ONLINE CHECK-IN:

Login to your GotSoccer TEAM ACCOUNT, and upload the following required documents by Friday, March 6th, 2020 Step by step instructions provided on next page.

Team Roster (Official US Youth Soccer or US Club Soccer roster) o Jersey number required for all players o If you have guest players, they should be hand written on the bottom of your roster copy o Jersey #, Full Name, Player ID # (from player card) and DOB

Player Passes o 2019-2020 season, sanctioned by your state association via US Youth Soccer or by US Club Soccer for

all rostered and guest players. o Guest player passes must match

Team Information Sheet – with signed acknowledgement of possession of all Medical Release Forms o Form can be found on the tournament website under “2020 Tourney Check-in and Rules“ o FIND TEAM INFORMATION SHEET HERE

Permission to Travel form o Only required for USYS Rostered Teams from outside of the East Region. Check with your State

association for the appropriate form. (East Region teams include: ME, NH, VT, MA, RI, PAE, PAW, NYE, NYW, MD, DE, VA, WV).

o Not Required for US Club Soccer rostered Teams. If you are unable to upload the documents, they can be emailed to tournamentdirector@staffordsoccer.com – Email subject line should contain "CHECK-IN/Age Group/Gender/Team Name" if you choose this option. GAME DAY DOCUMENTATION - Please ensure you have all required document with you at all games.

Roster checks can be requested before, or after, any game.

Requests for roster checks should be made to the Field Marshal prior to the start of the game. ROSTER LIMITS:

U9-10: 12 Players

U11-12: 16 Players

U13-14: 18 Players

U15-19: 22 Players [Can only dress/play 18 players in each game]

*Guest Players – 5 max per team (all age groups)

STEP BY STEP ONLINE CHECK-IN INSTRUCTIONS

1) Log into your GotSoccer TEAM ACCOUNT 2) Click on the event located under “Event Registration History”

3) Click on the “Documents” tab on the far right

4) Select Roster under the dropdown on the right side of the page

5) Click “Choose File” and select the file of your roster on your computer, then click upload file. Be sure to cross off any players not participating, and hand write in guest players on the roster before uploading.

6) Your roster will now appear on the left side of the page. Repeat Step 5 with your Player Passes and Team Contact Information Sheet.

ONSITE CHECK-IN (if online check-in not completed): You must be at your first game on Saturday morning 1-hour before kickoff Check-in is at the main tent at your field location Required documents:

Team Roster – Official Copy and Two Copies for Stafford Soccer Staff to keep.

Player Passes – Original Player Passes (2019-2020 season, sanctioned by your state association via US Youth

Soccer or by US Club Soccer) for all rostered and guest players.

Note: NO birth certificates will be accepted.

Signed Medical Release Forms for all players. They must be signed but do not need to be notarized. Your local

club form is acceptable.
