What are your strategies for change?
1. What change are you trying to make with your approach that will improve the health of your community?
2. What CTG strategic direction(s) does it impact?
3. Does it fit with a broader vision or campaign (CTG or otherwise) and how?
4. Who are the organizational and individual partners that are key to the success of your approach?
What are your strategies for change?
5. Who in your community will benefit most from this approach? How does it address health equity?
6. When did your work on this begin? Were there any catalysts that helped jumpstart it? How will this approach be fully implemented and when?
7. Is this work focused on a particular geographic area in your county (city/unincorporated area, zip code, etc.) and/or have an organizational/institutional or other target (e.g., schools/school district, youth orgs, clinics, public/market rate housing, etc.)? Why did you choose to focus there?
8. Do you have a theme or title for your approach?

Call Out Quotes1. Is there a community member/leader,
teacher/school official, business owner, department/county spokesperson or other individual who would help bring your approach to life?
2. What might this person(s) say? How will this change improve their health, the health of friends/family and the community in general?
3. Are these individuals available to provide quotes or appear in a video?
4. If your approach targets those with limited-English or where cultural relevance is an issue, do you have representatives of this group available?

• CA4Health will determine which counties are eligible for film production of their stories and will follow up with nominees
