Tab A PREPAREDNESS MEASURES AND ACTIONS TAKEN

1. NDRRMC

1.1 On 21 March 2013, issued a memorandum to all Chairpersons, RDRRMCs/OCD Regional Directors directing them to consolidate and review all related contingency plans of national agencies and local DRRMCs for the purpose of identifying potential gaps and arrive at contingent actions for various scenarios and establish pre-emptive plans and measures to avoid any potential disaster.

1.2 On 25 March 2013, selected member agency members convened a meeting for the review of their preparedness measures/actions in anticipation for this year’s Holy Week observance. The meeting dubbed “HOLY WEEK 2013”, sought to harmonize the contingency plans and coordinate efforts between member agencies and LDRRMCs to ensure public safety, transport security, health emergency, as well as peace and order.

1.3 On 26 March 2013, a memorandum was issued to concerned member agencies requesting for detailed duty officer from 27 March to 01 April 2013 at NDRRMC Operations Center.

1.4 On 27 March 2013, the NDRRMC Operations Center was activated and alert status was upgraded from WHITE to BLUE alert status to coordinate all efforts of government agencies for this year’s observance of the Holy Week.

2. DILG issued a Memorandum to all its Provincial/City Directors/Cluster Heads regarding local actions to ensure peace and order and public safety during the observance of the Holy Week 2013; activated LDRRMCs and LPOCs; deployed teams to maintain peace and order, manage traffic and secure transport hubs; Barangay Tanods and Barangay Peacekeeping Action Teams (BPATs) augmented police personnel; assigned garbage collection and disposal teams.

2.1 DILG I activated DILG Emergency Operations Center (DEmOC) for coordinating and reporting of untoward eventualities in their AOR as well as disseminating weather advisory/bulletins to LGUs.

2.2 DILG III issued memorandum to all Provincial Governors, City/ Municipal Mayors, Punong Barangays and to all concerned field officers re observance of Semana Santa 2013.

3. AFP

3.1 Philippine Air Force

a. Two (2) teams with mobility of 1 ¼ Troop Carrier and M35 Cargo truck of the Tactical Operations Group (TOG) 5 are ready to be deployed in case of emergency.

3.2 Philippine Army

a. 9th Infantry Division assisted the PNP in the maintenance of peace and order and in the safeguard of threatened vital installations and facilities.

b. 81st Infantry Batallion, 7 th Infantry Division activated IMPLAN SEMANA SANTA / SumVac.

3.3 Philippine Navy

a. Naval Forces of Southern Luzon adopted the NFSL IMPLAN 01-13- Disaster Response protocol of 2010; NFSL Disaster Response Task Unit subordinated to RDRRMC5 and SOLCOM DRTG; ready for mobilization when widespread disaster happens within AOR; NAVSOU3 is ready to respond on any man-made situation.

b. Naval Forces of Northern Luzon issued Operational Directive 70-1 Contingency Plan re Lenten Season and deployed personnel to augment LDRRMC rescue assets in AOR.

4. PIA I disseminated bulletins / advisory to the public.

5. PCG Action Center (CGAC) is continuously providing updates to all PCG Units on the latest Public Weather Forecast and advisories for gale warnings to warn small boats and fishermen as well as flood and landslide alerts to warn residents in low lying areas and those living near the mountain slopes; as of 6:00 AM, 1 April 2013, a total of 16,090 outbound passengers and 12,684 inbound passengers were monitored and 91 vessels and 2 motorbancas were inspected in different ports of the country.

5.1 PCG Northwestern Luzon, Eastern Luzon, Bicol, Western Visayas, Southern Tagalog, Palawan, Boracay, and Central Visayas are on on heightened alert status in all ports in their respective AOR; implemented Oplan Ligtas Byahe “Kwaresma 2013” and set-up Passenger Assistance Center (PACs) manned by PCG personnel and Special Operations Group to ensure orderly operation of sea transport facilities as well as safe and convenient travel of the public within their AOR; deployed response vehicles and equipment; implemented inspection of baggage and availability of security layers in ports and passenger terminals.

6. Civil Aviation Authority of the Philippines (CAAP) activated its annual information and assistance desk; strengthened defenses against unlawful acts or terrorism attacks on the air navigation equipment located in and out of the airport premises at all CAAP facilities; intensified security and conducted more visible and frequent security monitoring of all airport facilities; instructed Duty Air Traffic Management Officers to immediately send Air Flight Travel Notice messages re flight cancellations or delays.

7. Communications and Warning Group (CWG) members located in different areas continuously monitoring various situations and are available 24/7; circulated different advisories of agencies re influx of people during the summer vacation.

8. PNP provided maximum deployment of public safety services to motorists and general public particularly by conducting intensified security patrol assistance along highways, public roads, site of religious activities, resorts, and other places of convergence during the Holy Week. Implemented Memorandum Circular No 4-2013, SumVac 2013 IMPLAN to NHQ LOI SANG-INGAT-Charlie.

8.1 Police Regional Office I, IV-A, and IV-B ensured police visibility in identified critical areas and established police assistance centers.

8.2 Police Regional Office I implemented traffic routing scheme; intensified intelligence operations; maintained mobile reaction units / teams for immediate response to emergency cases.

8.3 Police Regional Office IV-A assisted the Local Traffic Management Office in conducting traffic management in all identified congestion and accident prone areas.

