Shawn O’Farrell – SLEMS employee from 2016 to current. Mr. O’Farrell is a

rebuttal witness and may provide testimony regarding SLEMS’ orientation and clearance

from base hospital, response times, dispatch of units, interfacility transports, relationship

with Timber Mesa, equipment and patient care. Mr. O’Farrell’s testimony is expected to

last approximately one hour.

Lehigh Jessop – SLEMS employee from 2007-2011 and 2013 to present.

Mr. Jessop is a rebuttal witness and may provide testimony regarding SLEMS’ orientation

and clearance from base hospital, response times, dispatch of units, interfacility transports,

Sean Werner, being duly sworn, upon oath, deposes and states:

1. I am an IT professional with over 15 years' experience in developing and

managing IT projects and staff, including implementations of 7124/365 support structure.

2. I have provided IT consultant services to Show Low EMS since 2011.

3. On November 30, 2015, at approximately 0300 hours, the Show Low EMS

email seryer crashed due to a hardware failure. The server was down until December l,2015 at approximately 0800 hours.

4. On December l, 2015,I built a new server in a virtualized environment on

the Microsoft Hyper-V platform and Show Low EMS employee emails were migrated to

the new server. All emails were populated back into employee email accounts, and users

had access their email shortly after the server crash.

5. To ensure that users were able to access their email, I sent individual "test"

emails to each Show Low EMS employee.

6. On December 3, 2015 at 0643, I sent a ootest" email to Show Low EMS

employee Miles Lambdin, to which he responded at 0644. See email conespondence,

attached as Exhibit 1.

7. I did not receive any further communication from Mr. Lambdin.

8. Had Mr. Lambdin had any issues accessing his emails, he certainly could

have sent me a follow up email. I am usually pretty responsive, especially when an issue

arises with email access. Additionally, I am accessible by telephone. My telephone

number has been written on a white board at Show Low EMS for many years.

9. Like all other Show Low EMS employees, Mr. Lambdin's emails were

populated into his account after the server crash.

Dorothy,They were enroute earlier, this is one of the only things I do not like about RIMS, it does not log times correctly. If theunit has an MDT (computer) attached to it, when we attach them to the call when they advise enroute, it shows them as“dispatched” on the call, and then we have to go back and make them “enroute”. For our interpretation on this end,“disp” is the same as “enrte”, because we do not attach the units to the call until they are enroute. We type in “tonedout” when we tone it, and then the time dispatched or enroute for a unit, is considered their enroute time. That’s whatwe give when we give times over the phone.

I attached the call with my notes on it. Call me if you have any questions.

I had a crew that was concerned about the times associated with call 1510130015. Per the Shift Lead that responded tothe call " I know for certain that we did NOT go en route 6 minutes after being dispatched. That would mean we got allthe way to Linden (11.3 miles according to our trip odometer) in less than 5 minutes. Didn't happen."Can you review it and let me know if you see something I don't? The accuracy of these times is huge for us so I want tomake sure we have accurate times in the CAD. If the crew has different times due to radio or other issues how do we getthose noted in the CAD?
