A denture that has been worn satisfactorily

for a period of time may become ill-fitting as

alveolar resorption proceeds. The patient isable to continue wear the denture since

neuro-mascular control is gradually learnt.

Elderly patients may never learn to control anew denture, if the contours of the later,

particularly the shape of its polished

surfaces, differ greatly from those of the old

appliance.

Summary of the Indications Replication of the dentures for a spare set.

Replacement of the dentures with provision of: New impression surface, and /or

New occlusal surface, and/or

Slight modification to the VD.

Replacement of dentures whose base material hasdeteriorated.

Production of temporary dentures that can be progressivelymodified if the patients capacity to adapt is in doubt. (slightincrease to the VD or if the cause of the complaint is not

clear/denture collectors). Once a satisfactory appliance hasbeen achieved, it can be copied to produce a definitivedenture

Clinic 1

History & assessment

Assess the old dentures

and decide those features

to be copied and those to

be modified.

Technique Correct any under

extensions by addition of

tracing compound.

The occlusal & polished

surfaces are invested in theselected mould material.

The set material is trimmed

in straight line with the

borders of the denture.

The exposed impression

surface of the denture is

covered by another mix of

the mould material beforethe lid of the container,

filled with more mix, is

seated.

Once set, the container isopened , any tracing

compound is removed,

the denture is cleaned

and returned to thepatient.

Lab 1

Sprue holes, approximately the

diameter of a pencil, are cut inthe alginate from the mostdistal surface to the exterior.

Modeling wax is melted intothe impression of the teeth tothe gingival margin.

After re assembling, self-cureacrylic is poured down onesprue hole to allow airescapement from the other.Once polymerized, thewax/acrylic template isremoved , trimmed andsmoothed.

Clin ic 2:

Impression should be taken as described for

relining techniques. Silicone impression material

is preferred since the template have to be re

inserted in the mouth.After the impression is made, wax is added or

removed to the occlusal surface as needed.

The desired occlusal relation is registered.

The shade and mould of the teeth are noted.

Lab 2

Casts are poured from

the impression surface

and mounted on the

articulator using the

registration provided.

Wax teeth are removed

one or two at a time and

replaced with acrylic

teeth. The templates arethen prepared for trial

insertion (Try-in stage)

Clinic 3

The modified templates are tried in the mouth and

their border extensions, occlusion, jaw relationship,

and aesthetics are noted. Post dame is determined

and carved on the upper master cast.

Lab 3

The templates are flasked and the self cure acrylicbase is removed before the moulds are packed with

the conventional denture base acrylic. The

polymerized dentures are prepared for insertion .

Clinic 4 The dentures are inserted and checked as usual.
