Coon has served as project coordinator and project manager for all types of highway, infrastructure, and land surveying projects. He manages the Huitt-Zollars office in Oklahoma City which serves ODOT, Circuit Engineering, municipal, and private sector clients. Circuit Engineering District 7 Roadway Projects Coon serves as Principal for two projects involving roadway widening and resurfacing. They are 5.5 miles, CR N259 and CR 6060 in Blaine County and 7.0 miles of CR N174 and Louis Road in Harmon County. SH 66 Widening at Saints Boulevard – City of Edmond, Oklahoma As Project Manager, Coon was responsible for the plan design of .75 miles of Saints boulevard and .50 miles of SH 66 widening. Improvements to SH 66, immediately east of IH-35, included widening for right and dual left turn lanes, a new signalized intersection, channelization, traffic signals, crosswalks, utilities, and stormwater collection facilities. Classen Boulevard Widening – Oklahoma City, Oklahoma As Project Manager, Coon was responsible for the development of plans to widen the Classen Boulevard Intersection at NW 36th Street. Improvements included intersection widening and channelization, new traffic signals, pavement overlay, pedestrian crossing, and minor utility/drainage improvements. Downtown Project 180 – Oklahoma City, Oklahoma Coon served as Project Manager and Project Coordinator for providing civil engineering services to the City of Oklahoma City for the large scale downtown revitalization project. This extensive project in downtown Oklahoma City involved reconstruction of one mile of Robinson Avenue, utilities, sidewalks, landscaping irrigation, and amenities. The scope of services included data collection, surveying, streetscape, drainage, and utility design. Coon also worked with the City to identify easements and coordinate construction issues with adjoining business owners. Dooley Farms Lane and Edmond Road Improvements – Edmond, Oklahoma As Project Manager, Coon prepared plans for the new location of Dooley Farms Lane (.80 miles) and improvements to .50 miles of West Edmond Road, east of Santa Fe Avenue. McClain County (J/P# 29498(04) 180th St. (May Ave. to High Ave. & Bryant Ave. to Johnson Ave.) To provide design and construction documents for 7.0 miles of pavement resurfacing in McClain County. The scope of work included providing a typical overlay section, a typical full depth patching section, and the details and notes necessary to accommodate driveways, mailboxes, quantities, etc., to properly construct this project. One of the key design elements was to identify the amount of existing roadway with subgrade failure in order to allocate the necessary patching materials for full depth pavement repair. Noble County (J/P# 23651(04) Red Rock Road (SH-64 to SH-15) To provide design and construction documents for 12.0 miles of grade, drain, and surfacing in Noble County. The scope of work included 12 miles of grade, drain, and surfacing for Red Rock Road in Noble County. Design services included survey, geotechnical investigations, pavement design, roadway plans for surfacing, right of way plans, and utility relocation coordination. Kay County (J/P# 22953(09) Waverly & Hubbard Roads To provide design and construction documents including survey, grade, drain, surfacing, widening, and storm sewer for 3.5 miles of roadway in Kay County. Project included open section roadway as well as curb & gutter section. Mark Hinderliter provided MB with the geotechnical recommendations for the subgrade and surfacing design. Bearing Tree provided MB with the necessary survey information. US-64 Houston Creek, J/P No. 26495(04), Woods Co. This responsibility began with a rewrite of the Huitt-Zollars Quality Manual in order to conform to the new version of ISO 9001:2008. She is a certified Quality Auditor. Pillar established an internal Quality SharePoint site to improve employee access to the company Quality Management System. SharePoint is a living resource and continues to evolve as needs are identified and revisions are necessary. Per the Huitt-Zollars Quality Manual, Pillar completed the cycle of internal office audits. Results produced a rewrite of the audit worksheet and an evaluation of the company quality objectives. Pillars ongoing responsibilities, as the company quality representative, include identifying and providing quality training, coordinating and conducting internal office audits, establishing quality plans, identifying and providing design checklist and review forms, assisting project managers and setting up internal quality work groups. One such work group is Records and Document Management; which is consolidating the company filing structure and defining what are project records versus documents. Pillar was the Quality Manager for the final design stage of the Dallas Love Field Airfield improvements and Landside Development projects. The final Quality reviews for $72 million of construction entailed: identifying the independent review team, conducting the kickoff meeting and training, providing a trackable review comment form, following the comments to completion and assembling hardcopy and electronic documentation of this effort. Other current assignments include serving as Quality Manager for projects with TxDOT, Sound Transit, US Army Corps of Engineers, and others.

