THE CLIENT

St. Joseph Health

St. Joseph Health (SJH) is an integrated Catholic health care delivery system sponsored by the St. Joseph Health Ministry. Based in Irvine, California, St. Joseph Health serves 10 distinct communities within three general regions – Northern California, Southern California and West Texas/Eastern New Mexico. A three-time recipient of the Gallup Great Workplace Award, SJH is comprised of over 24,000 dedicated employees. St. Joseph Health provides a full range of care from facilities including 14 acute care hospitals, home health agencies, hospice care, outpatient services, skilled nursing facilities, community clinics, and physician organizations. They employ over 24,000 professionals and have over 4,000 total licensed beds.

St. Joseph Health is ministry of The Sisters of St. Joseph of Orange, who trace their roots back to 17th Century France and the unique vision of a Jesuit priest named Jean-Pierre Medaille. He sought to organize an order of religious women who, rather than remaining safely cloistered in a convent, would venture out into the community, seek out "the dear neighbors" and minister to their needs. The congregation managed to survive the turbulence of the French Revolution and eventually expanded, not only throughout France, but throughout the world.

In 1912, a small contingent of the Sisters of St. Joseph went to Eureka, California, at the invitation of the local bishop, to establish a school. A few years later, the great influenza epidemic of 1918 caused the sisters to temporarily abandon their education efforts to care for the sick in their homes. They realized immediately that the small community desperately needed a hospital. In 1920, the sisters opened 28-bed St. Joseph Hospital of Eureka, and their new health care ministry was born.

St. Joseph Health Oncology Program

Saint Joseph Hospital in Eureka, California was recognized by the Commission on Cancer of the American College of Surgeons as offering the very best in cancer care and treatment. St. Joseph & Redwood Memorial Hospitals and St. Joseph Home Care, together with their community partners, form a comprehensive team offering care, support and information for cancer patients and their families. St. Joseph Hospital Eureka has received a three-year approval with commendation by the American College of Surgeons (ACS), so patients can rest assured that they are receiving the highest quality care. The cancer program saw approximately 800 analytic cases last year.

Radiation Oncology & The Latest Technology for Diagnosis & Treatment

The Radiation Oncology Department of St. Joseph Hospital has been serving the community since 1979. Together, a team of board-certified radiation oncologists, certified oncology nurse clinicians, and support staff provide an atmosphere that is highly conducive to the healing process, which includes healing of the whole person--body, mind and spirit. In treating patients and their families with compassion and dignity, their team focuses on alleviating fear and anxiety through calm understanding and reassurance from initial diagnosis through recovery.

Through their expert care, the Radiation Oncology Department applies external beam radiation of varying degrees to the patient’s area of concern, which could include cancerous areas of the prostate, breast, lung, colorectal, and head and neck. They are recognized as a leader in providing therapeutic services to all patients served and in bringing new technology and treatments to local cancer patients.

The New 64-Slice CT Scanner

St. Joseph Hospital Diagnostic Imaging maintains two CT Scanners at their main facility - delivering head-to-toe 3D imaging in less than 60 seconds. One of the scanners is a new 64 slice, Toshiba unit. St. Joseph’s CT services are fully ACR accredited. In addition, the Radiation Oncology department recently installed a new Toshiba large bore 16 slice CT used exclusively for CT simulation. All the CT systems are designed to better assist physicians in early diagnosis and treatment by offering a clearer anatomical visualization and improved soft tissue evaluation. This means a more comprehensive diagnosis in less time - and with greater patient comfort.

Page 9: St Joseph Health Chemo Infusion Nurse Manager-Job Profile

The Power of PET

Positron Emission Tomography (PET) is a non-invasive nuclear medicine scanning procedure which uses positron emitting radioactive isotopes to show three-dimensional functional images, reflecting an organ’s metabolism rather than just its anatomy, as in conventional scanning techniques. PET is used to diagnose and monitor cancer, in addition to diseases of the heart, brain, and lungs. PET allows for the diagnosis of cancer and other diseases in their early stages, yielding better patient outcomes.

