This handbook is your guide to using these services, and all tenants are encouraged to inquire about any services they would like to see in addition to those described in the following sections. 31st Floor. The normal hours of operation are 8:30 a.m. to 5:00 p.m. Monday through Friday, except holidays.

notify Building Security to ensure that they are properly prepared for the arrival of the emergency vehicles.

The Tenant is Our Guest Building surveys and questionnaires are valuable tools for any building manager.

They ask questions about current management services and building amenities. Periodically, you may be asked to participate in a Building survey which asks you to rate and comment about such services. The results are reviewed and compiled as they are returned to the Management Office. The Management staff uses the survey as a guide to help improve and expand upon current services.

Every weekday night, the Building Security staff will check your entrance office

doors while performing their routine building tours to ensure the doors are locked. In addition, they check for burned-out corridor lights. If you notice a bulb that was missed or recently burned out, please call (312) 558-3800 and someone will respond to change that bulb in ten minutes or less.

Service Requests Any request you have can be entered into the work order section of the website. Tenant Authorized Requestors will receive a log-in and password that will allow them access to make service requests. Once the service request is entered into the system it will be transmitted to the appropriate building staff person via a wireless digital pager. They will do their best to respond within 10 minutes. If you need instruction on how to use the website please contact the Management Office.

Hot? Cold? MB Real Estate is dedicated to providing you with the highest environmental air

quality conditions available. If your area is too hot or too cold, our engineers can provide you with immediate professional and personalized service. Just enter a request on the website or call (312) 558-3800 and a building engineer will be there in ten minutes or less to make any necessary adjustments. The engineering staff will even follow-up later to see if other adjustments are needed.

Umbrella Loan It's raining and you forgot your umbrella. No Problem! Just stop at the Lobby

Security Desk and pick up one of the loaner umbrellas. The umbrellas are available for you and your guests. Just remember to return it to the Lobby Security Desk and there should always be one waiting for you.

Umbrella Bags In addition to the umbrella loan program, the building will provide umbrella bags

on rainy days. During rainy days, umbrella bags will be available near all building entrances. Through the use of umbrella bags we hope to minimize floor slickness and maintain dry lobby floors.

Recycling Programs 181 West Madison has successfully implemented a building-wide recycling

program. Everything placed in the individual blue recycling container under your desk is automatically sent to a recycling plant for sorting. Tenants are also provided with one recycle bin for plastic, glass bottle, and aluminum can recycling. All non-recyclable waste (such as food waste, coffee grinds, etc.) should be placed in your kitchen trash container. For more information, call the Office of the Building at (312) 558-3800. 181 West Madison has also implemented a building wide battery and e-waste recycling program. Batteries accepted in the recycling program are batteries used most often in households and businesses such as, lithium, cell phone, camera and dry cell batteries, etc. We have placed a clear battery collection tube in the lobby near the ATM/mailbox vestibule. Please deposit all recyclable batteries in the tube. Once the tube is full, batteries will be sent to a recycling facility. Please contact the Office of the Building for more information on recycling small appliances, office equipment, computers, printers and toner cartridges.

No Smoking Policy A new Chicago Clean Indoor Air Ordinance was passed by the Chicago City Council in February, 2008. The ordinance replaces the current law, passed in 2005, and supersedes the Smoke Free Illinois Act.

The new law is intended to protect the health of patrons and workers against the

dangers of tobacco smoke, including secondhand smoke. Secondhand smoke is known to cause cancer and heart disease, and "safe" levels of secondhand smoke have not been identified.

The new Chicago Clean Indoor Air Ordinance regulates smoking in and near the

entrance to all public buildings and enclosed workplaces and facilities. It establishes an immediate prohibition on smoking in specified public facilities, and exempts exclusively private residences, some hotel and motel sleeping rooms, retail tobacco stores and certain private clubs or lodges from regulation.

To accommodate the individuals that smoke, smoking is permitted outside of the

east entrance of the building at least 25 feet away from the entrance. Building Security personnel will remind individuals who attempt to smoke in the building or within 25 feet of an entrance that such activity is strictly prohibited.

We are confident that this no smoking policy will serve the common interests of all

tenants at 181 West Madison. We ask for your cooperation in complying with the no smoking policy. If you have any questions, please direct them to MB Real Estate (312) 558-3800. Thank you.

SECTION III - BUILDING OPERATIONS AND SERVICES Building Signage The building will initially provide floor directory signage with the company name,

suite number and directional arrows. All signage in common area corridors must be of a consistent quality and design. If you require additional signage to be installed, please contact the Management Office for costs and further assistance.

Building Access All tenant’s employees will be issued a Building Photo identification card when

they are hired or move into the Building. This card is an important part of our overall security efforts. All new tenants must return the form on Section XI. A, (“Security Card Key Form”) indicating all authorized employees that are to receive a Building Photo Identification card. MB Real Estate is committed to providing comprehensive security for the tenants of 181 West Madison. To carry out this commitment, strict adherence to the building access policy must be followed:

7:00 a.m. – 6:00 pm. Monday – Friday Everyone who has a Building Photo identification card must show it to the security

officers stationed at the first floor elevator lobby. All visitors must check in, by visiting the Visitor Center or by using the touch screen directory system located at each end of the lobby desk. After confirming they are allowed access, the visitor will need to provide proper identification to receive a dated visitor’s badge. It is recommended that you pre-register your visitors in advance by using our Visitor Check system which is located through the web site www.181madison.com. This system allows for an efficient check-in process. Please see Section X for specific instructions on using the PassagePoint system. If you do not have a password please contact the Management Office at (312) 558-3800.

