SECTION 1.0
INTRODUCTION This Summary Report describes off-site hexavalent chromium (CrVI) treatment using reductant solution injection activities completed as of May 30, 2016 by Honeywell International Inc. (Honeywell). All work has been performed per the approved “Former Bendix Facility Off-Site Hexavalent Chromium Groundwater Treatment Work Plan (Work Plan)”, dated April 17, 2015. The Site location map is shown on Figure 1 and remediation system and monitoring well locations are shown on Figure 2. The Work Plan was approved by the Regional Water Quality Control Board, Los Angeles Region (Regional Board) on August 24, 2015. The initial due date of January 7, 2016 was extended, due to delays associated with securing access, to June 15, 2016 (Regional Board, 2015 and 2016). The remedial approach involves injection of chemical reductant directly in groundwater to create a reductive zone that acts as a Permeable Reactive Barrier (PRB). The reductant solution mixes with and flows with groundwater, providing treatment in the reductive zone until all the injected substrates react with soil minerals. Subsequently as groundwater containing CrVI passes through the reductive zone [or high CrVI concentration pore water from the vadose zone enters the reductive zone] the CrVI is treated to the insoluble form of trivalent chromium (bound to aquifer solids/immobilized). The off-site CrVI treatment will involve introducing a reductant (calcium polysulfide [CaSx]) solution to the subsurface by means of an injection well (IW). Honeywell was issued a revised General Waste Discharge Requirement (WDR) Permit (R4-2014-0187) on January 15, 2016. The WDR Permit authorizes Honeywell to inject reductant solution and chase water in the off-site plume. This Summary Report describes a preliminary investigation conducted to characterize the off-site CrVI plume width and concentration gradients in the proposed treatment/barrier area to optimize the IW locations, in-situ treatment, and PRB location. The preliminary investigation proposed up to four groundwater grab sample locations in the vicinity of existing monitoring well GW-17, where elevated levels of CrVI is present in groundwater off-site. The groundwater grab sample results were used to locate the injection well(s) and provide additional information to design the IW screened interval for reductant solution injection to optimize remedial actions. The Work Plan also proposed installation of a groundwater monitoring well between the injection area and well NHE-2, an extraction well installed by the Los Angeles Department of Water and Power (LADWP) for the North Hollywood Operable Unit (NHOU) First Interim Remedy groundwater remediation system and water production. Currently, access agreement negotiations are in progress with an off-site property to install this monitoring well.

SECTION 2.0
PRELIMINARY INVESTIGATION The preliminary investigation was conducted between November 5 and December 18, 2015 which included drilling two borings for groundwater grab sampling (SB-35 and SB-36) and installation of one injection well (IW-4). All field activities were conducted under direct supervision of a California-professional geologist or California-registered civil engineer, in accordance with the Site-Specific Health and Safety Plan for Remediation and Groundwater Monitoring Activities (MWH, 2005), and in accordance with updated access agreements with current off-site property owners. 2.1 PERMITS, AGREEMENTS, AND UTILITY CLEARANCE Prior to field work, the following activities were conducted:
Finalizing amendments to existing access agreements with off-site property owners.
Well construction permits were obtained from the County of Los Angeles Department of Health Services. Copies of the permits are provided in Appendix A.
Underground Service Alert (USA) was notified (ticket # B53350332) and a utility clearance survey was conducted to ensure that the locations were clear of any detectable utilities.
Final sampling locations were modified based on field observations and USA clearances. In addition, each boring location was hand augured to a depth of approximately 5-feet below ground surface (feet bgs) to identify potential underground utilities that were not identified as an added precaution. 2.2 GROUNDWATER GRAB SAMPLES Elevated CrVI concentrations have persisted in groundwater at existing monitoring well GW-17(-282). The Site’s Third Quarter 2015 routine groundwater sampling event (Ramboll Environ, 2015) indicated 690 micrograms per liter (µg/L), in addition to historical concentrations at GW-17(-282), shown below.

