Our teams now work in over 150 facilities serving over 3,200 patients. This care model improves access to care and supports patients, families, and staff at a crucial time in the life continuum. This model decreases unnecessary trips to the Emergency Department and Hospital when these visits and stays can be very disruptive, at times unhelpful, and always expensive. Eighteen months ago Dr. Stivland and the Bluestone Physician Services leadership team decided to expand access to this care model. Our intention is to expand access to on-site care for high-need, under-served patients by partnering with organizations that understand the growing aging population, unsustainable costs, and limited access to qualified providers as a challenge we best face together. Bluestone is now working with Fairview clinicians to bring enhanced on-site care via the Bluestone Vista model to patients in Ebenezer owned and managed facilities. Our team is also in various stages of conversation with other Minnesota and out of state healthcare systems wanting to expand access to this model of care. Bluestone believes collaboration with patients, families, and like-minded organizations is the most constructive way to improve how our health care system performs. Shelli Samson is the bubbly personality responsible for enrolling new patients in Bluestone Physician Services. Thanks to her diligence and attention to detail, patient information is accurate, up to date, and ensures that patients get seen in a timely fashion. Shelli grew up in Roseville, Minnesota. She attended college at the University of Wisconsin – River Falls where she majored in “Health and Human Performance.” Upon walking through the doors at Bluestone, Shelli knew it was the place for her: “It is a very welcoming place. You always hear people laughing and everyone gets along well. Everyone really seems to enjoy their job here.” Shelli’s favorite part about her role as intake coordinator is being able to

communicate with family members. In her free time, Shelli enjoys hanging out with her friends and family, coaching softball, going on road trips, and spending time out on the farm in Clayton, WI.

Transitions of Care

Please join us in welcoming Dr. Don Anderson to our team of providers. Dr. Anderson is a graduate of The University of Wisconsin - Madison and is Board Certified in Family Medicine. He completed his residency program at the University of Minnesota. Prior to joining Bluestone Physician Services, he worked as an ER Physician for St. John’s Hospital.

Dr. Anderson will be leading his team with his Physician Assistant, Jennifer Siegle, to provide on-site medical care to communities in the Twin Cities North Corridor.

Transitions. Almost every area of health care is working to decrease the number and improve the quality of transitions of care. When patients, particularly seniors, go to the emergency room, hospital, skilled nursing facility, or TCU, unplanned events happen all too often. The National Center for Assisted Living has cited the reduction of readmissions as the primary goal between now and 2015.

Bluestone Physician Services is committed to keeping patients in their homes whenever possible. Monthly on-site visits, timely response by phone and the Bluestone Bridge, and true facility based care all drive toward our goal of reduction in ER visits and acute admissions and readmissions. A recent survey conducted by Bluestone found that the majority of ER visits were for falls, with no injury and urinary tract infections. Our primary care teams are committed to working collaboratively with staff to prevent these types of admissions. Please call or visit us through the Bluestone Bridge.

Bluestone is in the process of reaching out to local hospitals to increase and improve the communication that occurs during a transition. The role of facility staff is also crucial to this goal. The Bluestone primary care teams ask that staff alert them as soon as possible via the Bluestone Bridge when a transition occurs and always when a resident is returning to the facility.

The care coordinator assigned to your facility is also an excellent resource during transitions. The Care Coordinator can visit the hospital, coordinate home care services, and assist with equipment and supplies.

Bluestone Physician ServicesVirtual Visits

As many of you are aware, there is a significant need for ongoing Psychiatric care in the communities we serve. We also are aware of the significant shortage of options and long waits for appointments for mental health providers. Additionally, many of our patients suffer from an inability to tolerate going out into a unfamiliar environment or sitting in a waiting room filled with strangers due to anxiety issues, which often prevent them from seeking out care.

Bluestone Physician Services offers a wonderful alternative to patients with the option of providing Virtual Visits with a Psychiatrist – Dr. David Gish. These visits are established whereby the patient is able to have a Psychiatrist appointment in their own home environment by conducting the visit over the internet, using a face-to-face application on a computer in their home. A Bluestone Physician Services Care Coordinator coordinates the visits and provides the computer access, following up with aftercare for any medications or additional services Dr. Gish recommends.

With this service, we are able to provide much needed Psychiatric care to a population that may not otherwise have access. The patient is given an opportunity to receive care without waiting, in their own home, allowing them an opportunity to feel relaxed and to actively participate in the sessions with Dr. Gish.

For Care Coordinators in the Greater Minnesota area, this has provided a great opportunity to provide care and services to members within the community that have declined to seek Psychiatric care due to lack of available appointments, discomfort and distrust or various other reasons that are easily overcome with the use of Virtual Visits. This option has provided significant benefit for members who would otherwise not receive help for their mental health issues.