6:00 p.m. – 7:00 a.m. Monday – Friday, All Weekends & Holidays Everyone entering must stop at the lobby desk and have their Identification Cards

checked. Visitors will only be allowed to enter a suite escorted by someone from that office. Visitors are required to sign in and out and provide valid identification.

Postal Services & Delivery The mail is delivered to each suite on a daily basis. Additionally, delivery

messengers can also deliver packages directly to your office. Additionally, express mail drop boxes including Federal Express, DHL

(international only), and UPS are located in the lobby on the ground floor, near the Sundry Shop.

Cleaning Services Cleaning and janitorial services at 181 West Madison are provided by a high

quality contractor that staffs the Building with management input based on occupancy and Tenant needs.

Presently within the building during the day are a Day Supervisor and a staff of

three, to respond to tenant's needs. At night, after 4:00 p.m., a Night Cleaning

Supervisor is also on duty. Should you wish to contact a cleaning supervisor or request service, please call the Management Office at (312) 558-3800.

If you need to have a large or unusual item discarded by the evening janitorial staff,

there are multi-lingual orange stickers, available in the Management Office, which should be affixed to the appropriate items to be discarded.

Heating and Heat, cooling, and ventilation are adjusted to provide comfortable Air Conditioning Services occupancy conditions and are provided free of charge Monday - Friday, 8:00 a.m.

to 6:00 p.m. and Saturdays 8:00 a.m. to 1:00 p.m. upon request. Service may be supplied on Sundays and holidays at an additional charge. Requests for information of service should be directed to the Management Office at (312) 558-3800.

The building policy is to comply with all governmental regulations that may be

imposed regarding energy and/or temperature restrictions. Your cooperation is appreciated.

After-Hours Heating Heating and air conditioning can be furnished for an additional and Cooling charge, during off-hours, if requested in writing 48 hours in advance by a tenant.

For specific rate information, refer to the building services price list included in Section VI. The rate is subject to change without notice.

Maintenance Requests Maintenance requests should be made through the web site www.181madison.com

or may be phoned in to the Management Office at (312) 558-3800, which will be dispatched to the appropriate staff personnel.

Upon the commencement of any maintenance by our staff, a Workorder form

(Section VII) will be filled out by building personnel. Both the tradesman and you will sign this Workorder when the work is complete. A copy of the Workorder will be given to you at the time it is signed.

Please note that a Workorder is not an invoice. If the Work order is chargeable,

you will be billed with the monthly rental statement. Maintenance rates are included on the building services price list found in Section VI.

Shipping and Receiving The receiving dock is located in the alley behind the building, between LaSalle and

Wells Streets. There are five (5) covered loading berths. This area is open Monday - Friday from 7:00 a.m. to 3:30 p.m. except holidays. If a large shipment is expected during off-hours, arrangements should be coordinated with the

Management Office at (312) 558-3800. The loading dock may be made available at other times, by written request to the Management Office.

Each tenant should make arrangements with its own carrier for shipments leaving

the building. If a tenant would like to have freight moved down to the dock, the Management Office can arrange for this service and will bill the tenant.

All package deliveries must be taken directly to tenant’s offices. Building

personnel cannot sign for shipments, nor can they accept shipping charges. All freight and service deliveries must be restricted to the freight elevator only, and

are not permitted on passenger elevators. All deliveries must be removed from the dock area immediately. There are no

facilities for storing shipments overnight and no responsibility will be assumed for material left in the dock area.

To provide the best service on the loading dock, the following rules should be

adhered to:

The loading dock is available on a first-come, first-serve basis for a thirty (30) minute maximum delivery time. If the dock is needed for longer than 30 minutes, such as for the delivery of furniture, the Management Office should be contacted first for approval of the necessary time allotment.

Approved suppliers, (i.e. bottled water, vending machine operators, etc.) will

be allowed to leave their trucks in the loading dock area while they make deliveries.

There is no room in the loading dock for service trucks servicing equipment

(i.e. telephone or computers.) The dock area is monitored by security and will not allow extended parking in that area.

Major Deliveries Furniture deliveries and moves must be made on weekends or and Moving after 5:00 p.m. on weekdays and must be scheduled through the Management

Office at least 72 hours in advance. A service elevator can be reserved for a tenant's exclusive use by contacting the

Management Office at least three days prior to the move date. There is a charge for this service, as a freight elevator operator will be assigned to manually control the service car.

A certificate of insurance indicating the extent of the moving contractor's coverage,

naming MB Real Estate Services Inc., 181 West Madison CF Borrower, LLC, GE Asset Management, and Bank of America as additional insureds, must be sent to the Management Office prior to the move date. MB Real Estate reserves the right to reasonably withhold approval of individual moving contractors.

Moves by company employees are not permitted. Protection of the building

carpets, walls and elevators is required and must be provided by the moving contractor.

Shoeshine/Repair 181 West Madison has an on-site Leather Care Specialist to handle all of your

leather care needs. He can shine your shoes or make minor repairs to any type of leather products. To reach our Leather Specialist, call his cell phone number (312) 399-4249 or email him at [email protected]. He will then either call back or go to the suite number requested by you. Please be sure to leave your name with your receptionist, so that she can direct the Leather Specialist to you.

the lobby’s southwest corner. Telecommunications Current Communications (708) 786-3400 Current Communications is the authorized vendor to install any cable in the

Building vertical shaft space. Tenants are welcome to use any communications vendor they select, however to maintain control over this critical riser area we are requiring that all requests be coordinated through them. They will maintain the integrity of the riser for all tenants. They may also be able to assist you in identifying cost effective methods by using existing available cables that are already installed in the riser. Please contact them for all work requiring additional line installations within the Building.