Two borings (SB-35 and SB-36) were advanced to groundwater to collect groundwater grab samples to help delineate the extent of off-site CrVI plume (Figure 2). The boring locations for collection of groundwater grab samples were placed east and west of existing monitoring well GW-17, with the distance and orientation from the well based upon historical and recent isoconcentration contours of CrVI in groundwater. The following sections summarize the drilling and sampling methodologies. 2.2.1 Borehole Drilling and Sampling SB-35 and SB-36 were advanced by Sonic drilling methods to 320 and 285 feet bgs, respectively. At each boring location, continuous core samples were collected for soils classification and the lithology was logged according to the Unified Soil Classification System as presented in American Society for Testing and Materials Standard D 2488, and classified by color using a Munsell Color Chart, with the percent fines estimated and recorded. Boring logs are presented in Appendix B. No soil samples were collected for laboratory analysis. Groundwater samples were collected in appropriate laboratory-provided sample containers via discharge tubing. All samples were capped, labeled, sealed in plastic bags, placed in ice-chilled, insulated coolers to maintain a shipping temperature of approximately 4 degrees Centigrade, and transported under a chain-of-custody protocol to Eurofins Calscience, a California state-certified laboratory. All sampling equipment were cleaned using a nonphosphate detergent, rinsed with tap water, and final-rinsed with deionized water before entering each borehole. All equipment was handled in a manner intended to prevent cross-contamination. Simulprobe groundwater samples were collected from each boring as summarized in Table 1; three groundwater samples (265, 300, and 320 feet bgs) were collected from SB-35 and two groundwater samples (265 and 285 feet bgs) were collected from SB-36. Field measurements of CrVI and water quality parameters (EC, pH, ORP, SC, DO, and turbidity) and off-site laboratory analysis for CrVI (following EPA Method 218.6) were collected on each grab groundwater sample collected. Following sampling, SB-35 and SB-36 were abandoned with bentonite-cement grout to 2 feet bgs and finished with a concrete cap to the surface. 2.2.1 Groundwater Data Evaluation The Work Plan indicated that if the CrVI concentration in groundwater at either SB-35 or SB-36 is equal to or greater than 100 µg/L, then a step-out boring would be advanced in that µdirection to the east and/or west (SB-37 and SB-38, respectively). If the result at SB-35 or SB-36 is less than 100 µg/L, then no step-out boring would be advanced. CrVI laboratory results were less than 100 µg/L in all groundwater grab samples collected. However, there was variability between the CrVI laboratory results and the CrVI field results (Table 1) where the CrVI field results were higher than the laboratory results. Variability in field and lab results may have occurred from the turbidity/cloudiness of the sample water and/or potential reaction/reduction of CrVI from sample collection time to laboratory analysis time. Therefore, groundwater grab samples were also analyzed in the laboratory for total chromium (following EPA Method 200.8) to evaluate the potential CrVI reaction/reduction that may have occurred from sample collection time to laboratory analysis time. The total chromium results from the

laboratory analysis were higher than the laboratory CrVI results indicating the likelihood that some reaction contributed to the variability. Therefore, based on CrVI laboratory results from groundwater samples collected at both SB-35 and SB-36 being less than 100 µg/L (in addition to the total chromium results in groundwater being less than 100 µg/L), no step-out borings were advanced. The results indicate that CrVI is limited to the GW-17 area and that one injection well, IW-4, would be adequate to target the CrVI hot spot in this area. Analytical laboratory reports are included as Appendix C. 2.3 INVESTIGATION DERIVED WASTE Soil cuttings, purge water, and decontamination water were generated during field activities. Soil cuttings were contained in 20 cubic yard covered bins located at the drilling site. Purge and decontamination water were contained in 6,000-gallon Baker tanks at the drilling site. All IDW was stored and properly disposed of off-site per the Work Plan following receipt of waste characterization sample results.