Western Ave. Streetscape, City of Oklahoma City – Oklahoma City, Oklahoma Project planning and design began in 2007 as an improvement to a two mile stretch of Oklahoma City ADA accessible. Darr took over the project in 2012 as Huitt-Zollars project manager; the project soon became a project to enhance the overall use of the corridor for pedestrians and vehicles. Keeping the initial goal of accessibility, Darr completed the design with multiple alternates for the client. Darr worked closely with the client to accomplish the task at hand; required meetings involving public officials, business owners, and construction managers were necessary to complete the design task for the public improvements. The total construction cost for this project came in just over $5million and included sidewalks of varying sizes for ADA and pedestrian uses, a bridge widening, traffic signal modifications, landscaping, new parking, and driveway modifications. Project construction completion estimated September 2015.

30” Waterline Reno Ave, City of Oklahoma City – Oklahoma City, Oklahoma This project was approximately two miles of 30” Ductile Iron Pipe. HZ utilized survey and implemented 3-D CAD technologies in an effort to minimize underground conflict. A few unusual obstacles encountered such as a 20’ wide arched corrugated metal storm pipe that had collapsed and was in repair at time of our projects bid. Changes were needed to adjust pipe elevations and bedding in this area to ensure installation methods wouldn’t influence this storm line. High pressure gas lines, varying sizes of storm sewer, roadside channels, and overhead electric all warranted careful precision for utility crossings. Standard construction methods were designed for and regularly monitored during installation.

Athletics Complex, Crossings Christian Schools – Oklahoma City, Oklahoma The school decided to utilize acquired property for on-site athletics and came to HZ. Darr took over as project manager for a football field w/track and stadium as phase 1 of 3. The facility was to meet national sports criteria for hosting football, track and field, as well as soccer. Phase 2 of 3 brought on a field house which added locker-rooms, offices, weight training, film room, and medical training to the facilities. The building was positioned for a dramatic appeal from the perspective of athletes coming up onto the field. The last phase of the complex was baseball and softball fields. Originally the school was going to have to plan for softball to be located in a different area of the Crossings property and become a future phase. Through design, the client and design team were able to integrate the softball playing field on the site utilizing the outfield of the baseball field and. there is very little space on this 14 acre site that does not serve an athletic purpose.

Delaware Nation Casino, Delaware Nation Tribe – Hinton, Oklahoma Darr provided site design for this commercial site involving three phases of construction for casino buildings, storage buildings, hotel, and parking lots. Design was provided for drainage, utilities, sidewalks, roadway paving. City improvements to the access roads and waterline improvements were additional items necessary for construction.

Crossings NE Parking, Crossings Community Church – Oklahoma City, Oklahoma New parking was required for the client's growing community. Darr provided site design for 2.5 acres of parking which connected the entire site by way of a through-road. The improvement was constructed on what once was part of an existing pond, which was previously filled in by the client for future expansion. The uniqueness of this project required an under-drain system for the parking lot and frequent soil compaction studies.

8” Waterline Relocation, Kickapoo Tribe of Oklahoma & ODOT – Harrah, Oklahoma The Kickapoo Tribe was approached by ODOT to gain additional easement for State Highway 62. This required utility relocations to make way for a new roadway easement. Tribal land requirements needed to be considered during the design. The waterline by the end of design was to be 1/4 mile in length and required permanent easement negotiations due to the new roadway easement.

Western Avenue Improvements – Oklahoma City, Oklahoma This project included improvements to streets, sidewalks, and landscape. Cowan produced landscape and civil engineering plans along with others on the Huitt-Zollars team. AAA Operations Center – Oklahoma City, Oklahoma Cowan was responsible for irrigation design and plans for the AAA Operations Center project which was built to Silver LEED specifications. The project included irrigation design and plans for this ten-acre office building and associated parking development in northwest Oklahoma City. Oklahoma State University Okmulgee – Okmulgee, Oklahoma Cowan was responsible for landscape and trail design for this recreational trails project in Okmulgee, Oklahoma. The project included recreational trails and exercise stations layout and landscape plans. Glenbrook Park in Nichols Hills – Nichols Hills, Oklahoma Cowan was responsible for park and streetscape landscape plans for this residential subdivision. She produced all civil engineering plans under the direction of a Huitt-Zollars civil engineer. Block B/562 Neighborhood Park – Dallas, Texas Under the senior landscape architect, Cowan developed plans for this undeveloped neighborhood park in Uptown Dallas, Texas. The project included park layout, planting, walls, grading, fountain and pool with details, sidewalks, steps, benches, and handrails.

