We are switching from the Hilton Garden Inn in Marlborough because they changed their booking procedures

requiring a much larger down payment on rooms far in advance and their ranking dropped by 3 places over the past

year with their switch in management. Hampton Inn is in the top three in the area and will accommodate all of our

needs, as well as they have been highly recommended by Suburban Tours.

NURSE / HEALTH FORMS / MEDICATION:

A registered nurse will be attending the trip and along with myself, will monitor all of the attendees and care to their

medical needs. All participants (students, staff, parents, and family members) are required to fill out the 2019-2020 SBRHS

Marching Band Health Form (included in this packet for your review) so we are aware of any medical needs. These will

be copied and given to the SBRHS school nurse as well. Additionally, all students and children that are participating in the

trip that require daily medication will be required to submit a SBRHS Music Student Medication Identification Sheet

(included in this packet for your review). This sheet informs us of what medication is to be taken by the student/child,

how it is to be taken, and when it is to be taken. Any medications must be given to the trip nurse during the Wednesday

Evening check-in and he/she will carry them on her person unless otherwise instructed by a physician. Students/children

will be required to submit medications (1) in an original pharmaceutical container and/or original packaging (2) place

container / packaging in a zip lock bag and (3) place the completed SBRHS Music Student Medication Identification Sheet

in the bag with the student’s/child’s name and necessary directions / instructions, (4) be sure to only include the amount

of medication needed to be administered during the trip.

TRANSPORTATION / TRAVEL AGENCY:

Transportation will be provided by Bloom’s Bus Lines, Inc. from Taunton, MA and will be hired through our travel agency

Suburban Tours out of North Providence, RI. I have worked with Suburban Tours for the past 21 years and have scheduled

more than a dozen band/music department trips with them. I have never had a major problem with them and upon minor

situations occurring such as a flat tire, or last minute changes to plans, they have always responded immediately and made

the necessary arrangements to keep our group’s safe and on schedule. Bloom’s Bus Lines, Inc. requires a background

check for all its drivers via a national database upon hire since a CORI check is only for the state of Massachusetts.

Additionally, their drivers also have a Massachusetts school bus certificate for which the state of Massachusetts runs an

annual CORI check on them to maintain the license. In regards to their safety record, they have provided me a copy of the

Safety Record Report from the Federal Motor Carriers Safety Association (included in this packet for your review).

CHAPERONES:

All Chaperones will be CORI checked and will be asked to adhere to all rules and guidelines stated in the 2019 SBRHS

Marching Band Allentown Trip Chaperone Responsibilities document (included in this packet for your review). I will meet

with all chaperones prior to the trip to review said responsibilities in detail.

RULES AND REGULATIONS:

All students are subject to school rules and regulations while on this field trip which will be sponsored by Somerset Berkley

Regional High School. Any student who violates school rules and regulations will be subject to disciplinary action as

prescribed in the Student/Parent Handbook. The Director of the trip will follow all procedures as outlined by the Somerset

Berkley Regional School Committee and Somerset Berkley Regional High School Administration in regards to the rules and

regulations of the SBRHS Student/Parent Handbook. Additionally, students and parents are required to sign the USBands

National Marching Band Championships Allentown Marching Band Trip Contract which is part of the 2019 SBRHS

Marching Band Allentown Trip Contract, Payment Schedule-Payment Slips, and Intent Form document (included in this

packet for your review) prior to participating in the trip.

MARCHING BAND CHAPERONES’ RESPONSIBILITIES

USBands National Marching Band Championships / NESBA Marching Band Finals

ALLENTOWN / LAWRENCE MARCHING BAND TRIP 2019

Thank you for volunteering to chaperone our trip to Allentown, PA / Lawrence, MA! You are providing a valuable service to our school community. Your supervision and participation are critical to the health and safety of our students. Listed

below are some of your primary responsibilities as a trip chaperone.