8.4 Police Regional Office IV-B organized NGO’s, CVO’s and other civic groups to support better traffic management and supervised the implementation of the strategic concept of Community Oriented Policing System (COPS); conducted 24/7 traffic management in all identified traffic congested and accident prone areas; coordinated with AFP counterparts, other government agencies and volunteer groups to establish joint assistance centers for more collaborative delivery of public safety services and assistance; deployed road safety marshals along the main thoroughfares with AOR.

8.5 Police Regional Office VI coordinated with church organizers and other agencies to ensure the efficient delivery of public safety services; distributed flyers on precautionary measures.

8.6 Police Regional Office CAR deployed personnel for tourists and motorists assistance in the whole region; alerted all PPOs in areas commonly visited by tourist; deployed police personnel for tourist assistance and crime prevention in Baguio City.

8.7 NCRPO deployed personnel in passenger holding areas within their AOR and conducted anti-criminality campaign thru distribution of flyers in Cubao.

8.8 Police Community Relation Group (PCRG) distributed Information Education Communication (IEC) materials (Tips Para Iwas Krimen at Sakuna sa Semana Santa at Panahon ng Bakasyon) to bus terminals in Pasay and Cubao; conducted a door to door distribution in Quezon City; conducted a meeting on 28 March 2018 to present the mandates of PCRG including the Roles of Police and Community in Crime Prevention attended by Managers and Operations officers of the Provincial Bus Association of the Philippines (PBOAP).

8.9 Provincial Police Office - Cebu set-up public assistance centers in different churches.

9. DOH activated CODE WHITE wherein all DOH-retained hospitals are on standby for any eventualities or untoward incidents; placed Health Emergency reporting system to monitor health emergency related cases; DOH-OpCen is operating 24/7.

9.1 CHD I OpCen is on activated status to monitor and document untoward events, Quick Response Team is on duty to provide guidance and assistance to the 4 PHOs re management of health impacts of any untoward incidents.

a. All DOH-retained hospitals (Ilocos Training and Regional Medical Center-San Fernando, La Union; Region 1 Medical Center-Dagupan City; Mariano Marcos Memorial Hospital Medical Center-Batac, Ilocos Norte) had their emergency medical service teams on 24-hour duty.

9.2 CHD II is on activated status from 26 March to 03 April 2013; HEMS-Operation Center is on alert status and actively monitoring for any health related emergencies in the region.

9.3 CHD IV-B and CAR is on alert status and alerted hospitals in their AOR for possible mass casualty incidents.

9.4 CHD V OpCen is on 24/7 operations and prepositioned medicines and supplies in different provincial health team offices.

10. BFP implemented “Oplan Semana Santa 2013” and placed all operating units on heightened alert status; deployed / prepositioned fire trucks with complete crew and Emergency Medical Services personnel on strategic places; activated the EMS/SRU teams and all its operating unit support groups.

10.1 BFP I prepared contingency plans on emergency incidents; installed EMT and Rescue Units on strategic places to assist motorist during emergencies and established Command Post on the following areas:

� Ilocos Norte-Laoag City, Badoc, Batac � Ilocos Sur-Vigan City, Candon City, Sta Maria � La Union-San Fernando City, Agoo, Bauang, Rosario � Pangasinan-Binalonan, Urdaneta City, Rosales, Alaminos City,

Dagupan City, San Carlos City & Manaoag

10.2 BFP II deployed fire trucks with appropriate personnel in high risk areas; officers and key personnel are on-call to extend assistance in case of emergencies and other eventualities.

10.3 BFP III deployed fire trucks, ambulances, rescue trucks, Command Group, seven (7) Task Force Groups, Emergency Medical Service (EMS), and Special Rescue Unit (SRU) to render public service and assistance to all motorists and individuals in Region 3.

10.3 BFP IV-A issued a memorandum to activate Oplan Lakbay Alalay and to set-up motorist assistance centers; directed Special Rescue Units in the Regional and Provincial Offices to render 24/7 duty and be ready for deployment when the need arises.

10.4 BFP IV-B fire trucks with appropriate personnel are ready for deployment in the high risk areas and ensured availability of fire/rescue/emergency medical personnel, apparatus and auxiliary units to suit all operations needs.

11. LTO / LTFRB personnel are deployed regarding “OPLAN Mahal na Araw” in coordination with PNP.

12. DSWD

12.1 DSWD Emergency Response Team is on-call in case of an emergency; prepositioned food and non-food items in ports for augmentation; Social Welfare and Development (SWAD) Teams in six (6) provinces are available; closely coordinated with the P/C/MDRRMCS thru P/CMWDOs.

12.2 DSWD I prepositioned relief assistance.

13. PRC set-up first aid stations along national roads, highways, religious places, and terminals/stations (bus, airports, and seaports); two (2) WASAR Teams are on standby; PRC Operation Center and Emergency Response Teams are monitoring the situation 24/7.

13.1 PRC Pangasinan, La Union and Ilocos Norte-Laoag City is on 24 hour alert status and on stand-by for response augmentation; WASAR teams are on standby for response mobilization; established first aid stations in strategic places.

13.2 PRC – Batangas Chapter deployed teams and established first-aid stations Mt. Maculot and different resorts in Nasugbu and San Juan to monitor/assist the hikers/tourists.

13.3 PRC VI in coordination with the PNP and Iloilo City Emergency Responders (ICER) deployed ambulances in Tagbak Terminal and Oton Terminal in Iloilo; two (2) SAR teams are on alert status.