Parking Garage There is limited parking available in the lower level of the building. Please contact

the Management office. There are also two public garages located on the Northeast and Southwest corners of Wells and Madison Street. Monthly and daily parking is available at these sites.

Discounted Parking MB Real Estate has secured discounted monthly parking with Standard Parking at their public garage located at 201 W. Madison. Our current group monthly rates are discounted to $350.00 per month, plus a one-time non-refundable $40 transponder fee. Please call the management office at (312) 558-3800 if you are interested. Please see Section IX for further details and the required form for this parking. Completed forms must be returned to the Management Office. Prices are subject to change.

Discounted daily parking coupons can be purchased for the 201 W. Madison

garage from the garage office on the 2nd floor. Packs of 10 are offered for $180. Coupons are for 24 hours and valid for 6 months. Building ID or Business Card showing proof of tenancy at 181 is required.

SECTION V - BUILDING SECURITY

General Information 181 West Madison is staffed with thoroughly trained, licensed Security personnel.

Security personnel enforce building regulations, maintain order, investigate building alarms, screen personnel entering and exiting the building, and are on alert for any unusual activities within the building 24 hours per day, 7 days per week.

Security within a tenant's space is the responsibility of the tenant, although Security

personnel will provide assistance when requested. During non-business hours, Security officers conduct routine patrols of the public areas of the Building at irregular intervals.

The following pages of this manual list the building rules, regulations and

procedures for various emergencies which, hopefully, will never occur in your suite or at 181 West Madison.

Please contact the Management Office at (312) 558-3800 should you have any

questions or problems regarding security issues. A complete list of rules and regulations is incorporated in each tenants' lease document.

Entrances – All entrances open at 7:00 a.m., Monday – Friday. The east and west lobby doors are secured and locked at 6:00 p.m, and the Madison

Street doors (except for one) are secured and locked at 6:30 p.m. After Hours Access MB Real Estate is committed to providing the most comprehensive 24-hour per

day security for the tenants of 181 West Madison. To carry out this commitment, strict adherence to the building access policy must be followed.

Everyone accessing the building must have a Building Photo Identification card.

All visitors are required to check in at the Lobby desk. Please see procedures for Visitor Registration below. No one will be permitted access to the building between 6:30 p.m. and 7:00 a.m. on weekdays, all hours during the weekends and legal holidays unless they possess a 181 West Madison Building Photo Identification card. This policy enhances 181 West Madison's existing security system and assists you in keeping your space secure. Cards will be issued at time of move-in and hiring of any new employees. Replacement cards will be charged at $20.00 each.

Visitors also may check-in at the Visitor Center at the lobby desk. Please call the management office at (312) 558-3800 to receive a user id for PassagePoint. Please see Section X for specific instructions on using PassagePoint.

Solicitation For the convenience of our tenants, 181 West Madison maintains a policy that

solicitors are not allowed in the building. We would like to reemphasize this policy because it is known that some solicitors conduct their solicitation merely as a pretext to survey tenants' premises for possible theft.

Be aware of the presence of strangers on your floor or in your office suite.

Immediately report this type of observation to the Management Office or to Building Security. 181 West Madison's Security personnel are instructed to direct the guests of our tenants to their offices when they ask for that tenant by name.

The "no solicitation" policy is intended to protect tenants from exposure to theft

and to prevent unwanted interruption of their business. Please report solicitors to Building Security by calling (312) 558-3800.

After-Hours There are often times when tenants are required to work after hours, frequently Security Checks alone. In an effort to ensure tenant security, we ask that all tenants working after

hours call the Security Desk at (312) 558-3806 and inform the guard of your presence in the building. Thereafter, a guard will visit or phone you on an hourly basis to make sure you are secure.

Making Security Reports In order to provide an efficient system in reporting security problems (i.e. intruders, solicitors, need of an ambulance, disorderly person, etc.) the following sequence should be used for reporting a problem.

On a 24-hour/7 day a week basis call the Security Desk at (312) 558-3806. Make

as detailed a report as possible and give the Security officer at least the following information:

Should the need arise for an ambulance, follow the procedures listed:

1. STAY CALM and DO NOT PANIC. 2. Call 911. This is the Chicago Fire Department rescue squad. Give the

following information: a) The name of your company. b) The BUILDING ADDRESS (181 West Madison) c) Your FLOOR and SUITE #. d) Describe the problem.

3. Call Building Security at (312) 558-3806 state your company name/suite number, and tell them to stand-by for an ambulance.

Security will have an elevator ready for the rescue squad upon their arrival and

escort them to your office. Tenant Office Security Building Management and Security will make every effort to provide a safe and

problem free environment for its tenants. Individual tenants are responsible for their demised space.

The following is a list of points suggested by the Chicago Police Department and

other security consulting firms which will help secure your office:

1. Keep corridor doors closed. Receptionist and other personnel nearest your corridor doors should be alert for unauthorized visitors. Make sure that the corridor door is closed and locked when leaving at night. Do not depend upon the door closer when leaving at night. When there is no one in the office, do not leave the door unlocked, even for a short time.

2. Handbags, coats and other articles of value should not be left unguarded in

reception rooms, even for a few minutes. Portable articles, when left on desks

or in open drawers, or even in unlocked drawers are easy for the expert sneak thief.

3. If your firm plans to close for a special holiday or plans an early closing on a

particular day, notify the post office to discontinue deliveries. Delivered items outside the door are a sure tip-off that the premises are unoccupied. Theft from within the building is possible.

4. Special care should be taken during the times best suited for pilferage - 30

minutes just after opening, at lunch hours, and before closing when there is maximum movement of personnel and absence from work areas and offices.