1.) Checking for attendance on the bus - please carry attendance / rooming lists with you at all times. 2.) Helping with the director / instructors - room checks at night and upon check-in (please ensure that the room

is in the correct condition before students occupy the rooms), getting students to settle down for lights out, supervising after lights out to make sure that no one leaves their room, collecting keys at check-out, checking to make sure that students are up in the morning, reminding kids what they need for the next day, supervising at the pool, etc. When we are at the hotel, chaperones will need to be in different areas to supervise the rooms, etc. which we can all coordinate together.

3.) Mr. Marshall will be handling all medical issues on the trip and will hold onto the medical forms and all student

medications. Please check with me before the trip to see if any of the students you are chaperoning have any medical concerns and create a BAND app Group Chat with them. Also, if any of your students need medical attention, contact Mr. Marshall (401) 952-5574 immediately!

4.) Please collect cell phone numbers from students that you are chaperoning and/or create a BAND app Group

Chat with them so that you have a line of communication with them in case you are separated from them at any point during the trip and be sure to give them yours as well.

In General - Understand that when dealing with a large group, I need to make decisions for the good of the group. You may

not always agree with my decisions, but please try to understand, enforce and abide by them.

- When dealing with the students, be firm but fair - don’t overreact!!!

- If any major problems arise, they should be brought to the attention of one the directors/instructors. Medical Please fill-out the enclosed medical form for yourselves and hand it in to me as soon as you can. You do not need to fill out everything; we just want to make sure that if you have any conditions or allergies that we are aware of them just in case. Thank you for understanding! Staff Rooms

I agree to abide by these rules and all other policies which have been adopted or may be adopted in the future by the Music department regarding this event. I understand that the policies of the Somerset Berkley Regional High School Music Department presented earlier in the year (and approved by the Somerset Berkley Regional School Committee) will be adhered to. I also promise that I will: 1. Learn all music/drill/routines to the best of my ability. 2. Attend all rehearsals and performances on time. An Excused Absence Form must accompany all absences and be

presented to Mr. Marshall 2 weeks ahead of time. Any failure to attend a rehearsal or continued absences without good reason may constitute grounds for exclusion.

3. Cooperate fully with chaperones and all other administrative officials. 4. Neither use nor have in my possession at any time alcoholic beverages, tobacco products or illegal drugs. 5. Stay in designated housing facility throughout the duration of the Trip. 6. Not request a room change. 7. Not be in an unassigned room without the permission of a chaperone. 8. Not use the Hotel Room telephones except in case of an emergency. 9. Not participate in pranks or vandalism of any kind. If I damage any property, my parents and/or I will assume full financial

responsibility. (Damage to any Hotel Room may be assessed equally among those students assigned to the room). 10. Be in my assigned room at curfew time and remain in my assigned room until wake-up time (unless I am otherwise

granted permission by a Music Instructor / Official or assigned chaperone). 11. Conduct myself courteously and appropriately at all times. 12. It is understood that I signed this contract and that I plan to be part of this trip. The cost of the trip is based on 64

participants. Therefore, any change in this number will increase the price of the trip divided among the remaining students.

13. Abide by all decisions made by appropriate Music Instructors / Officials and obey all regulations listed above as well as any other regulations which may be implemented in the future by the group leaders.

I understand that membership in the marching band is a privilege and that membership will be forfeited if I fail to comply with any of the above rules. If it is necessary that I be sent home from Allentown/Lawrence, my parents and school officials will be notified, and my parents will be asked to provide immediate transportation home. ALLENTOWN / LAWRENCE TRIP 2019 - INFORMATION & REMINDERS – 10/31-11/3