14. Philippine Ports Authority (PPA)

14.1 PPA Calapan implemented task force Semana Santa; activated their public information assistance center / help desk; intensified police visibility; provision of adequate medical assistance (personnel, paraphernalia, equipment, and drinking water facilities).

15. DPWH

15.1 DPWH I activated operation centers in its AOR; deployed motorist assistance teams; temporarily halted all construction activities from 27-31 March 2013 to decongest traffic and to utilize available equipment to respond on any untoward events.

15.2 DPWH III established motorist assistance task force and deployed 4 dump trucks, 2 JICA trucks, pay loader, and elf truck at Junction Northwest Diversion Road, Lazatin Flyover, Junction Main Road,and Dolores Flyover Road in the City of San Fernando Pampanga.

15.3 DPWH IV-A activated “Lakbay Alalay” program and directed contractors with on-going projects to properly install informative warnings signs and provide safety devices or barricades in construction sites.

15.4 DPWH IV-B deployed motorist assistance teams and heavy equipment in areas prone to rock fall, landslides and roads cuts in their respective area of assignment.

15.5 DPWH CAR is on alert status; deployed heavy equipments and sends situational reports to OCD-CAR OpCen.

16. NTC

16.1 NTC I alerted accredited Civic Communication Groups in AOR to assist on communication concerns in case of communication system failures and emergency communication back up.

16.2 NTC IV- A directed all amateur radio communication groups to provide assistance in the establishment of Advance Command Post (ACP) and Motorist Assistance Center (MAC) in their AOR.

17. RDRRMCs/OCDRCs on “BLUE ALERT STATUS” effective 27 March 2013 to 1 April 2013.

18. OCD I convened the RDRRMC I meeting presided by the RDRRMC I Chairperson and attended by officers/representatives of RDRRMC member agencies and Local DRRMOs re preparedness measures to be undertaken in observance of the Holy Week and SumVac 2013.

18.1 PDRRMCs Pangasinan, La Union, Ilocos Sur, Ilocos Norte and MDRRMC Bauang are on standby alert for any untoward incidents in their respective AOR.

19. OCD III LDRRMCs activated SumVac 2013 “Oplan Lakbay Alalay”; established first aid stations and Command Posts; deployed personnel for motorist assistance centers in bus terminals and churches.

19.1 PDRRMCs Bulacan, Pampanga, and Tarlac; CDRRMCs Malolos, San Fernando, Olongapo and Cabanatuan; MDRRMCs Gerona and Anao,

established first aid stations in strategic areas, deployed personnel and prepositioned rescue equipment and vehicles.

19.2 CDRRMC Malolos established monitoring activities in three (3) main areas in the city: In front of DPWH, Brgy San Pablo; Malolos Crossing; and in front on Barasoain Church

19.3 Malolos City Rescue Squad, DPWH, PNP, NGOs namely ARREST, BEMERT, SPECTRUM, and RESCUE 117 were deployed to the area. Two (2) PNP vehicles, fire trucks, ambulance, two (2) elf trucks, and a dump truck were also prepositioned

19.4 PDRRMC Pampanga deployed 256 personnel in 20 motorist assistance centers in 4 bus terminals and 6 churches. PRC Pampanga prepositioned ambulance at command post.

19.5 PDRRMC Command Post was established in front of Manabat Trading. 19.6 CDRRMC San Fernando deployed EMS/Rescue Teams, City Health

Office, Bureau of Fire Protection, Philippine National Police, and Traffic Management Division. They also prepositioned ambulances, double cab light truck, mobile patrols, and wrecker truck to conduct Oplan Maleldo (Mahal na Araw)

19.7 PDRRMC Tarlac activated SUMVAC 2013 “Oplan Lakbay Alalay” from March 27 to April 1. They also alerted PDRRMC/DART, PEO/PEPO composed of 30 personnel and rescue equipment and vehicles were prepositioned

19.8 CDRRMC Olangapo executed their Incident Action Plan on Semana Santa with operational period of 25 to 31 March 2013, included in the plan were the organizational list, assignment list, communication plan, medical plan, incident map, and safety message

19.9 CDRRMC Cabanatuan executed their contingency plan for Oplan SUMVAC 2013 and deployed personnel and prepositioned equipment in strategic areas in the city

19.10 Chairperson, RDRRMC3 visited the following areas: 19.10.1 Cabanatuan City, Nueva Ecija – March 26 19.10.2 Different areas in Pampanga and portions of

Balanga City, Bataan – March 27 19.10.3 Different areas in Bulacan – March 28

20. OCD IV-A and IV-B convened a meeting with PPA Manager, PPA Safety Officer, MARINA, PCG-STL and Batangas CDRRM officer for the preparation of Semana Santa.

21. OCD IV-A issued a memorandum advising all local LDRRMCs in the region to place their respective DRRM Operations Center and Search and Rescue Teams on standby alert; visited Rizal Police Station and Antipolo CDRRMO and checked the activities and status on the area.

21.1 PDRRMC Batangas convened a meeting with member agencies; established twenty-two (22) Police Action Centers (PAC) and 189 Motorists Action Centers (MAC) in different religious sites, accident prone and other populated areas; posted tarpaulin in difference areas of the province to inform travellers of assistance; activated PDRRMOC and disseminated RDRRMC Memo No. 5 S. 2013 to LDRRMCs.

a. CDRRMC Batangas City set up an Advance Command Post (ACP) in the port area manned by CDRRMC personnel, PCG, and PRC (Batangas Chapter) to cater the medical and other emergency needs of

passengers in the port area; coordinated with Smart Telecom and Globe Telecom to provide tents that can be used by the passengers as temporary shelter while waiting for their scheduled trips; prepositioned emergency and lifesaving equipment.