PET/CT - the all-in-one full body scan

The newest diagnostic system for detecting cancer at St. Joseph Hospital is called a PET/CT Scan -- the only one of its kind in Humboldt County. It is a combination of Positron Emission Tomography (PET), which detects cellular activity, and Computerized Tomography (CT), which reveals a cross-section of body tissues and organs. In the past, difficulties arose from trying to interpret the results of a CT scan done separately from a PET scan, due to the fact that the patient's body position had changed. By combining these technologies into a single device, the PET/CT Scanner makes it possible to collect both anatomical and biological or metabolic information at the same time - during a single examination. This provides physicians a much more complete and clearer picture of what is happening in the body. This integrated information permits accurate tumor detection and localization for a variety of cancers, including breast, esophageal, cervical, melanoma, lymphoma, lung, colorectal, head and neck, and ovarian cancer. With the help of a PET/CT scan, physicians can now detect cancers, as well as some heart disease and brain disorders, at a much earlier stage.

Magnetic Resonance Imaging (MRI)

Our Vantage MRI by Toshiba combines a compact magnet with noise-reduction technology and ergonomic design to produce superior image quality with greater patient comfort, allowing the patient's head to remain outside the tube for most procedures. A new MRI technique called Parallel Imaging, along with Toshiba's patented speeder coil technology, allows the Vantage to capture images in a fraction of the time required by older machines. The faster scans mean less chance of blurring from patient movement, less repositioning during the procedure and less time in the exam room. The Vantage also packs a full 1.5-Tesla field strength -- roughly double the magnetic strength of most 'open' MRI machines -- providing the top quality images needed for the most accurate diagnosis possible. Because of its ability to image soft tissue, MRI plays a critical role in diagnosing cancer. It can reveal the shape, size and location of tumors, and it is used to monitor tumor growth, including malignant and non-malignant tumors in the brain. MRI is even used to augment mammography tests for breast cancer.

Intensity Modulated Radiation Therapy (IMRT)

This newer type of radiation therapy means higher cure rates and fewer side effects through more exact and focused treatment. "Very simply, IMRT is a major breakthrough in the practice of Radiation Oncology," said Dr. Michael Harmon, Oncology physician at the St. Joseph Hospital Cancer Center. "It allows for pinpoint accuracy in targeting tumors, while at the same time greatly reducing the radiation dose to healthy vital tissues." The new IMRT technology has already contributed to substantially improved clinical outcomes in prostate cancer, according to studies in the Journal of Radiation, Oncology, Biology and Physics.

Page 10: St Joseph Health Chemo Infusion Nurse Manager-Job Profile

High Dose Rate Brachytherapy (HDR)

High dose rate brachytherapy is a form of radiotherapy commonly used to treat gynecologic cancers. For selected cases of breast cancer, the MammoSite catheter is used for treatment and therapy can be completed in just one week.

Prostate Seed Implant Program

Through the development of the Prostate Seed Implant Program, a new option in prostate cancer treatment is now available and is considered an alternative to radical surgery or conventional radiation treatments.

This technique essentially treats the prostate from the inside out rather than from the outside in. Decreased radiation doses to the surrounding normal tissues may decrease both the short term and long term side effects.

Medical Oncology

In February 2015, St. Joseph Health-Humboldt County opened the Peals Family Chemotherapy Infusion Center (CIC). This is a 14 chair dedicated chemotherapy suite. It is conveniently located next door to the HMS Oncology suite so staff and patients have immediate access to the Medical Oncology team of physicians. There are currently 3 full-time medical oncologists on staff and plans for a 4th physician by the end of the year.

The CIC is staffed with chemotherapy certified nurses and specific ancillary support staff. Services offered include chemotherapy and biotherapy infusion, IV hydration, antibiotic therapy, IVIG, blood product transfusions, electrolyte replacement, injections, parenteral/intraperitoneal chemotherapy administration and intravenous access/central line management.

Current volumes for outpatient IV therapy are approximately 350 visits per month.

STAR Program - Survivorship, Training, and Rehab

St. Joseph Health – Humboldt County’s STAR Program serves all survivors in need, whether they are beginning treatment, finished with treatment, or experiencing late effects or unresolved issues from long-ago treatment. The program is open to everyone no matter the prognosis, cancer stage or phase of recovery & was created to help survivors physically and emotionally heal from the side-effects of treatments. St Joseph Health Humboldt County is in the process of obtaining The STAR Program Certification. This is given by Massachusetts-based Oncology Rehab Partners (ORP), whose unparalleled development, training and support products in oncology rehabilitation give healthcare facilities the capacity to offer quality survivorship care to their patients.

