THE DANCE STUDIO OF UNION PARISH 380 BERNICE HWY, SUITE 3 FARMERVILLE, LA 71241 Arrival & Attendance – Please arrange for students to arrive 5-10 minutes before class to have time to prepare for class. A student arriving late seriously disrupts a class & the student will miss out on a part of the lesson not to be repeated, including injury preventing warm-ups. Please inform the teacher if you are going to be even five minutes late. Students under the age of 11 should be accompanied by an adult in the lobby. Classes cancelled or missed may be made up during another class time during the week until January. Classes cancelled or missed January-May will not have make-ups unless instructor deems it necessary. Illness – If a student is contagious or miserable, do not bring the student to class. Please inform the teacher before the class begins of the absence as a courtesy. If a younger student does not feel well, do not bring student to class, as we want happy students and a class that runs smooth for all. If older students are not feeling well, but not contagious, they should come to class to observe during recital season (Jan- May). Liability/Insurance: The nature of dance/tumble is considered activity that may expose students to risk of injury. All students are required to be covered on their own policies. If an injury occurs, it is understood that The Dance Studio of Union Parish will not be held responsible. DSUP is also not responsible for property lost/stolen/damaged. Class Drop If a child is considering dropping, please inform the teacher in order to discuss the problem in hopes we can work together for a resolution. To be removed from a class roster, we require a “Drop Form” which must be turned in to the office. The form can be downloaded online or picked up in the office. The person responsible for the account payment will be responsible for all costume, recital, tuition, and late fees charged until the form is turned in.