21.2 PDRRMC Cavite established a command post for Lakbay Alalay on 28-31 March 2013 and prepared first aid kits.

a. CDRRMC Tagaytay put up three (3) assistance centers manned by DRRMO, PNP, Traffic, and Rescue personnel, and advised BDRRMCs to activate their respective committee.

b. MDRRMC Ternate started and established four (4) stations for Oplan Lakbay Alalay Semana Santa 2013.

c. MDRRMOs and other agencies in Cavite activated their disaster operations center.

21.3 PDRRMC Laguna continuously monitored situation in its AOR and established a command post and prepared logistical requirements for effective response.

a. CDRRMC San Pablo convened a meeting with City Traffic Management Office, PNP, and Barangay Officials for the preparation for Holy Week and activated their disaster operations center.

21.4 PDRRMC Rizal established a command post and first–aid stations for Alay Lakad on 28-29 March 2013; convened a series of meetings re preparation for Holy Week; deployed trained volunteers in first aid stations in St. Joseph, Junction, Tikling, Singer, Cainta and in Melendres, Taytay; LDRRMCs in coordination with PNP, AFP, PRC, and NGOs put up booths in strategic locations in accordance to the full operation of Lakbay Alalay that includes traffic rerouting scheme on heavily affected areas.

21.5 PDRRMC Quezon established a command post for Alay Lakad on 28-29 March 2013 at Tayabas–Lucban Road and posted tarpaulins/streamers with emergency contact numbers in different strategic locations; dispatched Emergency Response Team in Tayabas and Lucban to assist the devotees; closely coordinated with MDRRMOs re situation in their respective AOR; QERT and Municipal ERT are on 24/7 stand-by alert for monitoring and assistance provision to possible stranded passengers.

22. OCD IV-B continuously coordinated and monitored the six (6) municipalities in the province of Marinduque where the annual Moriones Festival is being held; provided technical assistance in the establishment of Incident Command System in the province of Marinduque.

22.1 PDRRMC Marinduque established its Incident Command Post for the celebration of the Annual Moriones Festival.

22.2 Public Safety Department Calapan City established Oplan Bantay Dagat to monitor the public and tourist along the beaches.

23. OCD V disseminated a memorandum to LDRRMCs, RDRRMC Member Response and Support Agencies to submit respective contingency plans and/or list of available resources on standby that can be utilized.

23.1 PDRRMC Camarines Norte designated response team to assist at identified Holy Week 2013 motorist stations all over the province; coastal municipalities have organized the “Sagip Buhay Operation”.

23.2 PDRRMC Catanduanes delineated traffic management / routing scheme in anticipation to the increase of traffic volume.

23.3 PDRRMC Sorsogon designated field operation centers and Disaster Operation Center on Blue Alert Status to receive reports and advisories.

23.4 PDRRMC Albay released advisory for Holy Week; instructed LDRRMCs to organize respective emergency quick response team.

24. OCD VI lent one (1) rubber boat to PCG - Iloilo to monitor and ensure the safety of pilgrims who travel via pump boats from Iloilo to Guimaras.

24.1 PDRRMC Iloilo advised all deputized civil defence coordinators / MDRRMOs, Rural Health Unit Officers, Chief of local PNP, BFP Stations and local response teams to make coordinative linkages and take necessary response and safety preparation.

24.2 PDRRMC Antique established assistance desk and prepositioned emergency responders in strategic areas.

24.3 PDRRMC Capiz deployed one (1) medical team with ambulance in Municipality of President Roxas and one (1) team in Roxas City for monitoring and response.

24.4 PDRRMC Guimaras coordinated with Guimaras Resort Owner Association to ensure the safety of the tourists; deployed two (2) rescue boats along the Iloilo-Guimaras Strait and one (1) rescue vehicle in Jordan Wharf, Guimaras to monitor and ensure the safety of pilgrims.

24.5 PDRRMC Aklan - Boracay Action Group (PRC, PCG, PNP, AFP and other volunteers) under MDRRMC Malay conducted a meeting; LDRRMCs established public assistance desk; deployed first responders in churches.

24.6 PDRRMC Iloilo prepositioned ambulances in North, South and Central public transportation terminals.

25. OCD VII activated Oplan Alerto to closely monitor and assess the situation and submit report to national council on the status of operations as often as necessary.

26. OCD ARMM formulated an initial draft of preparedness/contingency plan for the Holy Week 2013, copy of which was provided to NDOC.

27. OCD CAR is continuously monitoring and disseminated weather advisories and Travel Safety Tips to CRDRRMC members, DRRM Officers, Local DRRMC, Tri-Media and General Public thru COCD-Info-Board, Social Networking media, SMS, and telephone calls.

27.1 CDRRMC Baguio deployed fifty five (55) civic action group volunteers for assistance in medical, traffic, rescue and communication issues. To date, some of the civic action group volunteers already pulled out from their respective areas.

27.2 Provincial and Municipal DRRMCs are on alert status and ready for deployment for any emergencies in their AORs; deployed PDRRMC local responders together with Barangay Officers; activated Volunteer Civic Action Groups in their AOR for motorist assistance and emergencies.

