STANDARD OPERATING ROCEDURE FOR OBTAINING DEGREE CERTIFICATES INRESPECT OF POSTG ADUATES AND SUPERSPECIALITY COURSES FROM
MUHS ASHIK FOR AND ISSUE TO OFFICERS
Introduction
'1. AFMC Pune conduct 25 MD/MS/MDS and 04 super speciality courses for approx 150
r=)Gand 10 Superspeciality students under the aegis of MUHS, Nashik. PG examination
(Theory & practical) are h ld in the month of May/Jun and super speciality examination
(Theory & practical) are hel in the month of JlIl/AlIg every year .. Results are announced on
MUHS website muhs.ac.in fter one month of examination. Degree certificates are issued
after the convocation held b MUHS Nashik.
2. On completion of t e advance specialist course, course ending examination and
University examinations, the service PG residents are posted to different units as per posting
orders received from respective Service HQs.

3. Aim of this SOP'jiS to lay down the procedures for applying for degree certificates from
MUHS Nashik on sucpessful completion of advance specialist / super splty courses and
prompt disposal/issue /despatch of the same to the concerned officers.
Procedures
4. All students who have geen declared passed by MUHS Nashik will download their
provisional mark sheets from MUHS N,ashik website muhs.ac.in alongwith applications for
convocation of respective degree. They will submit application forms duly completed
alongwith requisite documents Fither through their respective depts or direct to Brig Ie Trg,
HO Trg Team, AFMC for obtaiiing degree from MUHS Nashik. While ~pPIYing :or the degree
certificates, t~e students will intimate HO Trg .~eamAFMC ~une 0: th~lr new unit address and
contact details for despatch 0l~degree certificates to their destination once the same are
received from MUHS, Nashik.

5. If the students is appl ing for the degree cert through respective dept, the dept will fwd
the application to Trg Team office under a covering letter alongwith complete postal address
of the concerned offr so th t prompt despatch of the degree certificate can be ensur~d on
receipt from MUHS, Nashik.
6. On receipt of the a plications for degree certificates from the students, Trg Team
office, AFMC Pune will :-
(a) Check the app ication forms and enclosed documents for correctness. Enter the
details in a register aintained for the purpose. The register will have the following
headings :-
(i) Particul rs of offr
(ii) Camp Ie e postal address of unit with contact details
(iii) Speciali y / super speciality
(iv) Batch / ear
(v) date of receipt of application for degree cert from offr
(vi) date of despatch of application for degree cert to MUHS.
(vii) Date of receipt of degree certfram MUHS, Nashik
(viii) Date of despatch of degree cert to offr's unit under regd SOS.
(ix) Date of receipt of receipt / acknowledgement from offr / unit regardingreceipt of degree cert.
(b) Application forms will be fwd to MUHS Nashik under intimation to applicants
address. MUHS Nashik will process the application forms and fwd the degree certs to
AFMC, Pune. On receipt of the degree certs from MU,HS Nashik, the same will be
entered in the register maintained for the purpose and will be despatched by regd post!
SDS to offr's unit immediately and receipt / acknowledgemehlfrom the offr / unit will be
obtained. A receipt / acknowledgement form will also be enclosed alongwith the letter
while forwarding the degree certificate to the offr's unit. The receipt of
acknowledqernent from the unit of the offr / receipt from the offr will be closely
monitored by Trg Team office so as to ensure prompt receipt of the same by offr's unit /I
by the offr and to ensure no cases of loss of the degree cert in transit / misplace at the
destination. Thelreceipt of acknowledgemenifrom the unit / receipt from the offr will be
entered in the register. Reminder will be issued to units if ack is not received within 20.
days of despatch ,of the degree certificates.

(c) Generally the degree certs are fwd. by MUHS Nashik to AFMC Pune only. In
case, If the candidate receives the degree directly from MUHS Nashik, he/she must
intimate to HQ Trg Team, AFMC Pune.
(d) In case any o~r deputes his representative to collect the degree cert on his I
her behalf, the sa e will be given to the representative on production of proper
authority letter issue by the concerned offr and on production of proper identity proof'
by the deputed repr sentative. In such cases degree certificate will be issued through
a covering letter to t e rep and his signature willbe obtained in the covering letter and
register maintained or this purpose. A receipt from the concerned offr will also be
obtained for in such lases.
6. Degree certificates of civilian students. The same procedure as above for service
students will be followed for despatch of degree certificates in respect of civilian students. For
civilian students the degree certiticates will be despatched through registered post / ack due.
7. In any case no degree certificates will be kept with Trg Team office after receipt of the
same from MUHS, Nashik. The same will be disposed off within15 days of receipt from the
MUHS, Nashik. In case there is any difficulty in finding out the present address of the
student, necessary assistance from HOD of the concerned dept will be sought. In such cases
the HOD of the concerned dept will be responsible to provide correct / present postal address
of the offr and despatch of the degree certificate to the concerned student.
.Amendments
8. Amendments if any, to this SOP will be carried out as and when felt necessary by the
competent authority.
Effective Date
9. This SOP will come into force with immediate effet and will strictly be complied with by
all concerned.
Dissemination
10. A copy of this SOP may be uploaded in AFMC website and AFMC LAN for info of the
students. A copy will also be displayed in notice boards of all pts .
