This form can be completed online and signed with a digital signature. Alternatively you can complete the form and print it, before signing and scanning the form for attachment to an email to [email protected]

3. Add your Bank Account Details Online

Payment of your Police Scholarship will be made by electronic funds transfer (EFT) from the University directly to your bank account. To ensure there is no delay in you receiving your scholarship payments go to Student Admin Online (if you have not already done so) and enter the bank account details of where you would like your scholarship payment deposited.

To add your bank account details in Student Admin Online:

1. click on 'Bank Details' 2. click on ‘check banking details’ 3. add your bank account details

4. remember to 'Save' changes 5. log out when finished

Note: If your banking details are not entered, your payment will be delayed.

Any account details changes should not be made after 5pm the Tuesday before your payment is due - update details after 9am on the Thursday.

By submitting this form electronically or by paper copy, I accept my offer of a scholarship as detailed in my Scholarship letter advising of the outcome and acknowledge that: • The decision to award the scholarship is based on the information I provided to the University when I lodged my application - the scholarship may be withdrawn if this information is found to be inaccurate • The scholarship may be withdrawn if I am unable to continue to satisfy the professional and academic criteria on which the decision to award the scholarship was based. • It is my responsibility to notify the University immediately of any incident, event or information which would affect my ability to satisfy the professional and academic criteria on which the decision to award the scholarship was based. • It is my responsibility to notify the University of any change in my financial circumstances as this may affect my eligibility to receive a scholarship and/or the level of my entitlement. • The University has the right to adjust my scholarship on a pro-rata basis if both my enrolment load and course duration are reduced (e.g. due to academic credit). • The University has the right to suspend, withdraw or terminate my scholarship if I fail to maintain satisfactory progress in my course or if I am found guilty of academic or general misconduct. • The scholarship will be withdrawn if my enrolment in the course ceases for any reason. • It is my responsibility to notify the University immediately if I withdraw from the course. • If I fail to notify the University of my withdrawal, I will be required to repay to the University any scholarship payment made subsequent to my withdrawal. • This scholarship awarded for the duration of your session 1 and 2 of your Associate Degree in Policing Practice studies, will only commence with payments upon your arrival to the campus in week 9. Policing pay calendars will be made available to you on payment dates.
