Introduction to module Welcome to SOHP 602, inter-professional working in health and social care. This module is designed to prepare you for your transition into practice with an emphasis on the impact of inter-professional working on patient outcomes.The aim of this module is :-To provide students with an opportunity to engage in Inter-professional learning to promote a deep understanding of the benefits of working together to enhance the quality of care and management of service users.At the end of the module the learner will be expected to be able to: 1. Critically evaluate the links between effective inter-professional practice and patient outcomes2. Critically evaluate the evidence base for effective inter-professional working, including the relationship with relevant governmental drivers and policies3. Critically reflect on the impact of team dynamics on effective inter-professional working4. Evaluate the relationship between professional expertise, skills mix and service provisionWhat this workbook is for?The workbook is designed to enable you to meet the above learning outcomes and get the most out of this module and as a resource for your future practice. It is designed to encourage reflective practice and working within inter-professional teams.What we expect You are expected to attend all of the timetabled sessions and come prepared with your workbook to get the most out of this two week delivery. The person interviewed did not have any profound understanding of what the profession entailed, from what services they could offer to what age’s Occupational Therapists work with.

I think the term Paramedicine confused the lay person interviewed, they were familiar with the term paramedic but when asked what paramedicine was they were not able to identify the profession.

Physiotherapy

‘Someone who works with a person after injury’, ‘using physical manipulation to improve a patient after they’ve suffered from an illness’

This was the most understood out of the three professions but the level of their understanding only related to one aspect of the profession, this perhaps was to do with how the profession is perceived on social media.

Following Seminar 2: Summarise why understanding the professional role of others and being able to explain your own professional role is an essential interprofessional capability.As I progress into professional practice it will evitable that I will be working as a part of a multi-disciplinary team as a part of everyday practice and so therefore being able to understand the professional roles of others will be an essential interprofessional capability. Working on an interprofessional level can pose benefits as well as varying challenges. However, it is important that this collaboration is effective as it can have a significant impact on patient care and service delivery.

When considering interprofessional working, the initial challenges is with understanding how each profession communicates and the approaches taken. It is discussed throughout literature and imbedded in policy, legislation and guidelines how important communication is within healthcare. However, each profession talks a different language in terms of how they communicate and the professional language that is used, and therefore, setting up professional barriers that can limit successful multidisciplinary working. By being able to work alongside other professions through university and through practice placements it gives opportunity to start to understand how each professional role can contribute to effective team working and enhanced patient care. It enables each to learn from each other and broaden their knowledge of roles but also how to better inform a patient, creating opportunities to fill in knowledge gaps but also developing shared knowledge. In scope of this, in practice, it can support professionals own clinical decision making; maximizing the learning and professional experience, a profound movement with the NHS, to enable safer working practice.Understanding the professional roles of others can encourage effective interprofessional collaboration.

Role Misunderstanding reduces collaborative care. The way professions clinically reason is also known to differ. Understanding this difference can enable us to see where service improvements can be made and may influence role revision. Furthermore, being able to explain my own professional role, contributes greatly to interprofessional working as it gives opportunity to advocate for your own profession and showcase what skills and services you can provide, not only for patient care and recovery but also how our skills can contribute to working within a multidisciplinary team. It also enables us to recognise that, as a profession, working inter-professionally helps to work towards guidance and care pathways with greater effect. As one small incapability can cause a lack of depth of understanding of roles and reducing collaborative care.

Reflection

Were there any myths, regarding your own profession that surprised you?

There not necessarily any myths about the profession of occupational therapy but just the

confusion of how occupation fits within our service delivery and what each person’s

understanding of what occupation is.

As much as we can advocate for what we do, it is still widely associated with equipment

provision and adaptations and as much as that is a part of what we do as a profession it

does not fully represent what we do.

Going forward it will be so important to ensure that our professional role is thoroughly

understood to ensure patients are subject to the best services and care available.

What did you learn about your inter-professional colleagues?

That not only did the lay persons not have a full understanding on our profession but also my inter-

professional colleagues too had a vague understanding of what occupational therapy, as a

profession, entails.

Consent and capacity to consent to examination and/or treatment (XXXXX, 2015)

The policy and guidance provides information and ensure that all practitioners act in ways which are compliant with the legislation relating to capacity and consent regarding Codes of Practice relating to the legislation. It takes in account common law, new legislation and guidance relevant to capacity and consent, and clarification in the interpretation of the Mental Health Act (2007). It provides a means whereby practitioners can ensure they are adhering to notions of best practice in relation to capacity and consent.

How did it impact on your professional practice?

Patients have a fundamental legal and ethical right to determine what happens to their own bodies. Valid consent to treatment is therefore central in all forms of healthcare, from providing intimate care to undertaking major surgery. Seeking consent is also a matter of common courtesy between health professionals and patients.

