If so, they are provided the Research Study Informed Consent page and asked to agree to the privacy policy and terms of use. Screenshots are representative of app content as of April 28, 2020. App content is subject to change based on updates and new content.

Fig. S2. Participant registration, optional contact information, and study association. Participants are asked to register with an email and password and provide optional additional contact information, which can help with data linkage back to cohorts. Research participants are asked to select which studies they are associated with from cohorts who have recruited existing study members to use the app.

Fig. S3. Module to identify healthcare workers. Participants are asked whether or not they are a healthcare worker or work in a community care (e.g. home nursing/assistance) setting (left panel). Those that identify as healthcare workers are asked additional questions about work setting, possible interactions with confirmed and/or suspected COVID-19-positive patients, and use and availability of personal protective equipment (middle panel) at baseline. When subsequently returning daily, they are asked about recent exposures and availability of PPE.

Fig. S4. Baseline personal demographics. Participants are asked to provide basic demographic information upon first use of the app. Potential community exposure to COVID-19 is assessed here for non-healthcare workers.

Fig. S5. Baseline health information. Participants are asked to provide baseline information about potential comorbidities and medication use upon first use of the app.

Fig. S6. Daily isolation questions and COVID-19 status. Participants are asked how much they are isolating and whether or not they have received a COVID-19 test. If they have received a test, they are asked if they tested positive (yes/no) or are awaiting results.

Fig. S7. Daily health assessment and symptoms reporting. Participants are asked to report how they are feeling daily. If they are select that they feel physically normal, they complete the daily assessment and are asked to return the next day to record how they are feeling. If they select they are feeling unwell, they are asked about potential COVID-related symptoms with the option to report additional unqueried symptoms and are asked additional questions.

Fig. S8. Assessment for participants seeking or receiving treatment. Participants who reported not feeling well are additionally asked if they are or have sought treatment. If they sought treatment, they are provided with questions related to treatments they may have received with the option to record free-text answers.