Rivera Rivera has experience in design and project management for municipal public works, infrastructure, environmental, and hydrology and hydraulics projects. Col John Ensor/Carrol T. Welch Pedestrian Improvements, Town of Horizon City – Horizon City, Texas Rivera served as the Project Manager for the design of approximately 6,000 linear feet of sidewalks, ADA ramps, and driveways. The plans and specification were completed in accordance to TxDOT design standards. The project was completed to comply with the Local Government Project Procedures to receive federal funds for construction. The project completes the sidewalk construction identified in the Safe Routes to School Plans developed for the elementary schools in Horizon City. This project brings into ADA compliance approximately 70 ramps in established areas of the Town of Horizon City. Rivera coordinated with TxDOT Environmental Department to obtain a categorical exclusion for environmental clearance. Texas Safe Routes to School Plan, City of El Paso, Socorro Independent School District, City of Socorro, and City of Horizon City—El Paso, Texas, Socorro, Texas, and City of Horizon City, Texas The Safe Routes to School program provides improvements to walking and bicycling paths for elementary and middle schools. Rivera served as deputy project manager for the planning process of the Safe Routes to School for the development of Safe Routes to School Plans for 69 campuses in El Paso County, promoting walking and bicycling to school. Rivera met with school principals and held public meetings with teachers, school faculty, students, and parents to discuss improvements to existing walking paths and routes that would enhance pedestrian safety in the area. As part of data gathering Rivera, utilized database information provided by the City of El Paso for pedestrian traffic related accidents. This information was geo-coded to create a map identifying the location of these accidents and develop solutions to improve safety in these areas. Surveys were distributed to parents and collected by school teachers to further assist with data gathering. This information was analyzed and field visits were conducted to verify existing traffic signs, crosswalks, and traffic conditions. Based on this information Rivera along with several other engineers developed drawings highlighting sidewalk improvements, new walking/riding paths, accessible routes, school zone flashers and street signs. A detailed report discussing existing conditions, proposed infrastructure improvements and cost estimate were distributed to school Principals, City of El Paso, City of Socorro, and Texas Department of Transportation. South Link LRT Extension TIA & Signal Design – SeaTac, Washington Rivera served as Project Engineer for a traffic impact study for a new transit station along the South Link light rail line near the SeaTac airport. The traffic study was required for environmental approvals by the state and FTA, and identified mitigation improvements to SR-99, S. 200th Street, Military Road, and other streets in the vicinity. Tasks included trip generation study at an existing nearby station, trip distribution and assignment studies, traffic volume forecasts using data from the Puget Sound Regional Council transportation model, turning movement counts, a gap analysis for potentially affected driveways, and preparation of several reports. Conceptual plans were prepared of all streets, pedestrian, and traffic signal improvements. The station consists of a new park and ride facility with up to 1100 parking spaces, a new traffic signal, a passenger drop-off area, and improvements for pedestrians and bicycles.

