Supplemental Life Insurance
About your plan
– Employee - Increments of $5,000. Maximum amount is $300,000. Minimum is $10,000.
– Spouse - Increments of $5,000. Maximum is lesser of $250,000 or 100% of the amount of your Supplemental Life Insurance amount. Minimum is $10,000.
– Dependent children - $5,000 or $10,000. Coverage is for each child. One rate for all children.
If you currently have Supplemental Life Insurance coverage, your coverage will continue with The Hartford. You must elect Supplemental Life Insurance for yourself to be able to elect Spouse and/or Child Supplemental Life Insurance.

One-time Open Enrollment with Guarantee issue Opportunity:
Employee – If currently participating, may increase current coverage up to $100,000. If electing for first time, may elect in amount of $100,000.
Spouse – If currently participating, may increase current coverage up to $30,000. If electing for first time, may elect in amount of $30,000.
Child(ren) – all Guaranteed Issue for increases or initial elections.
