Headlines

• Total number of carers supported by voluntary sector schemes = 21,996 (assuming no double counting of carers in receipt of more than one service).
• The single most common source of referral for these schemes was GP / doctor (page 4).• Lower than expected proportion of carers from BME groups are supported (6% of those
supported, despite accounting for 10% of carers in Surrey) (page 4).• Case study quotes are provided to demonstrate the positive outcomes for carers. These
have not been updated since the last report.• Surrey Young Carers supported 1,849 young carers during the full year period (page 5).• Carers & Employment Support service supported over 350 people (page 8).• Crossroads Breaks service has a considerable waiting list (page 14). This information has

• Approx 16,600 carers are now coded as such in Surrey GP surgeries (pages 7 and 34). This information has not been updated since the last report.
• GP Carers Breaks - Total 2,165• North West Surrey CCG has by far the largest number of EOL hours support at over 44,500
– the next highest is NHS Surrey Downs CCG with just under 19,000.

Overview

• 29 of the 31 voluntary sector schemes sent in returns forms for the full year between April and March.
• These schemes are supporting nearly 22,000 carers who in turn are caring for over 17,000 people.
• Over 6,600 new referrals were made in this 12 month period.• Of the 29 schemes, 9 of them said they had waiting lists, though only 4
had people on their waiting list at the end of March with 96 people waiting for support. This is an average of 24 people per scheme with people on their waiting list.
• 68% of carers supported are aged over 18, with 25% aged over 65 and thus potentially being frail themselves.
• Of those supported, 6% of carers are from BME groups. This is lower than the Surrey proportion of 10% being from BME groups.
• Of the 6,682 referrals made to these schemes, 25% came from GP/doctor and 24% came from the carer self referring.

As a result of my involvement, both girls have engaged in numerous fun
activities with Surrey Young Carers, including NYCE days, a cinema trip and meals out, which has provided both girls with the opportunity to engage in activities they don’t normally have a chance to, time out from
their caring role and opportunity to develop peer networks with young people like
themselves.

Key activities
1. Supporting GPs and primary care providers to work better with carers, eg increasing numbers of GP-registered carers, raising awareness in practices, promoting carer-friendly strategies like flexible appointments, helping to identify inequalities

A surgery wasreluctant to forward the Carer
Registration forms on to Carer Support. The GP Carer Awareness Adviser for the SW had
previously discussed the issue with the senior GP who was unhappy about doing this because of the cost and
resource implications she believed it involved. By working closely and diplomatically with the surgery and by supporting the staff, the GP Carer Awareness Adviser helped to ensure that the surgery was able to progress its work for carers. By also taking
the opportunity to promote the Carers Prescription further referrals are possible including the Carers Break payment. The
practice has also nominated a member of the team to be a Carers Lead who will work to identify and support carers
at the practice.“I now feel very positive about this practice, they seem to be very keen to make
a difference to their carers”

Mr X, a carer for wife with serious Mental Health issues ,
has been a carer for 10 yrs. He worked full time and caring over 50 hours plus per week.
Mr X was given notification of redundancy and was extremely concerned on the impact of unemployment
on his wife’s health and his coping mechanisms and financial uncertainty.
“Just a quick email to let you know how I got on with my numeracy and literacy test for the organisation.
I managed to pass and have an interview on Monday, I'm really pleased.”

• There are 10 schemes that are referred to as Enabling carer’s training run by: Action for Carers, Rethink, Woking, Runnymede, Surrey Heath, Waverley, Epsom, Mole Valley, Elmbridge and Tandridge.
• 63% of the 647 referrals made to these schemes were self referrals by the carers. 16% were from GPs/doctors.
• None of the carers supported by these schemes is under 18 years old. 33% are aged over 65.
• 10% of carers supported are from BME groups.• Only 2 of the 10 Enabling Carers Training schemes
had waiting lists, with only 49 people on waiting lists in total.

