_______________________________________________________________________________Ahh … now for that FINE PRINT … that you actually DO need to read . Please check each box as you read through our policy:

0 Activities will include painting, sanding wood, working with glass*, glass fusing tools* and possibly other sharp objects. We will give guidance and supervision with each activity, but we will not be responsible for accidents that happen during workshops.*Parents must sign a liability waiver for all glass classes. Please come inside the studio to fill out this form. If forms are not signed children will not be able to work on glass projects. When working with glass it is important for children to be able to follow directions and stay focused with the project. Please be advised that there is a probability for your child to get cuts while working with glass.

0 In order for Art Workshops to be enjoyable for ALL campers, the instructors and for our walk-in customers that may be painting in the studio during workshops, appropriate behavior and participation is expected. In the event that your child becomes disruptive or uncooperative, we will contact you and your child will be excused for the day. No refund will be given for that session.

0 A nonrefundable deposit of $50 is due at the time of registration. Balance is due two week prior to start of workshops.0 We will do our best to complete every project scheduled. Projects are subject to change depending on supply

availability and the time it takes for students to complete each project within the time frame. If your child does not finish during the scheduled time, he/she can return during regular business hours to complete the project.

0 Please be prompt in dropping off and picking up your child so that we can stay on schedule. There is no after or before camp care. If you are going to be late in picking up your child, please notify us through the main store: 203-880-9738. Also, please note that if you are late in picking up your child, we will need to charge you a $15 “Child Care Fee”.

0 Your child will get messy. Please be sure that he/she is dressed accordingly. 0 Pottery and Glass Fused projects will take about 7-10 days for the firing process. WE WILL CONTACT YOU

when the projects are ready for pick up. 0 Only registered students can stay in the designated area for workshops. This is meant to be a fun & educational

experience for your child and other campers! 0 I certify that I understand that Kre8art Studio photographs campers in the course of creating their projects and

that these photographs may or may not be used for marketing purposes. Images are the sole property of Kre8art Studio and no compensation will be given, in any form, for use of these images. A written request must be presented no later than the first day of your child's camp session if you do not wish for their images to be used in print, online or other marketing for Kre8art Studio.
