About the Challenge

GOALS

• Encourage the development of digital health tools specifically for low‐income, complex populations

•Generate evidence that digital health tools can positively impact this group

PURPOSE

• Digital health is flourishing, but not focused on low income, high‐need individuals

•Many challenges faced by this population lend themselves to digital health solutions

• Functioning solutions that address one or more of the challenges faced by super‐utilizers

• Built out enough that could be piloted with consumers• Apps for Android, iOS, Blackberry, Windows phone, web, 

and mobile web; SMS‐based (text) solutions, and wearables

• Only US‐based companies• Demo video (hosted on YouTube or Vimeo) that walks 

through the main functionality of the submission via screencast or video

• At least one image/screenshot of submission

Connect individuals to healthy foods, reliable transportation, and wellness activities; address the knowledge gap related to selecting and preparing healthy foods; and/or tailor exercise 

for individuals with chronic conditions.