The carer felt in control and able to explore the
additional information and advice at a pace they considered they could cope with. The carer
has a reduction in manual handling effort and gained the knowledge of how to move someone without physical effort. The ability for access to
the community has been enhanced for the carer with the appropriate aids
and respite support.

Over this period Crossroads Care Surrey has delivered 3506 hours of care to people living within Surrey with an end of life prognosis.
The carers have been able to access sport and leisure facilities, attend medical appointments, have extra rest and relaxation, stay in work, spend time with other family members, and have regular shopping trips while their relative is cared for.
Over the period we have received 22 new referrals from GP’s, Voluntary organisations, Health services, self-referrals and Social Care teams.

Overall I justreally like how XX attending the
activities with Mencap has made him happier and has made him more independent. For example
he wanted to attend summer events without his friends from the social group, this is a massive change in XX in thathe wanted to go and try new things and meet new people, which before starting with Mencap he was reluctant to do.
He seems to be growing in his independence by partaking in Mencap activities which will hopefully mean in the future
that he will continue to do things more independentlyand will eventually even want to arrange social

Key activities
Before Rethink I felt desperately useless. People said I
seemed strong, managing to support everyone andkeeping the day to day routine going for the children. I
didn’t sleep well. I was neglecting my health. I was ignoring money issues. A call came from Rethink. They had been given my details by our GP as I had registered as my Husband’s carer. It was like a weight was lifted from my shoulders. She listened to our life
story, without judgement, but with care and empathy. I broke down. Most importantly, besides all the things I needed to know about
being a carer and how to support my husband. She helped me getmy life back in order. Rethink reminded me how important it
was to look after myself. She helped me mend my brokenbrain, husband and life but most of all my heart
and the strength to carry on.

The disabled young person is moving into further education
and claiming Employment and Support Allowance as an adult; The Employment and Support Allowance award means she will no longer be part of the benefit
household and therefore the benefit cap of £500 per week will be applied to the Mum and remaining siblings. Housing
Benefit will be significantly restricted so that her total benefit payments do not exceed the £500 cap. Client lives in social housing and is in rent arrears. She is already struggling to pay these arrears off. She has health problems of her own and may have to give up
the voluntary work she has been doing. An award of Disability Living Allowance for the second child in the family or Personal
Independence Payment award for the client would take The family outside of the scope of the benefit cap and
restore full Housing Benefit. The Unit is supporting the bureau in making both claims for
this family.

Mrs X attended the‘Carers Information Day’ that
CSE arranged during Carers Week. Mrs X needed information regarding a ramp to enableher husband To access the rear garden Mrs X had
become housebound with her husband, they both previously enjoyed gardening as a hobby. ReMap were
able to help Mrs X design and build a ramp in the garden which enabled Mr and Mrs X to spend time together in the
garden and invites friends and family to join them. By constructing the ramp into their rear garden, Mrs X was able to enjoy leisure time with her husband as well as
friends and family. This led to less isolation and reduced stress. Thanks to the construction of the
ramp, Mrs X is now able to get her husband into the garden safely with
Less risk.

The Adviser helped apply for funding towards a permanent
wheelchair, ramp, small handling equipment to help get in/out of the car and the satin sheets system.
From the advice given and immediate provision of equipment, Mrs X was able to continue her caring role,
avoiding further injury/pain. Sometime later the father in law agreed to accept a live in care worker. This enabled Mrs X to continue caring for her children and time to go
And visit her family abroad who were also unwell. She has since contacted us to say a huge
thank you.

Advice offered by CSW that X may benefit from having
regular support in order that she could talk regularly about her thoughts and feelings and
suggestion for therapies to manage her stress these could be potentially funded by SILC. A referral to the
Social Care Team for an assessment was made. Long term support to be offered. Once Y and X are comfortable with
this arrangement, the plan can be reviewed to look at increasing those hours, including a one off support payment for X to enrol with a gym, or purchase therapies. X accepted
this agreement with difficulty – appeared to be uncomfortable in allowing another individual to provide care, but at the same time recognises
that she is at the end of her tether and cannot maintain the level of care in which
she is providing

