The Irish Association for Palliative Care

This directory of hospice and palliative care services in Ireland is published by the Irish Association for Palliative Care. Over the past decade, we have witnessed enormous growth in the development of specialist hospice and

palliative care services, heralding a new and exciting era in modern health care provision. In 1995 the Medical Council included Palliative Medicine amongst its list of recognised specialties.

Traditionally, hospice services have been most closely associated with the care and support that is offered to patients with advanced cancer. In more recent times, the focus has broadened to include the care of patients with other advanced and progressive, but non-cancerous conditions.

Hospice teams, working in close collaboration with established in-patient and community based services, seek to ensure that all patients and those close to them, receive an optimal level of care and support.

The Irish Association for Palliative Care is committed to the continued appropriate development of hospice services in all areas of the country. The publication of this directory, which details all of the established services and summarises the range of their activities, is an important facet of the work of the association. We wish to ensure that all health care professionals, health boards, voluntary and charitable bodies and members of the general public, will have ready access to information on hospice services in their own region.

Hospice and palliative care services continue to rely heavily on charitable funding. Individual health boards must now playa more pro-active role in the planning of an integrated palliative care service in each region and must

ensure that adequate and equitable funding is provided to support such developments. There now exists a unique opportunity for true partnership and support between voluntary hospice groups and health boards, in terms of strategic planning and financial provision.

In the directory, hospice and palliative care services are listed under health board area. Bereavement services are listed separately, also under health board area. Groups and organisations working in support of hospice and palliative care services are also listed, as is a compilation of useful addresses.

PALLIATIVE CARE

Palliative Care is the continuing active, total care of patients and their families by a multi-professional team at a time when the medical expectation is not cure and the primary aim of treatment' is no longer to prolong life. The goal of Palliative Care is the highest possible quality of life for both patient and family. Palliative Care responds to physical, psychological, social and spiritual needs. If necessary, ite.tends to support in bereavement. (European Community, Europe Against Cancer Committee). I

ESSENTIAL ELEMENTS OF PALLIATIVE CARE.

In whatever setting it is delivered, Palliative Care should provide the following essential elements:

relief for patients from pain and 'other distreSSing symptoms; an integrated approach to physical, psychological, social and spiritual aspects of care so that patientS may come to terms with their own death in whatever way is appropriate to them; a support system to help the patient live as actively and creatively as possible until death; . a support system to help famili~cope during the patient's illness and in bereavement.

THE PERSON WHO IS DYING.

One of the main aims should be to improve the quality of daily life. This can be achieved by attention to the following issues:

the removal or alleviation of u~Pleasant symptoms; the removal or alleviation of fear and loneliness; the maintenance of choice and independence wherever possible; spiritual needs.

THE RELATIONS AND FRIENDS qF THE PERSON WHO IS DYING.

One of the main aims should be to provide support to relatives and friends, both before and after the death of the patient. There is a need to determine to whom support should be offered; sometim~s the person is not the ne><l of kin, nor even a blood relative. Staff will need to accept a range of a~emative relationships.

Good support will require attention to the following:

practical support of carers; emotional support of carers; the provision of day care facilities and in-patient beds for respite care; bereavement support.

STAFF_

In whatever setting Palliative Care is being delivered, the following need to be considered:

adequate staffing levels; skilled selection of staff; good communication between staff; adequate and continuous education; adequate and regular emotional support.

IN PATIENT HOSPICE UNIT.

An in-patient hospice unit offers specialist palliative care for patients and their families. It has a trained, multi-professional staff who are specialists in palliative care. As well as that it also has a high ratio of nursing staff to patients. The unit offers additional services such as bereavement counselling and day care. The in-patient hospice unit is also an important resource in the area by being a centre of research and education in palliative care.

HOME CARE TEAM,

This is a team of specialist staff set up to offer advice and support to patients, their families and to health care professionals. They do this by complementing the skills and resources of the primary care team thus helping families to care for the patient at home.

The location of units such as these offers an excellent opportunity for the improvement of standards of care in the hospital as a whole, not only for the dying but also for patients at an earlier stage of their illness whose professional carers may require advice on symptom control.

I , PALLIATI\IE CARE SERVICE WITHIN A GENERAL HOSPITAL

This service is provided by specialist ,staff who offer advice and support to patients, families and health professionals. They do this by complementing and supplementing the skills and resoJrces of the existing hospital care team.

PALUATIVE CARE SUPPORT BEDS

A growing number of general and di~trict hospitals are providing beds for patients requiring palliative care. These beds are also available to provide backup to the Primary Health Care Team.

A distinction Is drawn between this type of arrangement and the specialised Palliative Care Services in general hospitals.

PALLIATIVE DAY HOSPICES I A day hospice is a day care facility for patients requiring palliative care. Such patients may be referred from an in-patient or home care setting. This includes a carefully thought out programme of activities under the supervision of staff w~h specialist training in palliative care.

REFERRAL

Referral to a hospice or palliative care home care service is nonmally arranged by the patient's own GP or hospital doctor. Most services only accept patients from within a certain radius. Please check the area covered by each service under the entry for that service.

IS THERE A CHARGE?

Every citizen in the state is entitled to free in-patient hospice care. However, if you wish to go privately, please discuss this with your local hospice.