28. OCD NCR Regional Director joined the team of government officials during PNOY’s visit at Victory Liner EDSA at 12 noon, 26 March 2013.

28.1 CDRRMCs Taguig City, Quezon City, Malabon City, Pasay City, Valenzuela City, Navotas City, Muntinlupa City, Mandaluyong City, and Marikina City deployed personnel from PNP, City Health Office, Waste Management Division, City Social Welfare Development Office and Public Safety Division to provide assistance, safety and security of the motorists and tourists.

TAB B A. Fire Incidents - 12

1. NCR - 4

1.1 West Rembo, Makati

• On or about 12:46 AM, 30 March 2013, a fire incident occurred at 31-C Gen. Lopez Jaena St. (former 17th Street), Brgy. West Rembo, Makati City involving residential houses made of light materials

• Investigation disclosed that a stove was left unattended in the Padua residence where the fire started

• A total of seven (7) houses were damaged and the estimated cost of damage was pegged at Php 500,000.00

• The incident affected 8 families/44 individuals who are currently taking shelter at barangay hall

• No casualties reported

ACTIONS TAKEN:

• Makati BFP and other volunteer fire brigades responded to the incident • OCD-NCR coordinated with BFP Makati and Makati Rescue for the initial

report; and requested updates from the Arson Division of BFP Makati

1.2 Loyola Heights, Quezon City

• On or about 1:16 PM, 29 March 2013, a fire incident occurred at #277 Katipunan Ave. Cor. Xavierville Ave. Loyola Heights, Quezon City involving a two-storey commercial building

• Three (3) families were affected • No casualties reported • The estimated amount of damage is Php 5,000,000.00

ACTIONS TAKEN

• OCD-NCR coordinated with Quezon City Fire Department for the spot investigation report

1.3 Barangay Commonwealth, Quezon City

• On or about 8:50 PM, 30 March 2013, a fire broke out involving a residential establishment owned by a certain Agustina Ybe on Katarungan St., Barangay Commonwealth, Quezon City

• One (1) person, Hernando Ybe, 57 y/o, son of the owner died in the incident • The cause of fire is yet to be determined • The cost of damages is estimated at two hundred thousand pesos

(200,000Php) • The fire was put out at 9:30 PM • Other information on the incident is still under investigation

ACTIONS TAKEN

• BFP Quezon City and fire volunteers responded to the incident

• OCD-NCR coordinated with BFP QC and DPOS for information and spot report

1.4 Barangka Itaas, Mandaluyong City • On or about 10:55PM, 30 March 2013, a fire broke out involving a residential

establishment owned by a certain Victor Tagdibao at 13 Wayan St. Barangaka Itaas, Mandaluyong City.

• The cause of fire is yet to be determined • The cost of damages is estimated at two hundred fifty thousand pesos

(250,000Php) • The fire was declared out at 11:30 PM • No casualty was reported

ACTIONS TAKEN • Mandaluyong barangay fire volunteers responded to the incident • OCD-NCR coordinated with Mandaluyong BFP for information and spot

report

2. REGION II - 1

1.1 Delfin Albano, Isabela

• On or about 7:00 PM, 30 March 2013, a fire incident involving a sugarcane plantation occurred in the boundary between Brgy. San Macario and Carmencita, Delfin Albano, Isabela

• According to investigation, the cause of fire was due to a cigarette thrown at the dried cogon leaves near the sugarcane plantation.

• Fire out was declared at 11:50 PM, 30 March 2013. • No casualty was reported • The cost of damages is estimated at one hundred twenty thousand pesos

(120,000Php)

ACTIONS TAKEN • BFP Madayag responded to the incident.

3. REGION III - 2

3.1 Apalit, Pampanga

• On or about 2:10 pm, 30 March 2013, a fire occurred in Sun Valley Corporation, a commercial establishment located at Brgy. Silangang Kababahayan, Nagcarlan, Laguna.

• The estimated cost of damage incurred is more or less Php 50,000.00 • There were no casualties reported. • The fire was declared out at 2:45 PM.

• Bureau of Fire Protection personnel and members of Bantay Krimen of Nagcarlan headed by FO4 Mario C. Gesmundo immediately responded to the incident.

4.1 IGACOS, Davao Del Norte

• On or about 4:30 PM, 28 March 2013, a fire incident occurred in Sitio Libdan, Brgy. Pichon, Babak District, IGACOS.

• The cause of fire is still undetermined • No casualties reported • Seventy-four (74) families were affected and brought to Angel Villarica

Elementary School • Fifty-four (54) houses were totally damaged

a. On or about 1:00 AM, 28 March 2013 at Diversion Road, Bayombong, a collision occurred between a tricycle and a motorcycle resulting to four (4) people sustaining multiple minor injuries

b. On or about 11:30 AM, 28 March 2013 at Bonfal Proper, Bayombong, a 43 year old male, intoxicated with alcohol, lost control of his bicycle and hit the concrete road

c. On 28 March 2013 at Bolinao, Nueva Vizcaya, a 25 year old male, intoxicated with alcohol, lost control of his motorcycle resulting to multiple minor injuries (abrasions)

d. On or about 10:00 AM, 29 March 2013 at Bayombong, Nueva Vizcaya, a three (3) year old male was hit by a car and incurred concussion at occipital area

• The identities of the casualties are not yet available as to date

• On or about 10:56 PM, 30 March 2013, a vehicular accident was reported in Angeles Magalang road, Pulong Cacutud, Angeles City, Pampanga involving a single type motorcycle driven by Jay-ar Canlas, 19 y/o.

