Risk AssessmentRisk Assessment
1. Create a team. Select among the team a leader, recorder and presentor.
2. Using the assessment form list down all the possible unsafe act and unsafe condition in the workplace.
3. Evaluate each situation using the given evaluation standard. accident possibility degree of injury accident possibility degree of injury Great 10 points Death 20 points Average 5 points Severe injury 10 points Bare 2 points Slight injury 5 points
SSafety PProgram OObservation TTechnique: activity
3. Determine the risk level by adding the points of each situation.
Risk Level III – Great / Not Tolerable = 20 or more points
Risk Level II - Average / Currently Not Tolerable = 11 – 19 points
Risk Level I – Slight / Tolerable = 10 points or less
4. Establish major action plans to address identified hazards and re-evaluate risk level.
5. Implement action plans and document results.
6. Report results to the management.