Palliative Care Program

At St. Joseph Health – Humboldt County, the palliative care services are specifically customized to provide patients and their families with relief. In conjunction with pain treatment services, palliative care works to address every aspect of a patient’s recovery process devoted to helping maintain their quality of life. Devoted to the hospital's primary mission, palliative care services begin upon diagnosis and continue even after the bereavement process. "No One Dies Alone" program is a primary example of the philosophy

Page 11: St Joseph Health Chemo Infusion Nurse Manager-Job Profile

of care at work, striving to provide reassuring treatment to dying patients who would otherwise be alone in their final moments.

Collaboration with Stanford University

St. Joseph Hospital is pleased to announce a new and exciting collaboration with Stanford Medicine. The relationship will involve a series of projects related to cancer care, clinical research and education – all with the goal of continuing to provide local patients with the highest quality cancer care possible.

The collaboration includes: a new cancer telemedicine tumor board program; access to cancer clinical trials; and access to educational programs at Stanford for St. Joseph Hospital physicians and staff. The tumor board program is a formal way for a multi-disciplinary group of physicians to discuss treatment options for complex or rare cancers. This new relationship allows us to use telemedicine technology to present local cancer cases to physicians at Stanford via WebEx. The program will launch with gynecologic oncology cases.

Additionally, Stanford will provide St. Joseph Hospital access to cancer clinical trials and will assist in the training of clinical research staff. Clinical trials are protocols for cancers that do not respond to routine treatments. The clinical trials will offer patients at St. Joseph Hospital access to state-of-the-art treatment that currently requires travel to the San Francisco area for enrollment.

THE LOCATION

Eureka, CA

Eureka is the principal city and county seat of Humboldt County in the Redwood Empire region of California. The city is located on U.S. Route 101 on the shores of Humboldt Bay, 270 miles north of San Francisco and 100 miles south of the Oregon border. Eureka is the largest coastal city between San Francisco and Portland, and the westernmost city of more than 25,000 residents in the 48 contiguous states. It is the regional center for government, health care, trade, and the arts on the North Coast north of the San Francisco Bay Area. Greater Eureka, one of California's major commercial fishing ports, is the location of the largest deep water port between San Francisco and Coos Bay, a stretch of about 500 miles.

Page 12: St Joseph Health Chemo Infusion Nurse Manager-Job Profile

The headquarters of both the Six Rivers National Forest and the North Coast Redwoods District of the California State Parks System are in Eureka. As entrepôt for hundreds of lumber mills that once existed in the area, the city played a leading role in the historic West Coast lumber trade.

The entire city is a state historic landmark, which has hundreds of significant Victorian homes, including the nationally-recognized Carson Mansion, and the city has retained its original 19th century commercial core as a nationally-recognized Old Town Historic District. Eureka is home to California's oldest zoo, the Sequoia Park Zoo.

The North Coast's primary shopping facility, the Bayshore Mall, is the largest north of the San Francisco Bay Area on the California coast. The mall features over 70 stores, which is anchored by Kohl's, Sears, and Walmart. TJ Maxx opened August 25, 2013 and Sports Authority opened November 16, 2013. Eureka is the site of the only Costco in the region.

Other major shopping areas and centers include Henderson Center, the Eureka Mall, Burre Center, and Downtown and Old Town Eureka. Major stores not in centers include Target and Kmart.

Eureka is one of California's historic landmarks. The California State Historical marker, #477, designating Eureka, is located in Old Town, one of the nation's best preserved original Victorian era commercial districts. The city was voted as the No. 1 best small art town in John Villani's book "The 100 Best Small Art Towns in America." Eureka hosts the region's largest monthly cultural and arts event, "Arts' Alive!" on the first Saturday of each month. More than 80 Eureka businesses and local galleries open their doors to the public. Often local cuisine and beverages accompany live performances by acclaimed regional bands and other types of performance art. The downtown Eureka area is also decorated with many murals.

Theater offerings include year round productions from several various theater groups including the North Coast Repertory Theater and the Eureka Theater. Various events occur throughout the year at the Redwood Acres Fairgrounds. Art organizations include the Humboldt State University First Street Gallery, Humboldt Arts Council and the Morris Graves Museum of Art, Redwood Art Association, The Ink People and the Eureka Art and Culture Commission. As a regional center, the city offers lodging, restaurants and shopping areas, including dozens of specialty shops in its historic 19th Century Old Town commercial district and the only large mall in the region.