5. Notify the Security Desk when loiterers are observed in corridors or

washrooms. Report all peddlers and canvassers immediately. 6. Serial numbers of major office equipment should be recorded to aid police in

recovering property in the event of loss or theft. All calculators and dictation equipment should be kept in locked storage overnight.

7. Be sure to see that any combination is thoroughly mixed when closing a vault

or safe. Do not leave a memorandum of vault or safe combinations on desks. 8. Building Management is not responsible for loss of tenants' personal property.

Personal items should be locked in a secure area at night or whenever an employee is away from his/her desk for an extended length of time.

Also, please have all personnel removing items such as office furniture and

equipment from the premises obtain a Property Pass (Section VIII). The Property Pass is necessary to remove anything from the building, and an authorized employee must sign the pass. All tenants must submit a Property Pass Authorized Signature List, located in Section XI. B. Please fill out and return to the Office of the Building with the authorized names. A supply of Property Passes can be obtained from the Management Office.

After-Hours Emergency In case of emergency, Security personnel may be required to contact Contact List someone in your office. Please fill out the Emergency Contact List in Section XI.

ORGANIZATION

PURPOSE

The purpose of the Emergency Preparedness Plan is to explain in writing the responsibilities, procedures and duties that need to take place in an emergency. Efforts must always focus on safety in order to prevent fires and other emergencies; however, despite these efforts, fires and other emergencies may happen. That is why this plan, if used properly in conjunction with training, will help 181 West Madison recover from emergencies with a minimum amount of damage and disruption.

EMERGENCY INFORMATION

In case of an emergency, the Building Management staff will announce the emergency aloud, by phone or over the building’s Public Address system (PA). Speakers are located throughout tenant spaces. These speakers are used during drills and are tested and maintained. If you do not hear an announcement from a speaker near you please let the management office know as soon as possible.

RESPONSIBILITIES

The Building’s Chief Engineer and/or the Security Director are designated as the Emergency Plan Administrators and are responsible for activating the Emergency Preparedness Plan. In his/her absence, authority is delegated to the Assistant Manager and Building Engineer. The new Chicago Ordinance requires that selected employees (Engineers and Security officers) attend special seminars and take an exam prior to being certified as the Emergency Plan Administrator.

During evenings, weekends, and holidays, the Engineer and Security Officers will be in charge until an above listed representative arrives. Upon notification of a possible emergency, the switchboard operator in charge will notify the person(s) to activate the plan. When authorized to activate the plan, the emergency will be announced over the P.A. system using the Code(s) as listed in this plan.

Every floor or office suite should have its own emergency response team. The team should consist of a Floor Captain/Assistant Floor Captain, Stairwell Monitors and Searchers and Elevator Monitors.

Floor Captain/Assistant Floor Captain: The function of the floor Captain is to operate under the general supervision of the

Chicago Fire Department and the building management team and to supervise emergency measures for the safety and welfare of occupants on his/her floor, department or office. The assistant floor Captain shall assist the floor Captain in all areas of responsibility and assume the leadership role in his/her absence. The floor Captain/assistant floor Captain's responsibilities include: Notify building management, (312-558-3800) security and fire department (911) of an

emergency. Nominate personnel for key emergency positions that are vacant. Maintain a current roster of all searchers, stairwell monitors and alternates. Inform building management office of changes in emergency personnel. Provide building management with a roster of emergency personnel. Alert key emergency floor personnel of potential emergencies. Periodically review emergency procedures with the building management team. Ensure that special emergency procedures are implemented and followed on assigned

floor. Preplan the egress handling of handicapped/physically impaired personnel. Be responsible for evacuation of floor personnel. Maintain communications with key personnel within the building during time of emergencies. Know details of emergency preparedness procedures. Know location of all exits on the floor. Know location of all fire equipment on the floor. Be able to operate fire equipment. Know location of all light switches and other emergency disconnect switches.

Stairwell Monitors Stairwell monitors will be under the direct supervision of the floor Captain/assistant

Captain. Stairwell monitors are responsible for the orderly and safe evacuation of all occupants located on his/her floor. There will be one stairwell monitor at each stairwell exit. Responsibilities include: Be familiar with the location of all stairwell exits on your floor. Maintain an orderly evacuation down the stairwell and keep people moving safely. Know details of emergency preparedness procedure. Do not allow any occupant to use the elevators. Direct everyone to the stairs. Know location of fire extinguishers if you have been trained in using them. When orders are received for evacuation to begin, the stairway monitor shall: Report to designated post, open stairwell exit doors and direct occupants in

stairwells. Keep occupants in a single file holding on to handrails. Check his/her stairwell landing and make sure the immediate vicinity is not

obstructed or filled with smoke. If any problem arises, immediately report it to the fire Captain. Also, stop anyone from using obstructed stairwell and direct occupants to the alternative stairwell.

Remain at the post until the floor Captain/assistant Captain informs him/her that all occupants have been evacuated. He/she should then close the stairwell doors and continue downstairs to the evacuation site.

Searcher The searcher works under the direction of the fire Captain/assistant Captain to find and

evacuate all personnel from his/her area of responsibility and direct them to safe areas. The size of the floor or office will determine how many searchers are needed. The searcher's responsibilities include the following: From a common starting point, proceed in opposite directions and advise all

personnel to proceed to the nearest emergency exit. If there is only one searcher, he/she should start from the rear and move towards the exit. Using a calm voice can minimize fear and panic. Coordinate with floor Captain the evacuation of personnel with physical disabilities who require special assistance

Check all rooms, including restrooms, conference rooms, storerooms, file rooms and remote areas on the floor. Advise any remaining personnel on floor of emergency and insist on their evacuation. Evacuate visitors found on the floor to the nearest stairwell. Know details of emergency preparedness procedure. Know the floor or office structure and the location of all individual offices and

stairwell exits. After searching offices and rooms, place a “post-it note” at the bottom of the closed

door. This will let the fire department know that the office or area has been searched and no one is there.