• Canlas who suffered from cuts in the chin bruises and abrasion on his extremities was transported to Ospital ng Angeles for further medical treatment.

2.3 Bacolor, Pampanga • On or about 3:28 PM, a vehicular accident occurred in JASA road Bacolor,

Pampanga involving a Van alighted by two (2) individuals namely Oscar Ponge, 63 y/o and Paolo Ponge, 30 y/o.

• The said vehicle fell off the road causing minor injuries to the 2 victims. ACTIONS TAKEN

Baliwag, Magallanes, Cavite accidentally hit a bicycle driven by Natalio Diquit y Mendoza, 70 years old and a resident of Brgy. Bucal-2, Maragondon, Cavite causing injuries to both drivers.

• On or about 8:50 AM, 30 March 2013, a vehicular accident occurred along Maharlika Highway, Brgy. Brgy. Conception 1, Sariaya, Quezon involving a tractor head truck registered to Naga Rits Corporation and being driven by Edgar Collar Avila, 47 y/o and a motorcycle driven by Angelo Lucenarion Peña, 26 y/o.

• Investigation reveals that the two vehicles were traversing in an opposite direction. The motorcycle attempted to overtake a bus in front of it; while failing to anticipate the incoming truck which eventually led to a collision.

• Angelo Lucenarion Peña was brought to Candelaria United Doctors Hospital but was declared dead on arrival. The backride passenger, Marlon Singkol Caracaylas was brought to Quezon Medical Center, Lucena and is in critical condition.

• On or about 5:39 AM, 28 March 2013, a vehicular accident involving a Toyota Fortuner driven by Noroding Macabando Badron, 27y/o, and a motorcycle driven by Rev. Jasper Diesta Valdez, 37 y/o, occurred at the vicinity of the bridge in Brgy. Manhumlad, Matnog, Sorsogon.

• As a result, the driver of the motorcycle sustained injuries and was pronounced dead on arrival upon being brought to the hospital.

6. CORDILLERA ADMINISTRATIVE REGION - 1

3.7 Baguio City

• On or about 3:50 PM, 31 March 2013, a vehicular accident involving an Isuzu Elf Truck driven by Mr. George Necesito, 27y/o, occurred at the vicinity of Camp 6, View Deck, Kennon Road, Baguio City.

• According to investigation, the driver of the vehicle lost control of the vehicle and decided to collide with a wall rather than to fall into a ravine.

• On or about 3:59 PM, 30 March 2013, the body of the victim was recovered and immediately brought to Region 1 Medical Center, Dagupan City, but pronounced dead on arrival by the attending physician.

ACTIONS TAKEN:

• San Fabian PNP, Navy SWAG, Navy Reserves and Philippine Coast Guard conducted SAR operations.

1.2 San Fabian, Pangasinan

• At around 5:00 PM, 30 March 2013, a drowning incident occurred in Barangay Nibaliw Narvarte, San Fabian, Pangasinan involving a certain Eliseo N. Nagamus, 36 y.o.,from Brgy. Nancayasan, Urdaneta City, Pangasinan

• On or about 5:20 PM, 30 March 2013, the victim was recovered along Brgy. Nibaliw Narvarte, San Fabian, Pangasinan. The victim; however, was pronounced dead on arrival upon being rushed to Region 1 Medical Center, Dagupan City

1.3 Bolinao, Pangasinan

• On or about 1:30 PM, 30 March 2013, Roland Deo, 33 y.o., of Caloocan City drowned in Sitio Batog, Barangay Arnedo, Bolinao, Pangasinan

• The victim was immediately brought to Rillera Medical Hospital but eventually died

1.4 Candon, Ilocos Sur

• On or about 6:25 PM, 30 March 2013, a certain Nestor Valdez, 60 y/o of Brgy. Patac, Galimuyod, Ilocus Sur was found lifeless floating along the shoreline in Brgy. Darapidap, Candon City, Ilocos Sur.

2.1 Nagtipunan, Quirino

• On or about 8:30 AM, 28 March 2013, three-year old Blizz Eilengie P. Tesorio drowned at Cagayan River in Barangay Dipantan, Nagtipunan, Quirino

• Investigation revealed that the parents of the victim were preparing for a picnic when the victim walked towards the river. She was carried by a strong current and was rescued by her father approximately 30 meters away

• The victim was brought to Maddela District Hospital where she was declared dead on arrival (DOA) by the attending physician

2.2 Cabatuan, Isabela

• At around 2:30 PM, 29 March 2013, Avelino M. Daliri, 29 y.o. and resident of Brgy. Saranay, Ilagan, Isabela, drowned while swimming at Magat River in Sitio Pariir, Saranay, Cabatuan, Isabela. He was rescued by a companion then rushed to the hospital but was declared DOA

2.3 Echague, Isabela

• At around 5:45 PM, 29 March 2013, 33-year old Venancio G. Dumon of Brgy. Pangul Sur, Echague, Isabela drowned at Cagayan River in the said barangay. The victim, who was out on a picnic with relatives and friends, drowned after diving into the river to retrieve a kettle cover

3. REGION IV- A (CALABARZON) - 4

3.1 Isla Verde, Batangas City

• On or about 3:00 PM, 29 March 2013, a drowning incident occurred in San Agustin Silangan, Isla Verde, Batangas City was reported

• The victim was identified as Felimon Aclan Jr. y Icasio, 25 y/o and a resident of the said barangay.