SECTION 3.0
WELL INSTALLATION One IW location (IW-4) was installed for reductant solution injection in the vicinity of GW-17 (Figure 2). Well installation field activities were completed in general accordance with the Work Plan (MWH, 2015), the Site-Specific Health and Safety Plan (MWH, 2005), and under the direct supervision of a California Professional Geologist. Well installation was performed in general accordance with procedures outlined in Bulletin No. 74-81, Water Well Standards: State of California, and Supplement 74-90, California Well Standards (DWR, 1981 and 1991) and the procedures outlined below. 3.1 PERMITS, AGREEMENTS, AND UTILITY CLEARANCE Prior to field work, well construction permits were then obtained from the County of Los Angeles Department of Health Services and the City of Los Angeles. Copies of the permits are located in Appendix A. Amendments to existing access agreements with off-site property owners were finalized. USA was notified (ticket # B53350332) before the start of drilling activities to ensure that the location was clear of any detectable utilities. In addition, a utility clearance was conducted on each boring location to clear the borehole location of any detectable utilities. Also, the upper 5 feet bgs was cleared at each boring using a hand auger as an extra precaution to ensure that no utilities were present in the drilling location. Final well locations were modified based on field observations and USA clearance. 3.2 INJECTION WELL INSTALLATION One injection well, IW-4, was installed between January 11 and 23, 2016 (Figure 2). The construction of IW-4 is similar to previous wells installed on-site (IW-1). Well construction details are summarized in Table 2 and shown on well completion logs included as Appendix B The following sections summarize the drilling, sampling, and well construction procedures. 3.2.1 Borehole Drilling and Sampling IW-4 was installed using the air-rotary casing hammer drilling method. Groundwater samples were not collected during borehole drilling. Soil samples were collected for soil logging purposes only. Soil was characterized according to the Unified Soil Classification System as presented in American Society for Testing and Materials Standard D 2488, and classified by color using a Munsell Color Chart. 3.2.2 Injection Well Construction IW-4 was constructed using new flush-treaded, 8-inch-diameter Schedule 80 polyvinyl chloride (PVC) casing and 0.030-inch slotted Stainless Steel screen. A flush-mounted base cap was attached to the base well casing and screen assembly. IW-4 was constructed by lowering the screen attached to casing to the desired depth and then filling the annual space between the screen and borehole wall with #2/12 filter packs around the well screen intervals, bentonite seal material, and then bentonite-cement grout to ground surface. A brass security cap attached to the top of the well.

3.2.3 Well Development and Surface Completion The well screen was developed a minimum of 48 hours after the well was installed. IW-4 was developed following standard well development procedures, which included bailing and swabbing to remove heavy sediments followed by pumping until measured turbidity was less than 100 NTUs and measured water quality parameters stabilize with less than 10% variation for three consecutive readings and at least five well casing volumes of groundwater are removed. Approximately 1,400 gallons of groundwater were removed during well development. The surface was completed temporarily with a traffic-rated plate pending construction of the well vault, valves, and conveyance piping (described below). Upon well surface completion, the well will be surveyed for x, y, and z-coordinates based on the California State Plane Coordinate System (NAD83) and North American Vertical Datum of 1988 (NAVD88). 3.3 INVESTIGATION DERIVED WASTE Soil cuttings, purge water, and decontamination water were generated during field activities. Soil cuttings were contained in 20 cubic yard covered bins located at the drilling site. Purge and decontamination water were contained in 6,000-gallon Baker tanks at the drilling site. All IDW was stored and properly disposed of off-site per the Work Plan following receipt of waste characterization sample results. 3.4 FUTURE INJECTION WELL VAULT COMPLETION IW-4 will be completed within an approximately 4-foot square subsurface vault with drain sump to subsurface. As described in the Work Plan, the conveyance piping will pass through the vault, with a tee and valve from the conveyance piping to deliver solution to the well. Similar to the on-site IW (IW-1), a 2-inch diameter, 20-foot long section of solid drop pipe will be installed at the top of the well to recover some of the hydraulic head potential, reducing or eliminating the need for pumping. However, if solid pipe continued further, vaporization of the water and cavitation could result, damaging the pipe; therefore, similar to the on-site system, a 2-inch flexible hose will extend to a depth of 265 feet, approximately 10 feet below current groundwater level. As a vacuum forms, the pipe collapses, restricting flow, and dissipating the energy. This also prevents aeration and splashing of free-falling water in the well. This configuration performed well in IW-1.

