• W.E CCG have completed the roll out of our Staff Volunteer Programme in August 2014• - We have appointed 3 Volunteer Champions within the organisation to enable
implementation
2• Community Mobilising Steering Group Formed – June 2014 – embedding the CCG approach
to partnership and Collaboration. Enhancing and joining up what is working well.

• Developed and commenced roll out of co-produced, volunteer led Primary Care Based Wellness hubs. Provider Age Uk/Sept awarded 1 year pilot September 2014 for delivery in 5 initial pilot GP Practices

• Sought and secured the buy-in of our membership and partners – delivered a partnership approach to complimenting the work of Community Agents
