Outstanding Issues: **Milestones to develop Hospital Discharge Protocol for Runwell and others, Review CLG Pilot and share information about Public Targets. All due by October 2007 and have been delayed because we have no Project Officer. Recommend that we move target date to December 2007.

Housing and Supporting People (10 Milestones)• Accommodation based services exceed the Decent Homes Standard • Floating support services focused on providing support to socially excluded people, particularly to those living in temporary accommodation• Socially excluded people across tenures have access to appropriate support.

Outstanding issues: **This milestone is around getting Cambridge to do a mystery shop for Southend. Recommend that we move target date to November 2007.

Joint Working and Outcome Measures (9 Milestones)• Funding arrangements are in place for every service that reflect the needs of the service users determined by their needs rather than eligibility requirements.• Utilise information that demonstrates service effectiveness, in line with CLG and local requirements (especially LAA targets).

People with Mental Health Problems and Mentally Disordered Offenders (9 Milestones)

• Better understanding of the needs of people with mental health problems• Flexible floating support service provision providing community support services and assisting the rehabilitation process.

Outstanding Issues: ** All milestone are around access issues which will be resolved through proposals of new Access Gateway

People with Learning Disabilities (9 Milestones)• The programme will fund more self-contained provision. Outreach / resettlement services expanded to meet the increased demand for such services combined with greater access to mainstream housing. Service users offered more choices in their housing and more community based housing

Outstanding Issues: ** These milestones are around assessing current service user needs and reviewing current contracting arrangements towards an hourly based contract value. Needs Assessment will be delayed until Dec 07 due to lack of response from providers and Contract reviews will be delayed until March 2008 in line with rationalisation of LD contracts.

Older People, Frail Elders, Older People with Mental Health or Dementia (6 Milestones)

•The SP programme will fund a range of flexible support options and linked to domiciliary, extra care and alarm / assistive technology provision where appropriate. Service users will have a range of support options in addition to sheltered housing.

Outstanding Issues: **Delayed due to Sheltered Housing Review, which will cover most of these milestones. All milestones to be moved forward to Dec 07.

Homeless Families, Single Homeless & Rough Sleepers (16 Milestones)

• Currently homeless and potentially homeless families will have their support needs met.• Floating support will be available to potentially homeless families to prevent repeat homelessness

Offenders / People at Risk of Offending and People with substance misuse problems (21 Milestones)

• Appropriate floating support service that will meet the needs of this client group• Appropriate accommodation based services for ex offenders / youth offenders and substance misuse.

• Integrated approach to the development of a range of support services for young people which meet the aims and objectives of the children’s service.• SP will fund appropriate services for teenage parents with support needs