What legislative drivers shape this policy?

Legislation relating to consent and capacity to consent to treatment is shaped by the Mental Capacity Act 2005 and the Mental Health Act 2007.

How social media can be used as a communication tool with service users

What is your current public facing profile? Consider which platforms you use and why?Personal

Instagram – Private account. I use this platform purely for creativity, I have an active interest in photography and like to use it to edit my photos.

Facebook – private account. I use this platform to be able to communicate and share my life experiences within my personal life and with my friends I have connected with from all around the world, as much as I don’t like to share too much information, it is an easy way to keep in touch with friends and family that you do not get to see very often. (This sentence is too long and therefore it loses its clarity)

Snapchat - purely for fun, again a private account but just a fun way of communicating with my friends and family.

Professional LinkedIn – the digital version of my CV. Easy way to find out what is going on in the recruitment

world and what is happening with different healthcare trusts. I have managed to connect and network with many by being. Apart of this online platform/tool and have also become aware of some job vacancies that I have not found anywhere else.(This does not make sense, there seems to be an unfinished sentence)

Twitter – I have only recently started to use twitter. Only used as a professional profile as a student and when I gain employment, I have found it very useful to network but also to join in some profession specific discussions.

Professional portfolio – I have built an online CPD portfolio, this has been useful as I know everything is in one place and I do not need to refer, everything is one place that is accessible.(You are repeating yourself here, which detracts from the clarity. Be more concise).

Skype – another way to be able to communicate professionally, when a face ti face meeting is not possible. Whilst away on my exchange I used skype to have personal tutor sessions and I had an interview over skype. I found this just enabled me to continue with everyday life even though I was in a different country. (Skype is not a public facing profile, it is a means of communicaion, Your email address or Skype address may be public facing)

What Safe guards do you employ to ensure that you stay within professional boundaries when using social media?

Self-disclosure – ensuring Information about myself and my personal life is not disclosed to clients.

Dual relationships - to not hold more than one type of relationship with a client, for example, they cannot work for me, be a family member or receive extra private support, as well as be one of your clients.

Working within your competence – important to understand the limitations of my role and personal capabilities, and when to refer to other professionals or to seek further support and advice for myself and clients.

Looking after self – to recognise that it is my responsibility to ensure that I am are fit to work, be conscious of my behaviour outside work but also how I manage stress and emotions within work.

Consider what you may use in your future professional practice?

Using social media in my future practice would very much depend on the client group I would be working with.

Much of social media I use, is still on a personal basis, and so I would have to carefully consider which media platforms to work with if it meant a client would benefit from this way of communicating.

Consider how social media could have / was used in your last placement to communicate with patients?

The platforms available now, I do not feel, would be appropriate to use to communicate with patients on my last placement. However, Skye could be potentially have been used but I still feel you need to be able to meet face to face, talk on the phone, to retain quality of service delivery.

You could explain this a little further, what platforms available now? Some further exploration/discussion regarding SKYPE could have been useful. What are the advantages and disadvantages of this?

1. Reflect on a time where communication created a problem in your practice and write about it.

Initial assessment with a person with LD

Situation: initial assessment/info gathering for a person with a LD

Task: to gather enough information to be able to judge what assessments to follow with to be able to understand patient need. I was nervous and spoke to fast and sometimes was not clear enough.

Action: my educator, gave me some constructive feedback...and on second visit to patient. I had reflected on what she had said and was able to clearly communicate with my patient and gain appropriate amount of info and carry out an initial assessment successfully. Receiving good feedback from my educator. Your writing style, poor use of grammar and punctuation means that what you have written is not making sense. Some further explanation here of the feedback you received and how you used it would have been useful)

Result: always think about person who communicating with and flex my communication style, improved upon my own occupational performance. Ended up establish great rapport with the patient and therefore enabling me to positive difference in the community. (This first sentence does not make any sense)

2. Reflect on a time where communication across multiple agencies improved a patient outcome, what worked well and what will you want to implement into your future practice?

Being a part of a multidisciplinary team meeting weekly, whilst on my second-year placement I could observe how this regular communication facilitated effective team working and over all improved patient outcome. It gave opportunity for al health care professions (doctors, consultants, nursing, occupational therapy, physiotherapy and healthcare assistants) to voice and concerns they had with a patient. It worked well as many of the professionals we involved with that patients care, allowing for each to report and update on their progress within the service. On some occasions, depending on which team members were present for the weekly meeting, a sense of hierarchy was evident among professionals and this sometimes impact on the cohesion of the group. But overall, I attended the meeting for 10 consecutive weeks, reflecting on the experience, I would look to participate in and actively advocate for working in this sphere. It generally improved outcome of patient care and really gave opportunity to get to know other professional within the service, effectively building on the team dynamics. Even though I did not participate in the meetings, by the end of the ten weeks I felt like a valued member of the team, something I would like to ensure I have the opportunity to foster in future practice.A useful piece of reflection here that demonstrates the benefits of MDT meetings. Try to be more concise and much more attention to detail is needed in checking for errors.