1. Wednesday, October 30th – 2:30 pm – 4:00 pm – LUGGAGE DROP-OFF IN THE BAND ROOM. Bring your main piece of luggage only. You will receive a luggage tag to fill out for that piece of luggage and one for your instrument for those to

whom that applies. Bring carry-on WITH YOU ON THURSDAY and make sure that you have a label or tag on it identifying it as yours! Don’t go crazy bringing a lot of clothes – we are only away for 4 days! You are also responsible for making sure that you have your music, dot books, instrument, guard equipment, uniform undergarments, pair of black socks, Dinkles or Jazz Shoes, black gloves, guard uniform & make-up, extra tape for guard equipment, etc. If you forget your guard uniform, marching band undergarments (t-shirt, undergarments, socks & shoes), and or instrument/equipment, you will be unable to perform! BE SURE TO BRING CLOTHES FOR ALL WEATHER (HOT,

COLD & RAIN)! Also, there will be a pool at the hotel in Allentown and you may bring an appropriate bathing suit (Swimming time will be limited). Dress comfortably for the bus ride and remember that we will be rehearsing at Nazareth Area High School on Friday. Be prepared to practice in any weather!!!

2. WORK CREWS (a.k.a. Student leaders and others selected) – will be assigned to check in luggage/bring it to assigned

area/organize. All work crew students get to be in line first whenever we stop to eat. Work Crew Members will be required to help load the Trailer on Wednesday Afternoon from 2:30 pm - 3:45 pm as stated above!

3. Report time – Thursday, October 31st – 2:15 pm. We will be leaving at 2:30 pm sharp! PARENTS TRAVELING WITH US must park their cars to the east end of the teachers’ parking lot adjacent to County Street.

4. Bring some snacks for the various long duration bus rides (We will stop at a rest area for meals when necessary/appropriate). Parents have also purchased tons of snacks for the students as well!

5. Per the SBRHS Student Handbook, you are responsible for work due and assigned on the day of a field trip. Be sure to speak to your teachers ahead of time to know what work will be due on the day that you will miss school for the trip, what assignments will be given, and how they want you to submit assignments that are due. Mr. Marshall can run paper assignments down to teacher mailboxes before we leave if electronic submission is not available for some assignments.

6. I should have all permission slips/health forms! If there are any changes to health forms that were submitted this summer (i.e. changes in medications, injuries, etc.) please notify me immediately at [email protected]. Parents will be asked to sign off on student medical forms at baggage check-in to ensure that medical form information is up-to-date if new forms are not submitted. Also, any medications must be given to Mr. Marshall during the Wednesday Evening check-in. Please be sure to submit medications (1) in an original pharmaceutical container and/or original packaging (2) place container / packaging in a zip lock bag and (3) place the completed SBRHS Music

Student Medication Identification Sheet in the bag with the student’s name and necessary directions / instructions, (4) be sure to only include the amount of medication needed to be administered during the trip.

7. Bring a small backpack/bag to carry things in at the show. Bring a sweatshirt, hat, phone & charger, headphones, sunglasses, etc. All items you bring with you are your own responsibility. Be careful with your personal

possessions!

8. Bring a copy of your itinerary with you on the trip, so you know what is going on and times you need to report. Please remember that there are a lot of us traveling together and it is important that we are not kept waiting for anyone!

9. You must bring money for five non-included meals while on the trip (Thursday Dinner on the road, Friday Lunch at Nazareth Area HS (café will open a 4th lunch just for us and their band), Saturday lunch at USBands Finals, Saturday Dinner on the road and Sunday lunch at NESBA Finals). There will also be plenty of opportunities for souvenir purchases at the competition (pictures, performance DVD’s, patches, apparel, etc.); therefore, I would recommend bringing a little extra money for those opportunities. Snacks on the bus and in your hotel room are permissible, but

please remember that the cleanliness of the bus and your hotel room is your responsibility, not the bus driver’s, the staffs’ or your parent/chaperones’!!!

10. Read through all of the rules and regulations before we go on the trip. You are representing Somerset and Berkley, and we expect you to be on your best behavior! On this trip, as in life, allow common sense to prevail

and make smart decisions!