Eastern Avenue dam spillway footing experienced severe undermining and a part of stilling basin also received damage due to rainfall exceeding 500-year return period. Preliminary Damaged Assessment (PDA) was conducted following the rainfall to comply with FEMA requirement with anticipation that a Presidential Disaster Declaration is forthcoming. The Presidential Disaster Declaration – DR-4117 occurred on May 20, 2013 but later extended to cover damages due to storms and tornado of May 31, 2013. Agrawal’s group prepared preliminary cost estimates for funding and assisted FEMA officials in created Project Worksheets (commonly referred as “PW”) for approval of funds. Under his guidance the City Drainage and Flood Plain management staff prepared plans and specifications for the solicitation of bids. The Spillway and Stilling Basin was repaired and FEMA reimbursed the City. OWRB Inspections (per State Statute Title 785:25 and National Dam Safety Act) of the City of Oklahoma City Dams – Oklahoma City, Oklahoma Agrawald managed safety inspections for five high hazard dams and approximately 25 low and medium hazard dams per Oklahoma Statute pursuant to National Dam Safety Act (Amended 2006). Inspection of these dams was done by the city staff and then a consultant was hired to perform this task in 2009 under the Project Number WM-0110. Every high hazard dam inspection report had a condition assessment and list of issues requiring attention. These issues ranged from addressing minor issues of mowing and plant management to serious issues of damages to spillway and outlet structures. Many major issues required design of the repairs and preparation of plans and inspection for hiring contractor for the repairs. Many majors repairs required separate permits from OWRB for performing work and, upon completion of repairs, “Completion of Repair” notifications were also submitted to the Board. FEMA Funded Disaster Recovery DR 4117, May 20th Tornado and May 31st, 2013 Flooding and Tornado Damages, City of Oklahoma City – Oklahoma City, Oklahoma Agrawal managed Oklahoma City’s Flood Recovery project following the May 31, 2013 rainfall that resulted in city-wide damages to infrastructure bridges and streets. As the Chief Drainage and Bridge Engineer, he coordinated FEMA’s recovery effort through OEM (State Office of Emergency Management), closely assisted in completing H&H analysis, performed in-house design for all projects, obtained required 404 permits for all the locations by closely coordinating with Corp of Engineers offices located in in Tulsa Oklahoma. The infrastructure recovery project included 17separate locations with culvert/bridge damage/replacements, Channel Block Erosion Control System Failure, similar to the Imhoff Creek channel failures, and design solutions to replace the failed block systems with a superior Anchored Armored system. SH 6 Over Ditch “H” Bridge Replacement - TxDOT Houston District: Shuey served as Lead Structural Engineer for the three phase-bridge replacement of the existing limited freeboard, trestle-pile bent, box beam structure at Ditch “H”. The existing bridge was replaced with a 3-span, 250 ft. long, 122 ft. wide, flared structure to accommodate adjacent entrance and exit ramps. The profile was adjusted to satisfy the required freeboard. The replacement superstructure utilized TxDOT Type C prestressed beams; the substructure was composed of trestle pile bents placed to avoid the existing foundation elements. Project conformed to corridor aesthetic requirements. Lakewood Drive Roadway Improvements and Bridge Replacement: Hays County Department of Engineering - Hays County, Texas Shuey is the project manager for the preparation of construction plans, specifications and estimates for a new bridge (replacement) and approaches over Little Bear Creek, on Lakewood Drive in northern Hays County. The new bridge replaced an existing culvert/low water crossing, which was overtopped several times a year, becoming impassible. To provide the required freeboard the new bridge deck was located approximately eight feet higher than the existing roadway surface, which required extensive redesign of the roadway approaches. He was responsible for managing the work program and the team of sub-consultations as well as completion of the bridge layout; design and preparation of all bridge superstructure and substructure details; the design of the bridge foundations and construction contract administration. Project features included bridge roadway closings and associated traffic control, limited right of way, vertical and horizontal realignment to accommodate traffic safety and tie in with existing roadways. The replacement bridge was a 3-span, 240 ft. long, 26 ft wide structure with a superstructure utilizing TxDOT Type C prestressed beams; the substructure was composed of columns supported bents placed on concentric drilled shafts. Project conformed to corridor aesthetic requirements. FM 1626 Phased Bridge Replacement at Little Bear Creek -TxDOT Austin District: Shuey is the project manager in charge of providing design services for the preparation of construction plans, specifications and estimates for the construction of a new bridge (replacement) over Little Bear Creek on FM 1626. The new bridge replaces an existing culvert crossing as part of the realignment and widening of FM 1626. The procedures also verify that the final documents will contain no incongruities and conform to the client’s requirements, sound engineering practices, and professional standards. All project tasks will be progressed in accordance with a unified quality assurance program to ensure that the client receives the best quality services possible.

Design Reviews: Scheduled reviews of the design work products will be conducted. All reports and technical correspondence are reviewed prior to submittal.

Document Review Policy: Every document that leaves the office must be independently reviewed before submittal.

Subcontractor Adherence to QC Processes: Every sub-consultant must adhere to the project’s quality control procedures. The Quality Manager will conduct audits of the entire team to verify compliance.

Instructions for Completing DCAM-CAP Form 255

DCAM-CAP Form 255 is the companion form to CAP Form 254 which permits Architects, Landscape Architects, Engineers and Land Surveyors to respond to invitations to be considered

for design projects from the State of Oklahoma. It permits consultants to tailor their response to the specific project being considered by an agency. This form is used in conjunction with DCAM-CAP Form 254 in the same manner as Federal Standard Forms 254 and 255 are used for Federal selections. DCAM-CAP Form 255 is to be used for a specific project and DCAM-CAP Form 254 is used to be registered for consideration. These forms have been designed to be as similar as possible to the Federal forms but SF254 and SF255 MAY NOT be used for State registration and selection.

