Clinical Placements in NSW Health facilities require students to complete a

verification process to ensure the safety of themselves, other staff and the

patients in their care. Illawarra Shoalhaven Local Health District (ISLHD) staff

will be attending your university/college early in your course to undertake this

process.

Please refer to your Education Provider for details of the verification session

applicable to you.

In order to be prepared, you will need to start now to collect documentation

as follows (please note: some documents may take a number of weeks to

source):

Student ID card (issued by current Education Provider)

National Police Check (see below)

Overseas Police check / Statutory Declaration (for Overseas students

ONLY)

Fully completed vaccination record card

Evidence of immunisation compliance (see next page)

Completed NSW Health Forms – Code of Conduct, PD2018_009

Attachment 6 & Attachment 7 (all included in this pack)

How to source an Australian National Police Check

Apply online at www.police.nsw.gov.au, present confirmation page and proof

of identification at police station and pay appropriate fee (currently $57.20).

This document is acceptable for 3 years from issue date.

OR

Apply for an online police check from any approved ACIC accredited broker (list

available from www.acic.gov.au). Please be aware that these checks need to

be verifiable online and many of these companies purge the data after 3

months (check the fine print) meaning that you need to present the certificate

within 3 months of issue date. Once verified online by ISLHD staff, the check

remains acceptable for 3 years from issue date.

International Students: Please speak to your Education Provider about

sourcing additional police check requirements. Statutory Declaration forms

are available from them if required.

Sources of Evidence for Childhood / Adult Vaccinations

Childhood vaccinations documentation can be sourced from:

Your Blue Book or childhood vaccination book from your state or country

(records presented in a foreign language must be translated using the

vaccine translation website (http://www.immunize.org/vis/vis_english.asp)

or using a local translation service.

Vaccination Record Card

The Vaccination Record Card for Health Care Workers and Students will be

provided to you by your Education Provider. This card should be completed in

full and correctly by a GP or Immunisation Nurse. Please see examples

overleaf.

Please bring ALL of your original documents PLUS a photocopied set

of all originals when attending any student verification session

CHECKLIST FOR VERIFICATION APPOINTMENT

□ Original and photocopy of education provider-issued student photo ID card

□ Original and photocopy of current Police Check from Australia (in exactly

the same name as student photo ID card) and

□ Original and copy of overseas Police Check OR statutory declaration

stating no charges or convictions in country of origin for international

students only

□ Original and copy of Clinical Placement Card if any disclosable

outcomes on police check

□ Original and copy of signed Code of Conduct for NSW Health

□ Attachment 6 Undertaking/Declaration form AND originals with copies of

documented evidence of vaccinations and serology records (printed blood

test results) as follows:

□ dTPa (diphtheria, tetanus, pertussis) – at least one documented dose

in last 10 years

□ MMR (measles, mumps, rubella) – 2 documented doses at least one

month apart OR positive serology for measles mumps and rubella OR

born before 1966

□ Varicella – 2 documented doses at least one month apart OR 1 dose

of varicella received before age 14 OR positive serology for varicella

□ Hepatitis B – at least first dose documented for temporary

compliance only OR completed full age appropriate course AND

positive serology taken at least 4 weeks after last dose for full

compliance (if all attempts fail to obtain documented evidence of full

age appropriate course of Hepatitis B vaccination, then fully

completed statutory declaration AND positive serology result may be

presented) Note: Hepatitis B compliance pathway is attached

□ Attachment 7 – Tuberculosis risk form – A completed risk assessment form

will be required for this assessment. No further screening tests are

required prior to this session. Those students needing further screening will

be advised of their follow up plan and can be made temporarily compliant.