Museums include the Clarke Historical Museum, the Humboldt Bay Maritime Museum, the Morris Graves Museum of Art, HSU First Street Gallery, Discovery Museum for Children, the Fort Humboldt State Historic Park and the Blue Ox Millworks and Historic Park.

Due to northern isolation and unfavorable economic conditions in the latter part of the twentieth century, much of the post-war redevelopment and urban renewal that other cities experienced did not occur in Eureka. As a result, Eureka is resplendent with hundreds of examples of 19th- and early 20th-century architecture and historic districts. David Gebhard, Professor of architectural history at University of California, Santa Barbara, wrote that Eureka has the potential of becoming the West Coast Williamsburg. He stated Williamsburg, Virginia preserves an authentic colonial environment; Eureka preserves intact Victorian and early twentieth-century architecture. The extensive array of intact Victorian era and later homes and public buildings include many ornate examples of Colonial Revival, Eastlake, Greek Revival, Italianate, Queen Anne, and Stick styles of Victorian architecture. All of these styles are present in the most famous and possibly most ornate of Victorian homes, the Carson Mansion.

Page 13: St Joseph Health Chemo Infusion Nurse Manager-Job Profile

Approximately 16% of the city's structures are cataloged as important historical structures, with many of those attaining the status of state and national significance in terms of a particular structure's importance in relationship to the body of surviving examples of the architectural style attributed to its construction and related detail. 13 distinct districts have been identified which meet the criteria for the National Register of Historic Places.

Among them are the 2nd Street District (10 buildings), 15th Street district (13 buildings) and the O Street district (43 buildings). Hillsdale Street, a popular and well-preserved district, contains 17 buildings of historic interest. In all, some 1,500 buildings have been recognized as qualifying for the National Register. The Eureka Heritage Society, a local architectural preservation group founded in 1973, has been instrumental in protecting and preserving many of Eureka’s fine Victorians.

Sequoia Park Zoo, situated on more than 67 acres of mature second-growth Redwood forest, includes Eureka's largest public playground and a duck pond in addition to meticulously kept gardens and examples of the area's many varieties of rhododendron bushes.

The City of Eureka Recreation Department manages 13 playgrounds, including Cooper Gulch, which is 33 acres, and many ball fields as well as tennis courts and others, including basketball and soccer. Other parks in or near Eureka include the Humboldt Botanical Garden and the Humboldt Bay National Wildlife Refuge, and the Eureka Marsh, an accessible protected marsh between the Bayshore Mall and Humboldt Bay. There is a modern boardwalk along the city's waterfront and two large marinas capable of mooring over 400 small craft.

Trinidad, CA

Page 14: St Joseph Health Chemo Infusion Nurse Manager-Job Profile

The Trinidad area is one of the most spectacular and pristine segments of the California coast, and has been established as a California Coastal National Monument Gateway - an area that offers the best shore-based opportunities to discover and view offshore rocks and islands and their inhabitants. The coastal Trinidad rocks support one of the most diverse seabird colonies in California with twelve species and over 100,000 breeding individuals. The two most important rocks for seabirds are Green Rock and Flatiron Rock that host seven species of birds and 66,000 breeding individuals. These two rocks alone account for 25% of the most important seabird colonies along the California coast.

Trinidad is a seaside town in Humboldt County, located on the Pacific Ocean 8 miles north of the Arcata-Eureka Airport and 15 miles north of the college town of Arcata. Situated directly above its own natural harbor, Trinidad is one of California's smallest incorporated cities, and is noted for its spectacular coastline with ten public beaches and offshore rocks. These rocks are part of the California Coastal National Monument of which Trinidad is a Gateway City. This natural landmark is joined by three additional marine landmarks - the historic Trinidad Head Light, the Trinidad Memorial Lighthouse, and Humboldt State University's Fred Telonicher Marine Laboratory.

Fishing operations related to Trinidad Harbor are vital to both local tourism and commercial fishery interests in the region. In addition to its coastal assets, the city honors and works to protect its Native American cultural and historic heritage.