SECTION 4.0
FUTURE ACTIVITIES The following activities were outlined in the Work Plan and are anticipated to be conducted in the near future, however, have not been completed to date. Reductant solution injection will commence upon completion of monitoring well installation and reductant solution injection system construction anticipated in third quarter 2016, pending access agreement negotiations with off-site property owners. 4.1 GROUNDWATER MONITORING WELL INSTALLATION Installation of a monitoring well (GW-36) downgradient of the proposed application area off-site and upgradient from pumping well NHE-2 was proposed in the Work Plan. Due to several obstructions in the vicinity of the originally proposed well location, a revised well location was identified as shown on Figure 2. However, access agreement negotiations with the off-site property owner are ongoing and monitoring well installation activities have not been completed. The revised WDR requirements permit includes monitoring at GW-36, and therefore, injection activities will not take place until the monitoring well has been installed. Monitoring well GW-36 will be constructed similar to nearby monitoring well GW-12B (see Table 2). The details of the monitoring well installation will be provided in a subsequent report. 4.2 REDUCTANT SOLUTION INJECTION SYSTEM CONSTRUCTION The off-site direct injection water source will be obtained from the extracted NHE-2 water and the RO system/CaSx storage is proposed to be located at the NHE-2 wellhead within the existing fenced-area. This set-up will allow for discharging the RO concentrate containing elevated TDS along with the extra NHE-2 water extracted (currently pumping at an average rate of 125 gpm) to the existing sanitary sewer connection under the current City of Los Angeles Bureau of Sanitation (LABOS) discharge permit for NHE-2, upon providing updated treatment system drawings/information to LABOS. The RO system will be used to provide a continuous source of DI water to mix with the reductant (to prevent mineral precipitation), along with an aboveground storage tank (AST) to store the reductant, a metering pump to provide required reductant dosing in-line to generate the reductant solution for injection, and conveyance piping to the injection location. An access agreement with the LADWP is in progress, to authorize these construction activities. Construction activities will commence upon finalization of the access agreement. 4.3 REDUCTANT SOLUTION INJECTION As outlined in the Work Plan, a MODFLOW/RT3D reactive transport model prepared by Mutch Associates, LLC included various runs to simulate various reductant solution injection volumes which also included distribution of reductant solution migration and areal extent of soil oxidant demand (SOD) created over time. The reactive transport model was refined based on previous results from the IW-1 reductant solution injection pilot test. Using the model predictability and outcome as guide, the following injection volumes have been planned:

2.6 million gallons of a 1.5% by mass CaSx (approximately 104,000 gallons of the 29% by weight, as delivered) reductant solution for 72 days at a continuous 25 gallons per minute (gpm) injection rate followed by
5.2 million gallons of water (approximately 144 days at a continuous 25 gpm injection rate).
The Monitoring and Reporting requirements of the revised General WDR Permit require adequate upgradient, application area, and downgradient monitoring. These monitoring locations are shown on Figure 2. Monitoring will be conducted to evaluate the results of the proposed reductant injection to verify CrVI treatment in the vicinity of monitoring well GW-17 and upgradient of well NHE-2. Model also predicts and it will be further verified that this reductive zone south of the Site will perform as a long-term offsite permeable reactive barrier (PRB) in groundwater.

Condition Upon Receipt: Samples were received under Chain-of-Custody (COC) on 12/14/15. They were assigned to Work Order 15-12-1058. Unless otherwise noted on the Sample Receiving forms all samples were received in good condition and within the