Mr and Mrs X have3 children ,their daughter has
severe learning disabilities. Mrs X requestedthe assistance of CSG to help complete a DLA
form for their daughter. Mr X has an extra caring role as his mother became quite dependant on him as her
husband became more debilitated through dementia. This put a huge strain on Mr and Mrs X apart from dealing with
the issues of their own daughter they also had the two younger children to care for and Mrs X said that her husband was always at his parents’ home. However back in 2013 Mrs
X mother-in-law became involved with CSG she met new friends through the CS Carers group and also received
help advice and the opportunity to attend training daysto help her in her caring role. This intervention had a huge impact on Mr and Mrs X as Mrs X
mother-in-law became much less dependent on them.

This experimental workshop introduced tools that can be used in every day life to help reduce stress. Physical, mental and
emotional aspects of well being were addressed and specific techniques were used to release tension and find peace. Carers also participated in simple Tai

We encouraged X totake Y to the Carers Cafe which is run
by Alzheimers UK. This is a relaxed settingwhere Carer and Cared For get together. X was able
to chat to other Carers in the same situation as herself.X said it's nice to know there is support out there, and that
she is not alone. She was also delighted that Y chatted with other men there and said they will attend on a regular basis. We also contacted Social Services who arranged for Y to go to a local Day Care Centre for two days a week which means that X is now able to have some breathing space and time to 'think'. She has also resumed meeting up with friends once
a week. She now regularly attends our Carers Camp and enjoys chatting with other Carers and also has
regular chats on the phone with new found friends and said she is starting to feel
the dark cloud lifting.

NHS Free Health Checks for Carers in Surrey 2013-14In the second year of carer’s health check delivery period 2013-14, altogether 7 carers support schemes or organisations took part in the health checks delivery project and organised this service for their eligible registered carers. These Carers Support schemes that took part in the project were Woking, Surrey Heath, Waverley, Guildford, Runnymede, Elmbridge and Reigate & Banstead. This year Public Health Team:Offered 200 Health Checks to registered carers through carers network Action for Cares and their carers support schemes and organisations in Surrey.158 carers booked their appointments through their support workers and 148 attended the Health Checks appointments at the venues booked in their boroughs.

Royal College of Psychiatry - Mental and physical illness in caregivers report Published September 2014 this report evidences the following findings:• Carers had poorer mental health and higher psychiatric symptom scores than non-caregivers. There was an observable decline in mental health above 10 h per week.• A twofold increase in psychiatric symptom scores in the clinical range was recorded in those providing care for more than 20 h per week
Carers UK - Carers at breaking pointPublished on the 10th September this report examines the experiences of carers who have been pushed to breaking point by caring without the right support. Based on the experiences of over 5,000 carers, this analysis examines the causes of carer breakdown, the impact of crisis and what support can prevent carers’ mental and physical health breaking down.
Key Findings:• 6 in 10 are at breaking point• A quarter of those who had reached breaking point required medical treatment as a result.• 46% said despite being seriously ill they had no choice but to continue caring• 1 in 9 said the person they cared for had to be rushed into hospital, emergency care or that social services had to step in to look after them while the carer recovered. • 1in 5 were forced to give up their jobs because they were in crisis
Feedback by Surrey carers who attended the NHS Health Checks:• I felt it was a good idea to come to this health check, as I feel that the
doctors at X surgery have very little time for me.• Need to know own health status, so very useful.• I had already made changes but they were confirmed to me today that I am
doing the right thing.• It’s nice to know someone cares.• I found the event very useful.• I came to this event because I was offered this health check.• Great to have an overview of my health and not feel the pressure of time
and stress of doctor.• Relaxed and useful experience.• So useful as always find it hard getting an apt with my doctor.• Because, as a carer with my husband, we thought it would be a good idea in
a non- threatening environment.• It is good to have a free health check because this could not happen without
referral at local surgery especially for my age group 45-60.• All the staff involved in today’s service have been courteous, professional
combined with impeccable manners A+• Very well organised and very much needed by carers.
