Completing the Serious Injury Form The form fields are a mixture of pick lists, radial buttons, drop downs, tick boxes and free text. 1.1 To use the pick lists, click on the magnifying glass, and you will either be presented with a list immediately or be asked to insert a value to allow you to further define the list. Once you have a list displayed, select one of the options. 1.2 To use the radial buttons simply select the option which is applicable. You will only be able to select one answer. 1.3 To use the drop downs, click on the arrow at the side of the fields, and select the option from the list. If the option you require is not there leave it blank or select other and specify in the corresponding ‘other’ field (generally a free text field). 1.4 To use the tick boxes, simply tick the options which are applicable. Number of options may or may not be limited. 1.5 Free text fields allow you to type in your response to the question. 1.6 Important Note - Prior to data input you should have the following information: Player’s name Details of the injury e.g. date of injury, type of match, position playing whilst hurt, what type of tackle, which direction was the tackle from, was the player wearing any protection etc... Details of the treatment e.g. pitch side treatment if any, admission/release dates from hospital, name of hospital, body parts affected etc...

PART G:SERIOUS INJURY REPORTING

1. Completing the Serious Injury FormThe form fields are a mixture of pick lists, radial buttons, drop downs, tick boxes and free text.

1.1 To use the pick lists, click on the magnifying glass, and you will either be presented with alist immediately or be asked to insert a value to allow you to further define the list. Onceyou have a list displayed, select one of the options.

1.2 To use the radial buttons simply select the option which is applicable. You will only beable to select one answer.

1.3 To use the drop downs, click on the arrow at the side of the fields, and select the optionfrom the list. If the option you require is not there leave it blank or select other andspecify in the corresponding ‘other’ field (generally a free text field).

1.4 To use the tick boxes, simply tick the options which are applicable. Number of optionsmay or may not be limited.

1.5 Free text fields allow you to type in your response to the question.

1.6 Important Note - Prior to data input you should have the following information: Player’s name Details of the injury e.g. date of injury, type of match, position playing whilst hurt,

2. System Access2.1 To access the Serious Injury Report online use the following URL:

http://www.rugbyregistration.co.uk/SRPRS/Logon.aspx.2.2 There are two ways in which you can login into Serious Injury Report:

2.2.1 Generic login - this has been created to allow anyone to add a seriousinjury form. This login can be obtained from your club/school datamanager. Once you have logged in the following screen will bedisplayed. To access the serious injury form you should click on theserious injury report link.

2.2.2 Current club login details - C1 or C2 roles only. Once logged in yourcommunity page will be displayed. To input a new serious injury formyou should click on the Serious Injury tab.

3. Information TabThis tab collates information about you - the recorder of the injury, about the player and anyother additional information that is not recorded elsewhere in the form.

3.1 Within the information tab in ‘About You’ you should input your name in the field ‘Whatis Your Name’ and your contact phone number in the field ‘What is your Contact phoneNo’. Note: both these fields are mandatory.

3.2 The field ‘Name of you Club/School’ is a pick list. You should click on the magnifying glasssymbol which will provide the following screen:

3.3 Type the name of your club or school in the value field, and then click on the green andwhite arrow button. Tip: You can put in partial details relating to the club name forexample the first two letters, then click on the green/white arrow button and a list ofclubs/schools will be presented.

3.4 Once you have selected your club/school, you should then select your position from thedrop down list, by clicking on the arrow next to the field, as shown below:

If you are not a keycontact andtherefore do nothave an allocatedposition, you shouldselect N/A.

3.5 You should now begin to insert the player details within ‘Player Information’. Insert theplayers surname in the player surname field, and attach his/her club by following thesame process detailed in step 3.2 of this user guide. The player with that surname willnow appear in the area below. For data protection, only the fields shown will becompleted, no other fields such as date of Birth or address can be provided.Note: If the player does not appear or an incorrect player is presented, you can stillcontinue to complete the form without attaching the player details.

3.6 Prior to selecting the player which the report will be against, complete both question 3and four. Question 4 has been introduced by the insurance provider, to allow them todetermine whether the player will require loss of earnings, due to time off work as resultof the injury he/she has suffered.

3.7 To select the player, tick the box next to the correct individual.

3.8 Questions 8 and 9 allow you to insert any additional information which will not beobtained through the remaining report tabs and the name and address of theClub/Schools’ Dr or Players GP.

4. Injury CircumstancesThis tab collates the circumstances in which the injury happened.

4.1 Date and time of injury should be the date and time that the injury occurred. To input thedate, click on the calendar icon, you will then be able to select the date you require; thetime should be input in 24 hour format e.g. 6pm would be 18:00.

4.2 The opposition team field is a pick list. To select the team, click on the magnifying icon,and input the name of the club/school in the value field and click on the green and whitearrow. You will then be presented with the club or a list of clubs to select from (see screenillustrations of the process in section 3).

4.3 The next field is a new field as a result of the 2013 development, ‘What kind of Pitch wasit’. This is a drop down, and to view the options click on the arrow (indicated here):

4.5 The remainder of the tab continues to ask questions re the injury circumstances. Notethat none of the fields on this tab are mandatory therefore, if you do not know whichoption to select, leave it blank.

5. Injury InformationThis tab collates information about the injury and the parts of the body that are affected.

5.1 The first question on this tab is a drop down menu, and asks what side the injury affects.Question 2, 3 and 4 ask you to advise us where the injury was (region), the location of theinjury within the selected region (location) and the diagnosis (description).

5.2 For the primary body part that is affected click on the magnifying class, next to region (ashighlighted below):

5.3 You will then be presented with the following list:

5.4 Highlight the region of the body where the injury is, and click on select. The next pick list(location) is based on the region that you have selected. For example purposes, Head &Neck has been selected and the location’s for this region are as follows:

5.5 Highlight and select the location which has been injured and again, the options for thenext pick list – description – are determined by the option which you select withinlocation. For example purposes, ear has been selected and the description option for earare as follows:

5.6 Once you have selected the region, location and description for the primary body partwhich was injured, you can then go onto select these for secondary and tertiary bodyparts if applicable.

6. Treatment DetailsThis tab collates information about the treatment that the player received at the time of injuryand after.

6.1 Question 1 is a radial button. Simply select the option which is applicable and if theanswer is yes, you should provide further information on the pitch side treatment in 1b.Question 2a and 2b are both radial buttons; again simply select the option which isapplicable.

6.2 Question 3 through to 6 are all about the hospital – admission date and time, whathospital was the player treated at, what treatment did he/she receive and when the

player was released. None of these fields are mandatory and therefore if you do not knowthis information, you can simply leave it blank.

6.3 Question 7 through to 12, asks you about the treatment/rehabilitation the player receivedafter hospital, the recovery period and if the injury stopped the player playing. These newquestions have been added to the system to allow us to gain information on what injuryaftercare is taking place. Again none of these fields are mandatory and therefore if you donot know this information, you can simply leave it blank.

7. Submitting the Form7.1 Once you have completed all four tabs as fully as possible, click on the submit icon, which

is highlighted below.

7.2 The following message will be displayed to confirm the form has been submitted. Click okto close this message and the form.

7.3 Once you have submitted the form, the following emails will be auto sent, from thesystem with the serious injury report attached:

a) Email to the club/school Data Manager if player allocated
