffectsofsunscreenincludepreventionagainstacutesunburnanderythemaandchronicphotoaging,protectionofphotosensitiveskinandpreventionofskincancers.
Sunscreenusedshouldhavea broadspectrumagainstbothUVAandUVB rays.The inorganicphysicalblockers (e.g. titaniumdioxideandzincoxide)areusuallymoreeffective inblockingbothUVBandUVArays.Thenewerorganicsunscreene.g.MexorylXLandTinosorbMaresaidtohavecomparablefiltereffects.
Inchoosingapropersunscreen,theUSSkinCancerFoundationrecommendsaSunProtectionFactor(SPF)ofatleast15.SPFmeasuresthelengthoftimeasunscreenproduct(inastandardquantityof2mg/squarecm)protectsagainstskinerythema fromUVB,comparedtothetimefortheskintoreddenwithoutprotection.Forexample, ifyounormallyburnwithin10minutesofUVRexposure,aproductwithSPF15will theoreticallyallowyoutostayout15timeslongerwithoutburning(i.e.150minutes).Similarly,aproductwithSPF30shouldallowyoutostayoutfor300minutes.
However,oneshouldnotethataproductwithSPFof15canalreadyblock93%ofUVBandaproductwithSPFhigherthan30canblockanadditional4%ofUVB(i.e.97%)only.Nomatterhowhigh is theSPF,there isnoproductthatcanblockout100%ofUVBrays.OneshouldalsonotethatSPFratingisameasurementofprotectionaffordtoUVBraysonlyandthereisstillnointernationalconsensusonevaluationofUVAprotection.
Although sunlight isbeneficial for synthesisofVitaminDat theskin, there isstillnoconvincingevidencethat increasingaperson’sconventionallynormal serum 25 Hydroxy Vitamin D level hasdefinitehealthbenefits, especially in reducingcolorectalcancer risks. In factmostpeoplecanobtainampleVitaminDfromacombinationofdiet,oral supplementsand incidentalprotectedsunexposure. It isthereforenotadvisabletoaskourpatientstohaveregularintentionalUVexposurejustforthepurposeofobtainingsufficientVitaminD.