What was the aim of your session?

To advocate and represent our profession, raising awareness of and to help other healthcare professionals to begin to understand our profession in a little more depth.

Why did you select this aim?

It was clear from previous sessions that other healthcare professionals, as well as the lay people interviews, had a limited understanding of what Occupational Therapists are, what our job role is, how we can work with people, and what type of people we work with?

Reflect on the Seminar (continued).How did your session go? Is there anything you would change?

Really well, although we did not use a presentation as such, we chose to actively try and engage our audience and educate and raise of profession through doing, which is how we would work in practice! I think most people liked the quiz, and it helped to inform others of some facts about the profession that they will not have known, so hopefully they will be able to take something away with them.

If we could change anything:

We used a video at the beginning, maybe to have a shorter video and try generating a discussion/present information afterwards. It was challenging to put across in such a short space of time, everything about the profession.

What did you learn about your colleague’s professions? The session was good, very informative and felt I learnt a lot, especially from the presentation

from the physios.

Skills demonstrated by each profession through the presentations, showed how confident in their ability to advocate for their profession and for the skills they have learnt over the last 2/3 years on their degree program.

Everyone came across well and appeared to be able to convey information clearly.

It has made me recognize that professions overlap, that everyone has a skill set but sometimes these skill sets can be known to more than one profession.

That it will be important to be mindful of this when going into practice.

That we will be able to draw on knowledge and expertise from others and hopefully work better together.

How could this understanding impact on the quality of care provided by you in the future?

They raised our awareness of some simple breathing techniques, a skill/technique that will be good to know about when working in practice as a simple exercise can really help how someone feels in that moment in time.

Helped me to grasp a better understanding of professional roles of other allied health professionals and so therefore enable me to be able to effectively sign post/refer patients in the future, enhancing a seamless delivery of care and essentially improving the quality of the services I provide.

By completing 1000 hours of clinical hours whilst at university, I believe myself to be able work in partnership with people who use services and have been able to participate and be involved with the majority of the process involved with implementing an integrated action plan. For example, working on my triage skills to be able to gather information and work with others within the service to decide the best and most appropriate steps for patient care.

To further enhance this capability, I feel I need to gain more clinical experience, hoping to be able to take on more responsibility when transitioning to professional practice.

I feel I have developed my understanding and interpersonal communication skills through my final year placement where I worked within a learning disability setting. This built my awareness of how important communication is and enabled me to develop by highlighting the importance of accessible information and learning to flex my communication. I feel this skill would be applicable/transferable when working with any other service users or whom either do not speak English as their first language or who limited communication, such as people who have a neurological condition or dementia.

In addition to this, I have recently completed a Makaton foundation course which has given me a second form of communication, building upon and developing my interpersonal communication.

To develop my skills, I feel I need to practice my Makaton and gain more clinical experience to promote effective communication with the aim of enhancing person centred services.

Sharing profession-specific knowledge can enhance service delivery, which in turn impacts positively on the service user. Within one of my practice placements, a hand therapy team, the occupational therapists, on a weekly basis, would share their knowledge of a case study/condition/type of splint with the team. Sharing and exchanging knowledge. By attending the sessions, each professional would gain a deeper understanding of the topic discussed and the specific needs of the service user, enhancing collaborative multidisciplinary working and therefore service delivery.

By developing my understanding of the profession within the area of practice I will be working in, I feel I will enable me to share my own profession- specific knowledge. In addition, the inter-professional working module will enable me to gain more knowledge about the role of other health professionals

Over my past three practice placements, I have been able to observe and actively participate on interprofessional meetings, where the professionals and myself have discussed service user’s care, and relaying the service user’s preferences in the decision- making process. Some of the settings have had to hold best interest meetings, in line with the Mental Capacity Act 2005, to discuss with healthcare professionals, the family and wherever possible the service user themselves, to work collaboratively in planned decision making, in line with the person’s best interests.

To develop these skills further, I feel I need to gain clinical experience of collective decision making in practice.

In my third placement, I had various discussions with other occupational therapists about the limited profession specific evidence- base in working with adults with learning disabilities. I also reflected on the impact this could have on ensuring up to date practice, outside of government policy and guidelines, and adapting the service in line with the needs of the service user’s.

Acknowledging this led me to work in partnership with my educator to complete a clinical audit.Please see below, an exert from my practice placement report, as evidence to support this capability.

‘Charlotte has engaged in the process of writing and submitting an audit proposal to the R&D team, adjusting accordingly and gathering the data required to measure adherence to fidelity for individuals who have accessed the Ayres Sensory Integration Clinic, this is a valuable contribution to the Occupational Therapy service and the evidence base that is being collated and developed in this area of practice.’

