Postvention (assumptions)

All involved with suicide will follow reporting procedures as outlined in Naval instructions. CACO functions pursued/accomplished Postvention is not just the immediate responses to event, but long term follow through with survivors to mitigate further issues and problems in the future.

I am going to only focus only postvention issues so there are some assumptions I am making. All will follow the guidelines for reporting as laid out in our Policies. The CACO will function as outlined in that policy. That you will accept the fact that postvention is not a quick hit checklist item, but a longer ongoing process after a suicidal event.

Assumptions

Historically we have focused on prevention and intervention in Navy.

Postvention is directly related to prevention with:

-survivors of successful suicide-survivors of attempts

As I looked through most of the literature related to suicide in the military there is little dealing with postvention. Share section in OPNAV 1720 on “Response” There is not much detail or specifics here as far as how to conduct a postvention.

Postvention (defined)

Is all interventions that attempt to reduce the negative consequences that may affect those close to the victim after a suicide has occurred.

Is psychological first aid for those bereaved by suicide. It’s purpose ultimately is to restore hope in a person or community devastated by a suicidal event.

Postvention (definition)

A postvention is an intervention conducted after a suicide, largely taking the form of support for the bereaved (family, friends, professionals and peers). Family and friends of the suicide victim may be at increased risk of suicide themselves.The aim is to support and debrief those affected; and reduce the possibility of suicide contagion. Interventions recognize that those bereaved by suicide may be vulnerable to suicidal behaviour themselves and may develop complicated grief reactions.Postvention includes procedures to alleviate the distress of suicidally bereaved individuals, reduce the risk of imitative suicidal behavior, and promote the healthy recovery of the affected community. Postvention can also take many forms depending on the situation in which the suicide takes place. Schools and colleges may include postvention strategies in overall crisis plans. Individual and group counseling may be offered for survivors (people affected by the suicide of an individual).

Affected individuals are often referred to as “survivors" of suicide.

A category that seems to be left out in postvention is survivors of suicide attempts. I believe this category also needs to be part of a postvention plan. There is huge discomfort when the person comes back to the command. People don’t know what to do for say, so there is always the possibility of isolation which may be the last thing the suicidal person needs. They need support and acceptance. When we had attempts in any of my commands I would sit down with the immediate work section and give a short Wounded Marine/Sailor brief. What do you do when someone is hit. First Aid, follow through and follow up. Mental health wounds are no different.

Loss from suicide is shocking and unexpected.

Ensuing grief is more complex, intense and long term.

All losses are tough on the psyche and the spirit, but suicide has the added intensity of being shocking and unexpected. This type of loss may bring on or worsen existing physical and mental health issues. The grief is more prolonged and complicated. Symptoms include extreme focus on the loss, problems accepting the death, preoccupation with what could I have done differently to prevent the suicide, feelings that life has no purpose and a lack of trust in others. There are layers to the grief that is much more severe and complicated.

There are layers of grief involved.

1. Suicide avoidable2. Suicide intentional3. Suicide unanticipated4. Stigma and shame come with it

The first layer relates to suicide in our own mindset as being avoidable. Grievers feel responsible and deal with guilt for not doing or saying the right things to prevent the suicide. As things sometimes fall into place afterward as to reasons the self blame and finger pointing games may begin. The second layer relates to the intentionality of suicide. Some suicidal people may have second thoughts, but too late. Those left to grieve may feel betrayed, abandoned and rejected which leads to anger. The third layer has to do with the fact that suicides are rarely anticipated which leads to a search for answers to the why question. Anxiety, fear and vulnerability is generated. With suicide comes stigma and shame. Attitudes are better than they used to be, but there is still much room for bad responses or no response at all by people to suicide survivors.

Long term follow up needed to mitigate issues that may crop up in certain individuals after those not greatly affected by suicide return to normal.

It takes much longer to recover so long term follow up is needed. This may include therapy and support groups for suicide survivors. Recovery from suicide is not passive. It does not just happen by letting things run their course. Recovery takes a long time until a person can live with the grief and not just the grieving. Normal grief process 1-2 years. I would say for the suicide survivor much longer.

Ease trauma and related effects to survivors.Reduce the onset and impact of psychological / spiritual distress and disorders. Minimize risk of further suicidal behavior.Encourage resiliency and coping skills.

Postvention is necessary to ease trauma. Reduce the onset of long term distress which may lead to disorders. Suicide contagion, or the copycat effect of suicide is seen in younger high risk teens. Since that is part of our population we need to take that risk seriously. We want to build resiliency and coping skills into the force.

Postvention given in a strained environment not only because of sudden and unexpected loss, but it is an event that people don’t want to talk about.

Important to remember that postvention takes place in a more strained environment than normal death not only because more sudden, but most people are uncomfortable with it and don’t want to talk about it or keep talk to a minimum.

Immediate needs of suicide grievers. 1. Know what they are feeling is normal reaction to abnormal situation.

2. Support: preferably from other survivors.

3. Time to grieve: 1-3 days funeral leave not enough.

Postvention (procedures)

It is clear to me that everyone needs to be part of the outreach effort. Everyone should know what to watch for: anger, moodiness, insomnia, nightmares, gallows humor or none at all, jumpiness, suspicion, withdrawal, loss of appetite; how to ask if someone wants to talk, and what to do if they need to but don’t want to. We want people to know they are needed, and that they can get help anonymously at www.militaryonesource.mil, or www.militarymentalhealth.org, www.health.mil.These are things all chaplains know and do, every day. We in medicine know that, as the old French saying puts it, our job is to ‘‘cure sometimes, relieve often, comfort always.’’ But we do not always succeed. We need your help.So thank you for counseling our wounded; thank you for bringing home our lost sheep; thank you for counseling our caregivers. One hand washes the other. The great military surgeon Ambrose Pare said, ‘‘I dress the wound, God heals it’’. We are now struggling to support those who are struggling. We need your prayers. Whether the wound is of the flesh, or of the mind we only dress it. God heals it.
