How to Use this Directory

What is this directory for?• The purpose of this directory is to help youth and their families who have been involved with the family court system find services for youth in their communities. Case managers can use this directory to guide their clients in finding service providers in or near their neighborhoods.

This directory contains information about:•

Youth Service Providers (YSP) – Organizations with after-school, recreational, cultural, job/skills training and other programs.

Mental Health Providers (MHP) – Organizations that offer different types of mental health treatment, including individual counseling, family counseling, case management and substance abuse.

Teen Health Free Clinics (THFC) – Health clinics that provide free health care for teens such as STD and HIV testing, birth control, pregnancy tests, and gynecological services.

Parent Resource Centero – Center that provides parents of youth with family support services. There is only one center per borough (not mapped).

How is the directory organized?• The directory contains a map and a listing of services for each community district within this borough. The neighborhoods that are located within each community district are identified in the Index of Maps, the left top corner of the maps, and the right top corner of the listings page.

To find a service provider near the youth’s home:•

Find out the neighborhood where the youth resides and find the map(s) that include that neighborhood.o Look at the map with the youth and/or parent/guardian and find their home address in the map.o Discuss the needs of the youth and identify fitting service providers. Sometimes more than one provider is o located at the same address. Circle service providers in the handout, and include with plan.o If youth stays less than 11 days, send the map with instructions to the parent/guardian.o The map also has subway lines and stations. The listings include information about the nearest stations and o bus stops to the service provider.