• Initial reports disclosed that the victim was found dead and floating and was rushed to St. Patrick’s Hospital but was declared dead

ACTIONS TAKEN

• RDRRMC IV-A coordinated with PCG Southern Tagalog, Batangas CDRRMO and Batangas City PNP regarding the incident

3.2 Tanauan City, Batangas

• On 28 March 2013, a drowning incident was reported in Taal Lake, Brgy. Wawa, Tanauan City, Batangas

• Junior M. Redondo drowned after tripping from his lifesaver. His cousin, Richard M. Matanguihan, attempted to save him but both drifted to the deeper part of the lake and drowned

• Nearby witnesses and relatives found the bodies after an hour of searching. The victims were immediately brought to CP Reyes Hospital but were pronounced dead on arrival

3.3 San Juan, Batangas

• On 30 March 2013, a drowning incident in Laiya, San Juan, Batangas City was reported

• The victim identified as Librado Marundan a resident of the said barangay was rushed to the San Juan District Hospital and then transferred and died at Lipa Medix Medical Center.

ACTIONS TAKEN:

• PNP San Juan conducted investigation of the incident • RDRRMC IV-A coordinated with Batangas PDRRMO regarding the incident

3.4 San Pablo City, Laguna

• On or about 7:50 a.m., 31 March 2013, Noah C. Sombilla, 15 years old, single and a resident of Brgy. Antipolo, Rizal, Laguna, drowned at Lake Calibato, Brgy. Sto Angel, San Pablo City, Laguna.

• The victim was recovered through retrieval operation, brought to Pagamutang Panlalawigang Lungsod ng San Pablo Hospital, but pronounced dead on arrival by the attending physician.

• The cadaver of the victim was brought to St. Peter Funeral Homes-San Pablo City for autopsy.

4. REGION IV- B (MIMAROPA) - 1

4.1 Magsaysay, Occidental Mindoro

• On or about 9:00 AM, 30 March 2013, Apple Joy Carion, 13 y.o. and resident of Barangay Caguray, Sitio Tuong, Magsaysay, Occidental Mindoro was reported missing and believed to have drowned in Caguray River

5.1 Polangui, Albay • On or about 8:00 AM, 31 March 2013, a drowning incident involving a

certain Ted Vera y Dacara, 14 y/o, occurred at Magpanambo River, Polangui, Albay.

• At 3:40 PM, 31 March 2013, the body of the victim was retrieved by the NAVSOUN3-PN3 divers.

ACTIONS TAKEN • Joint elements of the NACSOG-PN, Albay WASAR Team, PNP and LGU

Polangui and MERIT Guinobatan conducted the retrieval operation.

6. REGION VIII - 1

6.1 Tolosa, Leyte • On or about 1:30 PM, 31 March 2013, a report of a drowning incident at Luis

Beach Resort, Brgy. Imelda, Tolosa, Leyte involving a certain Jerome Adonis Rocabo, 23 y/o, was made and submitted to proper authorities.

• According to the report, the victim made an attempt to save a seemingly drowning person but instead drowned himself in the process. Ten (10) minutes later, he was rescued on the spot where he was last seen. Upon being rushed to the hospital, he was declared dead on arrival.

D. ARMED CONFLICT/ SHOOTING INCIDENT - 4

1. REGION V - 1

1.1 Libertad, Misamis Oriental

• On or about 11:00 AM, 29 March 2013, a certain Jerome Viola Bernal, 29 y/o, and his live-in partner Myline Tenerife Tindugan, 28 y/o were shot by more or less five (5) unidentified gunmen, who are believed to be members of Communist Party of the Philippines (CPP) /New People’s Army (NPA) / National Democratic Front (NDF) in Brgy, Siay, San Miguel, Catanduanes.

• As a result, Bernal died while Tindugan was brought to Eastern Bicol Medical Center for treatment.

2. REGION X - 1

2.1 Libertad, Misamis Oriental

• On or about 9:40 PM, 28 March 2013, an unidentified gunman started shooting several individuals in Barangay Poblacion, Libertad, Misamis Oriental

• One (1) was reported dead and five (5) were injured

3. CARAGA REGION - 2

3.1 Encounter Between AFP 3 rd Special Forces and New People’s Army (NPA) in Butuan City

• On or about 5:00 AM, 29 March 2013, an encounter between AFP 3rd Special Forces and New People’s Army (NPA) occurred while the former was conducting combat/outer perimeter security operations at Station of the Cross (PANAAD 2013) in Sitio Eyao, Brgy. Anticala, Butuan City

• As a result, one (1) member of the CAFGU was killed

ACTIONS TAKEN: • PNP personnel provided security to the 14 stations maintained and held

their grounds to ensure the completion of the said religious activity • Adjacent City and Municipal Police Stations conducted strong checkpoint

operations and roadblocks at strategic places to intercept the fleeing enemy, and units of this PRO were alerted and directed to monitor/check the hospitals in their respective AORs for any patient admitted with gunshot wound

• OCD Caraga coordinated with PNP and CDRRMC-Butuan for updates on the encounter

3.2 Abduction in DOLE STANFILCO, Brgy. Anahao Daan, Tago, Surigao del Norte

• On or about 8:00 p.m., 27 March 2013, a number of armed men who were believed to be members of Guerilla Front 19B, Northeastern Mindanao Region Committee entered the vicinity of DOLE STANFILCO in Brgy. Anahao Daan, Tago, Surigao del Norte and detained five (5) personnel.

