    Client: HDR Inc

    Project: Stewart International Airport (PFAS)

    Report Number: 280-88381-1

    This case narrative is in the form of an exception report, where only the anomalies related to this report, method specific performance and/or QA/QC issues are discussed. If there are no issues to report, this narrative will include a statement that documents that there are no relevant data issues.

    It should be noted that samples with elevated Reporting Limits (RLs) as a result of a dilution may not be able to satisfy customer reporting limits in some cases. Such increases in the RLs are unavoidable but acceptable consequence of sample dilution that enables quantification of target analytes or interferences which exceed the calibration range of the instrument.

    Calculations are performed before rounding to avoid round-off errors in calculated results.

    All holding times were met and proper preservation noted for the methods performed on these samples, unless otherwise detailed in the individual sections below.

    RECEIPTThe samples were received on 09/17/2016; the samples arrived in good condition, properly preserved and on ice. The temperature of the coolers at receipt was 16.6 C.

    The following samples was received at the laboratory outside the required temperature criteria: PFC-MW-4 (280-88381-1), CB2K (280-88381-2), CB9A (280-88381-3), W6 (280-88381-4), ANG-CB-EB-091516 (280-88381-5), D27 (280-88381-6), D15-A (280-88381-7), ANG-MW-EB-091516 (280-88381-8), ANG-CB-FB-091516 (280-88381-9), STM-UP (280-88381-10), CB (280-88381-11), CB (280-88381-11[MS]), CB (280-88381-11[MSD]), C17 (280-88381-12), D8 (280-88381-13), CB-DUP (280-88381-14), PFC-MW-2 (280-88381-15), MW-DUP (280-88381-16), PFC-MW-3 (280-88381-17), PFC-MW-3 (280-88381-17[MS]), PFC-MW-3 (280-88381-17[MSD]), CB-FB-0914-16 (280-88381-18), CB-EB-091416 (280-88381-19), ANG-CB-EB-091616 (280-88381-20) and ANG-FB-091616 (280-88381-21). The client was contacted regarding this issue, and the laboratory was instructed to proceed with/cancel analysis.

    Cooler received with not enough ice and ice melted, out of temp at 16.6°C.

    The container label for the following sample(s) did not match the information listed on the Chain-of-Custody (COC): D8, SAMPLE 13 The container labels list DB-8, while the COC lists D8.

    Note: All samples which require thermal preservation are considered acceptable if the arrival temperature is within 2C of the required temperature or method specified range. For samples with a specified temperature of 4C, samples with a temperature ranging from just above freezing temperature of water to 6C shall be acceptable. Samples that are hand delivered immediately following collection may not meet these criteria, however they will be deemed acceptable according to NELAC standards, if there is evidence that the chilling process has begun, such as arrival on ice, etc.

    PERFLUORINATED HYDROCARBONS (PFC)Samples PFC-MW-4 (280-88381-1), CB2K (280-88381-2), CB9A (280-88381-3), W6 (280-88381-4), ANG-CB-EB-091516 (280-88381-5), D27 (280-88381-6), D15-A (280-88381-7), ANG-MW-EB-091516 (280-88381-8), ANG-CB-FB-091516 (280-88381-9), STM-UP (280-88381-10), CB (280-88381-11), C17 (280-88381-12), D8 (280-88381-13), CB-DUP (280-88381-14), PFC-MW-2 (280-88381-15), MW-DUP (280-88381-16), PFC-MW-3 (280-88381-17), CB-FB-0914-16 (280-88381-18), CB-EB-091416 (280-88381-19), ANG-CB-EB-091616 (280-88381-20) and ANG-FB-091616 (280-88381-21) were analyzed for Perfluorinated Hydrocarbons (PFC) in accordance with DV-LC-0012. The samples were prepared on 09/27/2016 and analyzed on 09/28/2016 and 09/29/2016.

    13C4 PFBA (ISTD) retention times for the following samples were outside the acceptance criteria of +/-0.5 minutes from the mid-point of the initial calibration: MW-DUP (280-88381-16), PFC-MW-3 (280-88381-17), PFC-MW-3 (280-88381-17[MS]), PFC-MW-3 (280-88381-17[MSD]), CB-FB-0914-16 (280-88381-18), CB-EB-091416 (280-88381-19), ANG-CB-EB-091616 (280-88381-20), ANG-FB-091616 (280-88381-21), (LCS 280-343904/2-A), (LCSD 280-343904/3-A) and (MB 280-343904/1-A).

    This particular IS is associated with PFBA which was selected based on retention time of 13C4 PFBA. The CCV's are just in control, while the samples and batch QC are just over the limits of being 0.5minutes.

    The samples failed for internal standards, the method blank and LCS were in control..

    Internal standard responses were outside of acceptance limits for the following sample: D27 (280-88381-6). The sample(s) shows evidence of matrix interference.

    PFC preparation batch 280-343884 and analytical batch 280-344348

    The sample failed for % recovery for several internal standards, the method blank and LCS were in control.

    The surrogate recovery for the blank associated with preparation batch 280-343884 and analytical batch 280-344280 was outside the upper control limits for 13C8 PFOS at 131% (limits 45-130). All sample surrogates were in control, therefore, the data has been reported with narrative.

    Internal standard responses were outside of acceptance limits for the following samples: PFC-MW-4 (280-88381-1), CB2K (280-88381-2), CB9A (280-88381-3), W6 (280-88381-4), D27 (280-88381-6), D15-A (280-88381-7), ANG-CB-FB-091516 (280-88381-9), STM-UP (280-88381-10), CB (280-88381-11[MS]), CB (280-88381-11[MSD]), C17 (280-88381-12), D8 (280-88381-13), CB-DUP (280-88381-14) and PFC-MW-2 (280-88381-15). The samples show evidence of matrix interference. Failing internal standards are not associated with the clients requested target analytes. Report with narrative.

    3C4 PFBA (ISTD) retention times for the following samples were outside the acceptance criteria of +/-0.5 minutes from the mid-point of the initial calibration: CB9A (280-88381-3), ANG-CB-FB-091516 (280-88381-9), STM-UP (280-88381-10) and (LCS 280-343884/2-A). This IS is associated with compound PFBA which is not on the client's requested target analyte list. Report with narrative.

    13C8 PFOS failed the surrogate recovery criteria high for MB 280-343884/1-A. Refer to the QC report for details.

    Sample D27 (280-88381-6)[4X] required dilution prior to analysis. The reporting limits have been adjusted accordingly.

    A deviation from the Standard Operating Procedure (SOP) occurred. Details are as follows: The PFC LCS was not verified at time of extraction.