Know location of all fire extinguishers if trained to use them. Know location of all light switches and emergency disconnect switches. After all areas are clear report to the floor Captain/assistant Captain and let them

know that all occupants have been evacuated. He/she should then go to the stairwell exits close the stairwell doors and continue downstairs to the evacuation site.

Elevator Monitors/ Assistants to the Disabled The Elevator Monitor and Assistant to the Disabled also work under the direction of the

fire Captain/assistant Captain to ensure that no one uses the elevators in a fire drill or an evacuation. Assistants to the Disabled will establish a plan specific to the needs of the person they are assisting. The floor captain will determine how many Elevator monitors and Assistants to the Disabled are needed. Their responsibilities include the following:

Report to their assigned location on the floor. Keep all occupants and visitors out of the elevators. Assist Disabled persons as required. Notify the Building staff or fire department of location of disabled person, if not able to descend stairs. After all areas are clear report to the floor Captain/assistant Captain and let them know that all occupants have been evacuated. He/she should then go to the stairwell exits close the stairwell doors and continue downstairs to the evacuation site.

TRAINING AND DRILLS

This Emergency Preparedness Plan will be reviewed with all staff on an annual basis and when a staff member is newly hired. 181 West Madison is a category 2 building according to the City of Chicago therefore, training drills will take place annually and will involve all staff. Documentation of all training and drills will be kept by the Building Manager. Whenever possible the local fire department should be included in the Emergency Preparedness Plan's training and drills.

FIRE AND ALARM SYSTEMS

THE AUTOMATION SYSTEM

181 West Madison has incorporated a state-of-the-art building Automation System, including the Life Safety System.

Smoke Alarms Smoke detectors are located in each electrical closet and in various locations on each floor. The smoke detectors operate by a photoelectric cell in conjunction with a heat-sensing unit. If the unit "senses" fire or smoke, the Automation System immediately goes into fire mode.

Fire Mode In fire mode, the air handling system will remove smoke from the effected areas and be directly routed outdoors. This avoids the smell of smoke within the building and assists in clearing the effected area of fumes.

FIRE AND LIFE SAFETY SYSTEM

The Fire and Life Safety System is located on the first floor at the lobby desk. It works with the mechanical system in the building to alert security of a potential fire or tampering with the system. Smoke detectors and flow switches (which are attached to the sprinkler system and send a signal in the event of a fire or trouble) notify the Life Safety System of any unusual amounts of smoke in a particular area or movement of water through the sprinkler system.

Electronic Door Releases If a fire occurs, a signal from the Fire and Life System will unlock all stairwell doors, allowing the occupants to leave the building.

Simplex Monitoring If a fire alarm is activated, a signal is sent to a central station that immediately notifies the Chicago Fire Department.

Public Address Speakers Building management or the Fire Department can notify occupants of a necessary evacuation by using public address speakers that are dispersed throughout the building. Occupants can be notified if there is danger on a specific floor or in a specific area on the floor and where they should proceed.

Sprinkler System Every occupied area of the building has a sprinkler system in the ceiling. The sprinklers are activated at a high temperature indicating a fire is present. Each sprinkler head operates independently to avoid water damage in unaffected areas.

Fire Extinguishers ABC type fire extinguishers are located in the both stairwells and in the fire cabinets on each floor. ABC type fire extinguishers can put out most types of small fires that might occur in an office building. Fire extinguishers are ineffective on major fires and should only be used on small fires. Fire extinguishers should only be used by those people that have been properly trained

Stairwells There are two stairwells in the building. They are labeled A and B. Because the elevator and washroom locations vary on groups of floors the stairwell exits also vary. Listed below is a general description of the stairwell exit doors locations. Each floor has two stairwell exits and everyone on your floor should know the location of both in the event that one is not accessible.

On floors 4-34, use the stairwells on the north and west side of the building.

On floors 35-41, use the stairwells on the west and east side of the building.

On floors 42-49, use the stairwells on the east and southwest side of the building, just west of the elevators.

There are two stairwells at each floor and they are labeled Stair A and Stair B. The following page shows typical Floors Plans with typical evacuation routes.

Emergency Phones Emergency phones are located in the stairwells of floors 4, 8, 12, 17, 21, 25, 29, 33, 37, 41, 45 and 50. These phones are direct lines to the Lobby Desk. You may also call Building Security in an emergency at (312) 558-3806.

FIRE ALARM PROCEDURE

When a smoke detector or a flow switch is activated, the fire panel located at the lobby desk will go into alarm. When this happens, the security and engineering staff will investigate or a call will come over the Public Address system. All occupants must follow the directions for an evacuation if called at that time.

TROUBLE ALARM PROCEDURES

The Fire and Life Safety System at 181 West Madison can detect trouble alarms on the fire control panels. A trouble alarm will indicate to the Building staff that a device, such as a smoke detector or speaker is not functioning correctly. The device either will need maintenance or a wire was damaged somewhere causing the device to send a trouble signal to the panel. The Building staff will investigate these trouble alarms and make the necessary corrections.

EVACUATION

PURPOSE

The primary purpose of our evacuation procedures is to establish guidelines for a safe and orderly evacuation process should the need arise.

Evacuation procedures must be followed by all personnel as outlined within this plan or that may be issued by the person in charge.

Evacuation procedures have been established for total evacuation from the facility.