• The armed men also chopped down the banana trees, destroyed the stockroom, and brought with them rubber boots, tents, and fertilizers.

• After an hour, the detainees were also released safely.

ACTIONS TAKEN: • Surigao del Sur PPO conducted investigation. • 29th IB, PA and Surigao del Sur PPSC conducted pursuit operation against

the armed men.

E. SNAKE BITE INCIDENTS - 2

1. REGION IV-A (CALABARZON)

1.1 Cuenca, Batangas

• At around 5:00 AM, 29 March 2013, a snake bite incident at the Grotto Site of Mt. Maculot, Cuenca, Batangas was reported

• Joshua G. Metrillo, 17 y.o. of Barangay Labac, Cuenca, Batangas, was on his way to the Grotto when a snake bit his left big toe. He was rushed to Metro San Jose Hospital in San Jose Batangas for initial treatment before he was transferred to Research Institute for Tropical Medicine in Alabang, Muntinlupa City

ACTIONS TAKEN • RDRRMC IV-A coordinated with PDRRMO Batangas and DOH-CHD IV-A

regarding the incident

2. REGION X

2.1 Catarman, Camiguin

• On or about 12:30 AM, 29 March 2013, 21 year old Mitzie Rica Panlaroche was bitten by a snake in Bonbon, Catarman while observing the stations of the cross

• She was admitted to Camiguin General Hospital and was discharged on 30 March 2013

F. OTHER INCIDENTS - 7

1. NCR

1.1 Black Nazarene Procession, Manila

• As of 3:30 PM, 30 March 2013, the approximation of the number of devotees who participated in the Black Nazarene procession increased from an estimated 15,000 to 1 million attendees. The total number of injured individuals increased from 145 to 490

• The Black Nazarene procession started at around 3:00 AM, 29 March 2013, resulted to minor injuries to a number of devotees. One (1) was treated with serious case of injury, two (2) were hospitalized and four (4) children were reported lost but were later found

ACTIONS TAKEN

• OCD-NCR continuously monitored the situation

1.2 Chemical Leakage in Tondo, Manila

• On or about 9:00 p.m., 31 March 2013, a chemical leakage occurred at Fernandez St., cor. Callejon St., Brgy. 173 Zone 15, Gagalangin, Tondo, Manila.

• Three (3) patients who are yet to be identified experienced dizziness and vomiting. They were immediately treated and allowed to go home.

• Capt. Tagyam of Makati SRU identified the chemical as “Malathion” and declared the area cleared and out of danger.

• Responding agencies were Blue Dragon of CDAG, Makati SRU , BFP San Lazaro , and Barangay officials .

• OCD-NCR coordinated with CDAG for the report.

2. REGION V

2.1 Maritime Fire Incident in Ilawod, Caramoan, Camarines Sur

• On or about 5:55 PM, 30 March 2013, a speedboat owned by Aga Mulach and skippered by a certain Gregoria dela Cruz, 49 y/o with a crew member named Jerome Pervera, 21 y/o had its gas tank exploded.

• According to investigation the fiber-hulled speedboat produced a spark during ignition which rapidly crawled towards the gas tank.

ACTIONS TAKEN: • Philippine Coast Guard Search responded to the scene of the incident • OCD VI coordinated with PCG Caramoan for information re the incident

2.2 Reported Missing Person in Sta. Magadalena Sorsogon

• On or about 09:00 AM, 31 March 2013, a missing person report was filed at Sta Magdalena, Sorsogon.

• According to the report a fisherman named Bonifacio Guatno went fishing on or about 09:00 AM, 30 March 2013, but at around 3:00PM of the same day, only his boat was recovered.

ACTIONS TAKEN: • Local fishfolks have joined in the efforts to search and rescue the victim

2.3 Stabbing incident in Panganiban, Catanduanes

• On or about 05:30 PM, 28 March 2013, a stabbing incident involving a victim, Diogenes Bernardo Fernandez, 64 y/o, and the suspect, Reynaldo Clopino Flores, 65 y/o occurred in Brgy. Cagdarao, Panganiban, Catanduanes.

• As a result, the victim died while the suspect remains at large.

3. REGION X

3.1 Capsized Banca in Talicud Island, IGACOS, Davao Del Norte

• On or about 5:30 PM, 28 March 2013, a motorbanca capsized due to broken engine shaft in Talicud Island, IGACOS, Davao del Norte

• Fifteen (15) passengers were safely rescued by passing fishing boat named F/B Evelyn:

4. REGION XII

4.1 Belated Report on Flooding Incident in Banga, South Cotabato

• At around 7:15 PM, 27 March 2013, a flooding incident caused by heavy downpour took place in Mondejar Street and Sitio T’boli, Barangay Kusan, Banga, South Cotabato

• The flooding destroyed the approach of Zupad Bridge in the boundaries of Sitio T’boli and Liwayway, and devastated approximately 267 hectares of rice field

ACTIONS TAKEN: • PDRRMC South Cotabato , in coordination with Banga MDRRMC, conducted

damage and needs assessment (DANA) and submitted report to OCD and DSWD XII
