RemotePatientMonitoring
Monitoring key patient health measures such as weight, blood pressure, oxygen saturation, temperature, and glucose for patients at home.
eVisitsEnabling patients to request and receive care asynchronously using MyChart patient portal.
eConsultsEnabling provider to provider asynchronous consultation concerning patients via Epic workflows.
Tele-MentoringProviding video education and case presentation focused on specialty care for rural providers to enable care in their communities.
Video ConsultsEnabling provider to provider synchronous consultation concerning patients for stroke care and other emergent episodes.

started Project CORE in 2014 to help solve care
coordination issues
• Over 30 Academic Medical Centers are participating with
Project Core to deliver eConsults for their patients
• UVMHN joined with Project Core in 2019 and will bring it
to our patients in 2020