This form is divided into (6) break sections. To easily view where these sections occur, use the “View/Normal” command. The first section contains a table, is unprotected, and allows for the first sheet/instructions to be deleted. The table can be deleted by selecting the whole table and using the “Edit/Cut” command.

The second section (Items 1 thru 5a) are protected and contains fields that can be filled in with the appropriate information. The third and fourth sections (Item 6.) are unprotected and

don’t contain any fields. This allows for the whole table to be copied and pasted so that additional “Brief resumes…” can be input. The table can be copied by selecting the whole table and using the “Edit/Copy” And “Edit/Paste” commands. The fifth and sixth sections (Items 7, 8, 9, 10) are protected and contain fields that can be filled in.

To better assist you in inputting information, you can turn on “Form Field Shading” and “Show Gridlines” using the forms toolbar.

Mr. Homan is responsible for administration and technical operations in the Tulsa office of Terracon Consultants, Inc. He oversees all geotechnical, construction services, and environmental operations, prepares and reviews reports, and is responsible for general administration of the office. Major projects that he has been involved in include pavement design for the H.E. Bailey and Creek Nation Turnpike expansions. He has also lead geotechnical studies for bridge foundation design for well over 100 bridges using LRFD design and Texas Cone Penetrometer design. Mr. Homan has lead pedological surveys collecting data to be used by ODOT in pavement design analysis. He has conducted geotechnical studies for cast in place and MSE retaining walls, as well as soil nail walls for several ODOT projects. He lead efforts for a geophysical survey of US 412 in Major and Woodward counties that utilized electrical resistivity and multichannel analysis of surface wave seismic geophysical surveys to investigate the impact of the karst formation within the highway right-of-way. Mr. Homan has been project manager and Principal in Charge over the past 20 years for various ODOT demand services contracts including: geotechnical services, prestressed concrete bridge beam inspection services, structural steel bridge member inspection services, NEPA environmental services and Phase I Environmental services. Prior to joining Terracon in 2000, Mr. Homan was Principal Engineer and Office Manager for another consulting firm located in Tulsa. She has directed both technical and project management aspects environmental projects ranging in size and complexity from multimedia, multi-contaminant Superfund sites to post-accident fuel spill clean-ups within highway right-of-ways. Management of these projects involved both on-site daily management of manpower and methodologies as well as remote management of teams in other states. Ms. McDonnell has experience in investigating and remediating impacts to all types of environmental media (soil, sediment, groundwater, surface water, air) from the full range of typical environmental contaminants (solvents, petroleum hydrocarbons, explosives, toxic metals, non-metallic inorganics, pesticides, PCBs, etc.), utilizing a variety of remedial technologies to achieve site closure (groundwater extraction/treatment, soil excavation/disposal, soil vapor extraction, in-situ bioremediation, in-situ neutralization, zero-valent metal, nano-scale zero-valent metals, and natural attenuation/risk-based demonstrations). He has over 25 years of experience in the environmental field and six years of experience in oil and gas exploration. His environmental work experience encompasses site characterization, environmental assessments, remedial design and oversight, regulatory compliance, and quality assurance. Phase I and Phase II Environmental Site Assessments - Recent experience (last 17 years) was dedicated to conducting Phase I and Phase II Environmental Site Assessments at industrial-use facilities in the oil-field service industry. These projects were located anywhere in the United States and Alberta, Canada where oil and natural gas exploration was conducted. The Phase I ESAs incorporated limited soil sampling activities at the time the site visits were conducted. The purpose of the Phase I services was to develop a baseline environmental setting during the sale or lease of the property when the client entered or left a property. Site Remediation - Conducted site management and site closure sampling at industrial and manufacturing facilities throughout the United States. Remedial activities included soil remediation by exavation at various U.S. locations and monitored natural attenuation groundwater sampling and documentation of petroleum product and chlorinated hydrocarbon plumes in Texas, Louisiana, and New Mexico. Mr. Lampi’s most recent site remediation project was in 2013 and was conducted at a former pipeyard distribution yard in Tampa, Florida. The historic fill material beneath the site was impacted by Polynuclear Aromatic Hydrocarbons (PAHs) and arsenic. The remedial action included shallow groundwater monitoring and surface soil removal within the warehouse structure and around the building perimeter.