NOTIFICATION AND ASSEMBLY AREAS

The local fire department will be notified of the need to evacuate unless it is already involved and the Emergency Plan Administrator will direct the evacuation. If you are required to evacuate down to the ground floor we have established the following meeting areas. Your company may select an alternative meeting area. If you do select an alternative site, please let building management know as soon as possible.

The evacuation assembly areas are:

Stairwell B exits on the first floor on the west side of the Building. Occupants using those stairs should meet at 1 N. LaSalle, unless directed otherwise by the safety or fire personnel.

Stairwell A exits the first floor on the east side of the Building. Occupants using those stairs should meet at 1 N. LaSalle, unless directed otherwise by the safety or fire personnel.

If upon evacuation of the building temporary shelter is needed, personnel will be taken to 1 N. LaSalle. Agreements with the other buildings will be renewed annually by the Administrator. Attached is the first floor plans showing where the stairwells exit.

EVACUATION PROCEDURES If a fire emergency exists on your floor notify the Chicago Fire Department (call 911) and then call the Building Management office (312-558-3800). Indicate exactly where the emergency is occurring by specifying floor, suite, or section of the floor. The building staff will then assist the Fire Department in gaining access into the building and getting the fire department to your floor. If you feel an evacuation is necessary please let the Building Management know as soon as possible.

EVACUATION MESSAGE

If the building is evacuated, the following message should be read in a calm, clear voice.

Attention. Attention please. There is an emergency situation on the ____ floor. Please evacuate the floor immediately using either the north or west stairwell. DO NOT USE THE ELEVATORS. Please descend a minimum of two floors (or lobby level if instructed to do so) to the ____ floor and exit the stairwell.

If an emergency happens during non-business hours, the message should be read in Polish, Spanish and English. You will need to contact the cleaning supervisor and a Spanish-speaking member of his/her staff. If the cleaning supervisor is not available or does not speak Polish, contact someone who does.

FIRE PROCEDURES

PURPOSE

The primary purpose of our fire procedures is to establish guidelines to follow in the event of a fire.

The prevention of fire is the most important aspect of any fire program. Therefore, it is important for all personnel to follow established fire safety procedures and rules.

UPON DISCOVERY OF FIRE - FOLLOW THE R.A.C.E. PROCEDURES

Remove anyone in the room. Close the door to the fire room. Activate the fire alarm. Call Chicago Fire Department (911) and building management (312-

558-3800) Indicate exact location of fire. Close all remaining doors in the fire zone. Extinguish, if possible, or

Evacuate as directed by person in charge.

NOTES

If a fire or smoke is detected in the building, contact the Floor Captain/Assistant Floor Captain immediately.

Order of the above steps is somewhat flexible. However, the evacuation of the room's occupant(s) and confinement of the fire shall be the top priorities.

If evacuation is necessary, do not use the elevators. Use the nearest stairwell to descend at least two floors unless otherwise instructed.

Avoid crowding or undue haste. Descend stairs carefully, for a fall might injure you and those who are following you. When approaching the next landing, prepare to move from the center towards the inside of the stairwell to allow evacuees from that level to move onto the outside of the stairwell. Occupants should remove high-heeled or platform shoes when descending the stairs.

Remember to direct visitors who are not familiar with the building. Occupants should not re-enter the building in the event of a full evacuation until directed to

do so by the Building Manager or the fire fighter in command. Once the Chicago Fire Department arrives, members of the fire safety team should

relinquish their duties to the Fire Department. Because of mechanical and automated life safety systems in the building, building personnel

will probably be aware of emergencies before they receive calls from the affected areas. It is wise to report problems anyway; there is no such thing as being too safe.

If the room is filled with smoke, get down on the floor where there is more clean air and take short breaths through the nose.

Before opening any door during a fire, place your hand on the door. If the door is hot, there is probably a fire on the other side. DO NOT OPEN THE DOOR. If the door is not hot, open the door slightly. If smoke escapes, CLOSE DOOR IMMEDIATELY. If no smoke escapes, cautiously proceed.

4.2

RESPONSE TO THE ALARM ANNOUNCEMENT

Staff in the immediate area:

Activate the fire alarm. Assist in the evacuation of the fire room and confinement of fire. Close doors and windows and turn off electrical equipment in remaining rooms in the fire

zone. Clear corridor of equipment.

TYPES OF FIRE EMERGENCIES

Sensing smoke If someone senses smoke but cannot see it, immediately notify the Floor

Captain/Assistant Floor Captain who will then notify security at (312) 558-3806. Security will notify building management and engineers and proceed immediately

to the effected area. If an evacuation is necessary, occupants will be notified through the speaker system.

Minor Fire Use the portable fire extinguishers located in the stairwells and fire cabinets

located on each floor, if you are trained in the use of fire extinguishers. Notify security at 558-3806 and notify the Building Management Team. One person should never attempt to extinguish a fire; therefore the floor Captain

or assistant Captain should be contacted for assistance. Immediate action is essential: small fires can become large fires quickly.

Major Fire Call 911 and then security at 558-3806 and inform them of the fire. Notify the floor Captain/assistant Captain of the location and size of the fire and

what action has been taken. Await instructions given over the speaker system or from the Floor

Captain/Assistant Captain.

Acting Supervisor o Page Location of Fire o Place back-up phone call to the fire department (911). Identify yourself and the

facility. Describe the situation to the best of your ability. o Set up Control Station. The Control Station will be responsible for directing

additional staff to the fire area or to other areas within the building, as necessary.

All other Staff o Assist in the evacuation of building. Specific instructions are given to the building

staff.

Administration o Establish a command post if evacuation of rooms other than the fire room is

taking place. The administrative command post will be set up at the lobby desk.

