This catalog contains many types of forms which will enhance the use of the software for the convenience of Handicomp software users. Convenience in ordering makes it a time saver for you. Most generic items are in stock and shipped out the same day you place your order. Easy payment . . . we bill on open account and you can take up to 30 days to pay. Handicomp will also accept credit card payments. Handicomp Guarantee . . . we guarantee all forms sold from this catalog to be fully compatible with the latest release of our software. Prices in this catalog are subject to change without notice. Shipping charges are prepaid by Handicomp and added to your invoice.

HANDICOMP SUPPLY ORDER POLICIES
1. PRICING Items listed in this catalog are priced for laser forms. 2. TAX AND SHIPPING CHARGES Handicomp, Inc. is a corporate entity required by law to collect the appropriate sales tax for all
shipments within the State of Michigan, Ohio and Florida. Appropriate tax and shipping will be added to your invoice.
3. PAYMENT TERMS Terms are NET 30 days from date of invoice to customers. Handicomp will accept credit card pay-
ments. Accounts over 60 days are subject to credit hold, and the account will be put in the Delin-quent status. No new shipments will be made until all past due balances are paid in full.
4. CUSTOM ORDERS Custom orders are guaranteed to be fully compatible with the latest version of our software. Cus-
tom order quantities are subject to a 10% over/under run as allowed by printing trade customs. Composition charges will be added as necessary. Any cancellations on special orders will be charged at cost according to materials ordered and work completed to date.
5. RETURNS Be sure to check all materials upon receipt. Every effort has been taken to ensure your satisfac-
tion with your order. If you are not satisfied with our merchandise, or have received the wrong item, contact our supply department regarding the problem, and immediate action will be taken to resolve the situation. Returned merchandise may be subject to a 15% restocking fee. Imprinted items, or custom printed items cannot be returned for credit.
6. SHIPPING METHOD Shipments will be sent via UPS Ground, unless otherwise specified. Faster shipments are availa-
ble at an additional charge. UPS Next-Day, UPS 2nd-Day and Federal Express shipments can be arranged when necessary.
7. NON-CUSTOMER REQUESTS The supplies and related prices in this catalog are provided for the convenience of our clients.
Others wishing to purchase an item listed in the catalog, to use with a software module other than a specific Handicomp module for which it was intended will be charged the listed price of the item. Non-clients must pay by credit card the day of the order.

Generate assignment cards from GolfTournamentNetwork Use these cards to denote the golfer names, tee number, and starting times. The cart number may also be printed on the card. Expanded print makes easier reading for the cart assignments. The cards are white with green print over a light green phantom screen of a golfer/club background. Cards are a tag stock weight, two cards per sheet. Below quantities are number of cards in shrink wrap.

This is a two-part form with one part given to the bettor and one part kept by the person in charge of the bet-ting. This form is not run on the printer, but is hand written at the time the bets are placed. After all of the bets are placed, the information is entered into the computer from the retained copy.

Generate scorecards from GolfTournamentNetwork. Pertinent information can be printed on the scorecard: flight number, starting tee, date, starting time, cart number, course (white blue, red, gold tees), hole-by-hole yardage and hole-handicaps. The golfer names and handicaps with strokes given are also printed on the card.
A four-line message can be printed on the front of the continuous form scorecards and seven on the laser scorecards, also longer messages or rules of the tournament can be printed on the reverse side. Minimum order of 250 scorecards. The background color of the cards are shown in the below examples.

GolfTournamentNetwork can print on two different sizes of scorecards. (12 1/2 x 5 or 11 7/8 x 5) Scorecards are tri-fold The club can choose from 1-color, 2-color, or 4-color process. Front side of card and be customized to your specifications. Digital Artwork (photos of facility or golf course) can be imprinted on the front side of your card and will be
considered a 4-color process (sample below). First time setup fees will apply. New clients: average turn-around is approximately 5 weeks.
