Ingredients of IPC

• 1 – People who might benefit are targeted directly

• 2 – It starts with the different kind of conversation

• 3 – It’s about what matters to me, not what’s the matter with me

• 4 – Everyone works together, its integrated

• 5 – Its co-ordinated

• 6 – Personal health or integrated budgets are offered

• 7 – People are helped to link to their community

What is the South West IPC programme?

• We are one of nine demonstrator sites nationally • Bringing together local government, VCSE sector

and the NHS to work differently to support people with complex care needs

• It is about a power shift from services to the people they are there to serve

• IPC aims to use person led approaches, with the option of a personal budget to integrate support for people

• People will have one care plan focused on what matters to the person.

How are we making it happen? • Starting with small scale

implementation sites across the region, testing and learning in different settings, working with people with different needs

• Developing a systems change programme, taking the learning from sites and making the changes needed to scale up

Progress in local implementation sites • 51 potential sites

• Over 30 started up

• Number of personal health budgets up from <400 in 2014/15 to >1000 in 2015/16
