STEM EXTENSION FILING INSTRUCTIONS When you receive the STEM Extension recommended I-20, sign in the appropriate place then prepare your packet to be mailed to the appropriate USCIS lockbox location. See the bottom for lockbox information. Please note that it is your responsibility to ensure that all application materials are received by USCIS prior to the end of your current OPT Authorization. You can apply for STEM Extension 90 days before the end of your authorization. Once you are on STEM extension you are required to report to Bradley every 6 months, an email will be sent to the email address on file so make sure you keep your email address up-to-date and that you check it regularly.

The materials to include in your STEM Extension application packet (in order from on top to on the bottom) are:

Form G-1145 “e-Notification of Application/Petition” o Form can be found at https://www.uscis.gov/sites/default/files/files/form/g-1145.pdfo Place on top of the applicationo Not required but recommended

Form I-765 “Application for Employment Authorization” – Sample included o Form can be found at https://www.uscis.gov/sites/default/files/files/form/i-765.pdfo It is recommended that you complete the form online and print.o For any section that does not pertain to your, fill in with “N/A” or “None”.o Print the Form I-765 and SIGN INSIDE THE BOX IN BLACK INK.

Check or money order for $410.00, made payable to U.S. Department of Homeland Security

Two passport-style photos that are less than 30 days old o Guidelines online at https://travel.state.gov/content/travel/en/passports/requirements/photos.htmlo On the back, lightly write your name in pencil or felt-tip pen

Copy of the front and back of current Employment Authorization Document (OPT Card)

Copy of the new I-20 with the STEM Extension recommendation on the 2nd page o Must be less than 60 days oldo Make sure to sign on the first page

Copies of all Bradley I-20s issued to you o Ordered newest on top to oldest on the bottomo Not required but highly recommended, especially if you have previous CPT/OPT authorization

Copy of Bradley Diploma or final transcripts showing degree awarded date

Copy of passport biographical and expiration page(s)

Copy of F-1 visa

Copy of most recent Form I-94 o Form can be found at https://i94.cbp.dhs.gov/I94/#/recent-search
