NEW SERVICE We now have a new team working with the surgery, they are the Living Well Team. They are working on the Social Prescribing Project, and you may see them around the surgery from time to time. Your Living Well Co-ordinators are called Anne-Marie and Ellie and we would like to welcome them to the team, and look forward to working with them.

But, what is Social Prescribing? Social prescribing is a means of enabling patients with social, emotional or practical needs to access a range of local, non-medical services, often provided by the voluntary and community sector. In other words it is about health and care professionals (GP, nurses, social care etc.) providing patients with “prescriptions” for activities rather than drugs and medical treatment.

They might be able to help if you are:

Feeling low or worried

Feeling Lonely

Feeling Isolated

Bereaved

Experiencing relationship problems

Experiencing financial problems

Experiencing housing problems

Having problems related to employment or unemployment

A victim of bullying

A victim of crime How do I get referred to this service? If you feel you would benefit from this service please speak to someone within the practice, this doesn’t have to be a GP but can be any member of our team from nurses, reception, admin and of course GPs. We can pass your name onwards to Anne-Marie and Ellie the Living Well Co-ordinators.

What Happens Next? To help make sure the service is what you need, a Living Well Co-ordinator will contact you and ask a few questions. Then they will spend time talking to you to agree a plan to help improve your health and wellbeing. Time spent with each person will depend on their needs and ranges from simple signposting to a full assessment and support.

The Christmas and New Year period can seem like a time where

everyone is celebrating and having fun. However it can also be

challenging for those going through difficult times.

Whatever the festive period means to you, it’s important that

everyone feels able to manage their mental health throughout the

holidays.
