Here are the acceptable forms of receipt:

‐ Dog License Place your rabies cer ficate (if needed) and
payment in the dropbox Include a self addressed stamped envelope
Village staff will return your license in the mail
‐ Lo ery Credit If the lo ery credit is missing from your bill
and
you lived in the home as of 1/1/2020 and the home is your
primary residence, please complete form LC 100 available in the
Village Hall ves bule and return with your payment.
If the lo ery credit is missing from your bill and the home was
purchased a er 1/1/2020 but you know the prior owner used the home
as their pri‐mary residence, please complete form LC 110 available
in the Village Hall ves bule and return with your payment.
If you moved into a brand new home in 2020, you are not eligible
for the lo ery credit un l 2021.
Reminders...
‐ Check Endorsement If the check is made out to the Village of
DeForest
AND you (homeowner), all names listed on the check will need to
endorse the back of the check.
If the check is made out to the homeowner only, the homeowner
will need to write “PAY TO THE ORDER OF THE VILLAGE OF DEFOREST”
and sign the back of the check.
‐ Payment Dates Payments in the dropbox or USPS postmarked
by
December 31, 2020 will be credited to 2020. Payments postmarked
or received a er December 31,