Maintenance o Meet the fire department and inform them of the entry door that will bring them

directly into the fire area without going against the resident evacuation flow.

4.3

IF ALARM SOUNDS FROM ELSEWHERE IN THE BUILDING

Direct appropriate staff to respond to fire area. Direct remaining staff in securing unit (clearing halls, closing windows/doors, etc.)

EXTENDED EVACUATION

General Concept First stage of evacuation will be horizontally past the fire/smoke door. If evacuation of the building is necessary, this shall take place from the non fire

side of the building, using the exit farthest from the fire. Building evacuation will be a fire department/administrative decision

FIRE ALARM SYSTEM

The Fire Alarm system is controlled by these devices:

Smoke Detectors (corridors) Smoke Detectors Range Hood (in certain areas) Heat Detectors (in certain areas)

When the fire alarm system is activated a specific set of events occurs:

All automatic fire doors will close. Fire Alarm bells will ring. Annunciator panel will light up indicating fire area. Emergency signal will be sent to the fire department. Air ventilation will shut down to help prevent spread of smoke and fire.

5.1

TORNADO AND SEVERE WEATHER

PURPOSE

The primary purpose of our tornado and high winds procedures is to establish guidelines to follow when a tornado, hurricane, severe thunderstorms, high winds alert, etc., "WATCH" or "WARNING" is issued by the National Weather Service for our area.

When a "WATCH" or "WARNING" is issued, you must follow our established procedures to assure the safety and well-being of all concerned.

NOTIFICATION OF WATCH/WARNING ALERTS

A tornado or other severe weather "WATCH" means that conditions are favorable for such storms and that certain precautionary procedure must be implemented.

A tornado or other severe weather "WARNING" means that a tornado has been sighted or that severe weather is rapidly approaching and that immediate action must be taken.

When a "WATCH" or "WARNING" has been issued for our area, you will be notified of such alerts, and will be instructed to implement our established procedures.

TORNADO OR SEVERE WEATHER "WATCH" PROCEDURES

When a tornado or severe weather "WATCH" has been issued, the following procedures shall be implemented: Staff will make rounds of rooms. They will close drapes, blinds, etc., unplug any

unnecessary electrical equipment, and store glass and other breakable objects. Staff will get emergency equipment and medical supplies ready, turn on hallway lights,

and monitor weather bulletins. Maintenance will unplug all unnecessary electrical equipment, store all glass and other

breakable objects, secure all outside furnishings (lawn chairs, tables, gardening equipment, trash cans, etc.) by tying down or storing in designated areas.

If office personnel are on duty, they will stay away from windows as much as possible and unplug all unnecessary electrical equipment.

Continue daily routine assignments. However, be ready to implement other procedures when instructed.

Several hours may lapse between the "WATCH" and "WARNING." You should have ample time to perform your assignments.

Upon cancellation of the alert, return to your normal routine duties, and plug in electrical equipment unplugged during "WATCH."

5.2

TORNADO OR SEVERE WEATHER "WARNING" PROCEDURES

When a tornado or severe weather “WARNING” has been issued, the following procedures shall be implemented immediately.

Staff will move into hallways. Keep all personnel away from windows. Have wheelchair personnel sit against the hallway wall. Keep on one side of the hallway ONLY! Stay away from windows Close all doors to offices after removing staff or checking to ensure room is

empty. Reassure your personnel that they are safe. Remain with staff, unless

otherwise instructed. Assigned personnel will double check rooms for evacuation and indicate with tape that

the room is empty. Personnel will keep flashlights readily available. All occupants will follow instructions issued. REMAIN CALM. DO NOT PANIC.

Remain in hallways until an "ALL CLEAR" has been called by the person in charge. Stay away from windows!

DAMAGE TO THE BUILDING

Should damage occur to the building, that is if the facility is hit by the storm, begin to search, in pairs, for injured persons as quickly and safely as possible.

One person will be assigned the task of going to each area to obtain an injury count and reporting such information to the person in charge.

Once the storm has passed, and injuries have been reported, it may be necessary to evacuate the area. However, do not evacuate until the order has been given, unless emergency action warrants other procedures be taken.

CANCELLATION OF "WARNING" ALERT

When the tornado or severe weather "WARNING" has been canceled, you will be instructed to return all personnel and staff to working areas.

Upon cancellation of the alert, return to normal, routine procedures and return all emergency equipment to its assigned location.

6.1

WINTER STORMS

PURPOSE

The primary purpose of our Winter Storm procedures is to establish guidelines to follow during blizzards, heavy snows, freezing rain, ice storms, sleet, etc. Follow these safety precautions as well as any others that may be issued by the Emergency Plan Administrator.

PROCEDURES

Keep posted on all area weather bulletins. Have portable transistor radios available; make sure extra batteries are available. Be prepared for isolation at the facility. Make sure all emergency equipment and supplies are on hand or can be readily obtained. Make sure emergency food supplies and equipment are on hand. Make sure emergency supply of water is available. Make sure emergency power supply is operable. Make sure heating system is operable. Have extra blankets available. Make sure emergency heating equipment is on hand or can be readily obtained. Made sure adequate staff is available. Keep flashlights handy. Have extra batteries available. Keep personnel as warm as possible. Close drapes, blinds, shades, etc., on cloudy days and at nights. Be prepared to remain at the facility. Know the cold stress policy and procedures. Know your job assignments. Travel only when necessary and only during daylight hours. Never travel alone. Use only the travel routes assigned. Report road conditions to the facility. Be prepared to evacuate staff if necessary. Do not make unnecessary trips outside. If trips outside become necessary, make sure you are properly dressed to include head gear,

wraps, gloves/mittens, etc. Avoid overexertion. Cold weather causes extra strain on the heart. Do only what is

necessary. Do not panic. Remain calm.

Contact the facility when transportation is needed. Do not attempt to travel at night unless absolutely necessary. Follow all instructions issued.

6.2

SUPPLY AND EQUIPMENT PICK UPS

Make sure vehicle is in good operating order. Make sure vehicle has an adequate supply of gasoline. Two people must make trips together. Follow routes as assigned or as road conditions allow. Do not speed. Turn back if necessary.

PICK UP OF PERSONNEL

Personnel unable to get to the facility will be picked up by support personnel assigned such responsibilities.

When road conditions are hazardous, the Police and Sheriff's Departments, Civil Defense, National Guard, etc., will assist in picking up occupants when requested.

Any occupant not able to report to work shall call the facility and request such assistance in getting to the facility.

Occupants shall travel only if necessary and only during daylight hours. Do not take unnecessary risks. Call the facility. Transportation will be provided if possible.

Bomb Threat

7.1

BOMB THREAT

PURPOSE

The primary purpose of our Bomb Threat procedures is to establish guidelines to safely follow in the event of a bomb threat. Follow these safety precautions as well as any others that may be issued by the Emergency Plan Administrator.

PHONE PROCEDURES

Whoever takes the call announcing the placement of a bomb will make every effort to KEEP THE CALLER ON THE PHONE and question the caller, making notes regarding the following:

o Time of Call o When bomb will go off o Location of bomb o What KIND of bomb o Description of bomb o Reason for placement o Will it hurt people (tell caller it will). Make sure the caller is aware that the building is

occupied and serious injury or death could result if this information is not obtained o Ask NAME of caller o Ask caller’s location (local, internal, booth, distant). Listen for any unusual background

noises such as music playing, type of music, motors running, traffic sounds, etc, which might be helpful in providing clues to determine where the call was made from.

o Note voice features such as accent, gender, age, familiarity, speech impairments, nervousness, etc.

o Record, as near as possible, every word spoken by the caller. Record as much information as you possibly can. You may not be able to get everything but do get all you can.

o Remember, remain calm. DO NOT PANIC. Panic will only cause confusion.

SEARCH PROCEDURES

As soon as the call is completed, contact the Building Manager or designee and relay as much information as possible. The Building Manager or designee will call the fire department and the police department. The Building Manager or designee is in charge until the fire department arrives and assumes charge.

Staff should remain where they are when code is paged. Supervisors are responsible for verifying the exact location of each occupant. Visitors will be detained from entry to the building. As soon as the Building Manager or designee has called the fire department and police

department, he/she will summon appropriate supervisory and personnel via the PA. He/she gives them available information and asks that they select appropriate staff for a preliminary search. Searchers should go in pairs. Searchers should be those that are MOST FAMILIAR with what belongs in given area.

During the search, particular attention shall be given to all accessible areas to the general public; i.e., window ledges, behind shrubbery, platforms, lobbies, waste cans, restrooms, public telephone alcoves, ceiling lights, vents, corridors, closet areas, etc.

Walk entire assigned area looking for unfamiliar objects that are "out of place" in the environment. Use flashlights as necessary but DO NOT turn lights on or off. DO NOT close/shut drapes and/or blinds.

Loss of

Utilities

8.1

LOSS OF UTILITIES

PURPOSE

The primary purpose of our Loss of Utilities/Water procedures is to restore power and water as quickly as possible while maintaining the comfort and security of the residents. Follow these safety precautions as well as any others that may be issued by the Emergency Plan Administrator.

POWER FAILURE

In the event of failure in the municipal power system, an auxiliary generator will automatically begin operation. This generator will supply power for essential, lighting, and other pertinent equipment.

The emergency power system is tested monthly and any correction is immediately made. Turn off all unnecessary electrical equipment. As soon as the emergency power goes on, the Supervisor tells the occupants what happened

to avoid panic. These lights are checked regularly by the Maintenance staff. Continue with normal duties as much as possible.

Once power is restored, turn on everything that was shut down at one minute intervals; this ensures no heavy electrical load at once to damage the switch gear. Recheck all equipment with tester to ensure everything is working properly.

BURNS AND FIRST AID TREATMENT

WHAT CAN CAUSE A BURN?

Thermal Agents o Flames o Hot Liquids o Steam o Radiation o Chemicals o Acids and Alkalis o Other Corrosives

Electricity

o Becoming part of an electrical circuit (while grounded)

WHAT CAN YOU DO?

Rescue o Remove the person from the source of the potential injury. o If the injury involves electricity, turn electricity off and make sure the victim is not in

contact with the electrical circuit. o If dealing with thermal or chemical agents, remove the person carefully from the

source of injury and move them to a safe environment.

Stop the Burning Process o Remove the substance causing injury. o If clothing catches fire, Stop, Drop and Roll to smother the flames. DO NOT RUN.

It will only increase the flames and extent of injury. o Remove burned clothing and jewelry, which can retain heat and intensify the injury. If

the clothing adheres to the skin, leave it in place and cut or tear around it to remove the source of the heat.

Treat the Burn

o Pour cool, clean water over the burned area for 10-15 minutes. Longer periods are recommended for chemical burns. Do not put ice on the injured area, this may cause greater injury.

Minor Burn Injuries

o These can be treated with commercial antiseptic sprays and covered with a clean, dry dressing.

o If infection occurs, seek medical attention immediately.

Severe Burn Injuries o These burns and burns involving chemicals and electricity should prompt an

immediate response from your local Emergency Medical Service System, and/or the Fire Department. More sophisticated medical care is mandatory for any type of severe burn injury.
