2. Click the DOWNLOAD button to download your Registration Form

Fill Up the Registration Form

1. Encode the required information in the Registration Form then Save

2. The Registration Form must be filled up electronically

3. All entries in the Registration Form are required unless marked as optional

4. Incomplete entry will invalidate your Registration

Payment can be made through RCBC Savings Bank or pay directly to the PUP Information and Communications Technology Office (ICTO):

1. Go to any RCBC Branch to remit payment via ONLINE COLLECTION

2. Fill up Deposit Slip with the Account Number, Account Name, and Amount to be deposited

3. After processing your payment, the ICTO personnel will give you the Official Receipt. Keep the Official Receipt

Email Registration Form and Deposit Slip/Official Receipt

Deadline of submission of Registration Form and Deposit Slip/Official Receipt is on FEBRUARY 19, 2013

1. Mail your accomplished Registration Form and Deposit Slip/Official Receipt to [email protected]

2. Type 'Technology Festival 2013 Registration' as the subject of your email

3. Attach your accomplished Registration Form and scanned copy of your Deposit Slip/Official Receipt

4. Send it to [email protected]

5. Please allow three (3) working days after uploading your registration before claiming your Technology Fest ival 2013 Entrance Ticket

Claim Ticket Online

1. Go to Technology Festival 2013 Claim Ticket page http://apps.pup.edu.ph/Registration/

and type the required information. Please make sure you type the same information encoded in the registration form.

If you belong to a group registration, each registrant must claim the Entrance Ticket individually.

2. Click the DOWNLOAD button to download your Technology Festival 2013 Entrance Ticket

3. Print your Technology Festival 2013 Entrance Ticket

Note

Please read all information printed in your Technology Festival 2013 Entrance Ticket.

The ticket contains the date, time, venue, and any other information about the PUP Technology Festival 2013.

You will present this ticket to the Registration Officer and Ushers/Usherettes during the event. NO TICKET, NO ENTRY

You must be i n the venue at least 30 minutes before the Event schedule. LATE COMER PARTICIPANTS are not allowed to enter.

Remember: Do not misplace these documents:
