restenosis on inflow lesions of autogenous radiocephalic fistulas: a pilot study.
PURPOSE: To determine whether the use of a paclitaxel-coated balloon (PCB) improves patency in
patients undergoing percutaneous transluminal angioplasty (PTA) for recurrent juxtaanastomotic stenosis of radiocephalic arteriovenous fistulas (RCAVFs).
MATERIAL AND METHODS: prospective study hemodialysis patients with two short (< 2 cm) and separated inflow
RCAVF lesions.
After dilation of lesions using a 4-mm plain balloon (PB), half of the lesions were randomly selected for treatment with PTA using PCB (size, 4 mm; length, 2 cm) and PB (size, 5-mm or 6-mm) (group 1), and the other half were treated with PTA using PB alone (group 2).
After the index PTA, dysfunction-driven angiography was performed to confirm target
lesion restenosis (TLR). TLR and lesion patency were compared. RESULTS: The analysis of 20 lesions in 10 patients revealed that the TLR-free duration in group 1 was
significantly longer than the TLR-free duration in group 2 (251.2 d vs 103.2 d; P < .01). The
patency rate of the target lesion was significantly higher in group 1 than in group 2 at 6 months (70% vs 0%; P < .01) but not at 12 months (20% vs 0%; P > .05).

arteriovenous fistulas: our experience in the treatment of juxta-anastomotic stenoses. PURPOSE:
primary and secondary patency (PP and SP) of juxta-anastomotic stenoses of failing radiocephalic hemodialytic arteriovenous shunt.
METHODS: 26 consecutive patients with juxta-anastomotic stenosis of radiocephalic hemodialytic
shunt treated with angioplasty with drug-eluting balloon. Main objective PP defined as the absence of dysfunction of the vascular access, patent
lesion or residual stenosis <30% and no need for further reintervention of the target lesion
(TL). PP and SP at 6, 12 and 24 months evaluated with echo color doppler and phlebography, for both arteriovenous fistulae, defined as absolute, and TL.
RESULTS: 100% Immediate postprocedural technical and clinical success
only one technical failure in repeated treatments. absolute and TL PP 96.1% At 6 months at 12 months the absolute PP 81.8%, TL PP 90.9%, absolute SP 95.4%, TL SP 100% at 24 months the absolute and TL PP 57.8%; absolute and TL SP 94.7%
only one arteriovenous fistula lost during the period.

▪ afin de montrer une augmentation du taux de perméabilité à 1 an supérieure à 20% dans le bras traité par ballon actif par rapport au bras traité par angioplastie conventionnelle, avec un risque alpha de 1er ordre de 5% et un risque béta de 2ème ordre de 10%
L’objectif principal est de montrer une amélioration de la perméabilité primaire à 1 an de la sténose traitée par ballon actif en comparaison à celle traitée par angioplastie conventionnelle.
Critère de jugement principal

dysfonctions de la FAV en rapport avec la sténose initiale dilatée ainsi que les sténoses à distance du site initialement traité)

temps moyen avant resténose de la lésion dilatée entre les 2 groupes (intervalle libre avant réintervention)
analyser en sous-groupes l’intérêt de ces ballons actifs sur: ▪ Les FAV avec sténoses itératives (≥ 3 /an) vs. FAV ayant moins de 3
sténoses/an ▪ les FAV primitives (n’ayant jamais eu d’angioplastie) vs. FAV ayant déjà
bénéficié d’au moins une angioplastie

1.1.1 Descriptive analysis A first overall descriptive analysis and analysis by group will be performed. This consists of separate estimates,
numbers and percentages for qualitative variables, means, standard error, medians and interquartile intervals for quantitative variables. The normal feature of the distribution of quantitative variables is checked (Shapiro test).
1.1.2 Comparison of groups at inclusion Student’s t test or a Mann-Whitney test if necessary will be used to compare quantitative variables, and a Chi² or
Fisher’s exact test if necessary will be used to compare qualitative variables between two groups at inclusion. 1.1.3 Analysis of the primary endpoint Time to fistulae dilatation (i.e the time between first fistulae dilatation and reintervention on the same stenosis
site) will be compared between the 2 groups at 1 year with a multivariable Cox proportional hazards regression analysis to adjust for known prognostic baseline variable (stenosis > 2 cm in length, diabetes, HT, smoker, age of the fistulae).
1.1.4 Analysis of other endpoints Student’s t test or a Mann-Whitney test if necessary will be used to compare quantitative variables. Quantitative
criteria evaluated several times during the monitoring will be compared with repeated measures ANOVA with two factors (time, group).
For qualitative criteria, a Chi² test will be used. For censored endpoints, the log rank test will be used. For all these analyses, adjustments can be made in case of heterogeneity at inclusion. Results will be stratified according to the type of treatment (DEB vs. plain balloon). 1.2 Planned number of persons to be enrolled in the trial, and planned number of persons in each centre
with statistical justification Our hypothesis is a 1-year patency rate of 40% in the control group using conventional angioplasty with “plain”
balloons6,8,10. A total sample size of at least 262 patients (113 events) is required to achieve a 90% power to detect an increase of 20% of primary patency at 1 year with DEB (HR = 1.794), using a two-sided test with a type 1 error of 5%. (nTerim, V1.1, Statistical solution Ltd, Cork, Ireland).