To continue developing this capability, I will need to ensure that I am able to continually evaluate the evidence base for which ever NHS service I work within upon qualifying and how this is implemented in practice.

Over the course of my placement experiences, I have developed my skills in recognising my own learning needs, and utilised a learning contract for each placement in setting personal learning objectives to ensure I reached these goals. I feel this skill will enable me to identify my own learning needs throughout my career to ensure I maintain effective and safe practice.

In addition to that, I take an active interest in my professional development, I have built an online e-portfolio and try to add to it every 2-4 weeks.

Going forward, when embarking on my first clinical role as a qualified professional, I feel I will need to address the gaps in my own knowledge and abilities in clinical practice; formulating planned activities to support my future learning needs to help aid personal, professional and service development.

I have participated and engaged in reciprocal formal and informal processes of reflection and supervision over my last three clinical placements that has supported organisational/service development. Working alongside and generating reflective discussion with other professionals in order to develop my own understanding of the service, and how each professional fit within the service. ‘She will generate good reflective discussion when reflecting on the use of specific tools, underpinning theory and philosophy and has been able to provide best practice evidence in line with local and national guidelines for example NICE, BJOT.’

I have also participated in formal and informal discussions between the MDT regarding service development, such as developing a care pathway for the service and attending the service development review, where we had to present our updates and ideas for how to improve upon our sector of the service.

However, by gaining further clinical experience, I will be able to enhance my understanding of how reflection and supervision across professions can enhance professional development.

I feel I understand some differing cultures and values belief systems and how these can be respected within a health and social care setting. For example, my recent experience on an Erasmus exchange has given me further insight to differing cultural and value belief systems and I feel I have been able to learn skills that will be transferable, when working with communities of practice. However, by working in a person-centred and holistic way, I feel this can enable professionals to respect and be aware of cultural beliefs and values.

During my final year placement, I engaged and worked within a specialist community based learning disability service. One aspect of my role was to be involved in enabling access to services, to promote their health, well- being and independence. I used assessments to enable service users to state their interests and provide choice and autonomy in the care they received. Through using accessible information, I was able to enable empower the service user to make informed decisions. This also gave me opportunity to develop my own easy read information for the service user I was working with.

I feel I can develop this skill further by learning from other professionals on the strategies they use to enable autonomy and informed decisions in service users who may have difficulty in this area.

I am aware that professions within the community of practice may be subject to differing demands of law and regulation, where other health and social care professionals should adhere to their own professional body guidelines and procedures. However, I do not have clear knowledge of many of these. I have an understanding about ethical practices, and in avoiding harm to service users.

To further develop this knowledge, I feel that when I gain professional employment, I should take time to learn more about other professionals and the various legal frameworks, statutory and regulatory requirements of the differing professions. This will enable me to co-operate with others to ensure legal requirements of differing professions, for example in the exercise of a duty of care, are met.

Through my Learning Disability practice placement, I could gain a better understanding of the importance of enabling access to mainstream services, such as using advocates to ensure the service users choices are adhered to or by adapting the service; ensuring person centred care is implemented to address service users’ needs holistically. This, I feel, will be transferable to and when working with people of cultures or those who are marginalised to ensure equal access to mainstream services, and ensuring their needs are met.

To be able to develop and apply knowledge of critical understanding of the requirements of non-judgemental and anti-discriminatory practice to contribute to the delivery of culturally appropriate and person centred services, I feel I will need to gain more clinical experience – understanding the service I will be working with. This will enable me to identify how the service could be developed to be culturally appropriate in relation to the needs of the service users accessing the service.

Through completing numerous assignments at university and a interprofessional practice module, it has allowed me to apply and discuss how policy and practice influence changing role boundaries and service provision. For example, discussing NHS reports, such as the Winterbourne View inquiry and the Francis report, has enabled me to recognise and understand how they have influenced the development of best practice within health and social care settings; evaluating its influence on practice within various settings.

To further develop my understanding of this, I feel I need more clinical experience to apply this knowledge into practice in the area I will be working in, and how this applies to the multidisciplinary/interprofessional teams.Capable

I feel I have gained a good understanding of the team structures within the settings I have been on placement, and the various ways in which they communicate, formally and informally. Please see below an exert from a placement report, evidence to support this capability:

‘Charlotte has had the opportunity to attend and engage successfully in multi-disciplinary meetings whereby she has become a valued member of the team. She has demonstrated her ability to effectively adjust her communication style when talking among professionals through to working with service users accessing the service who often have complex communication needs which require both verbal and non-verbal skills.’

‘Charlotte has independently written service user notes which were timely and accurate, for the remainder of her placement she is going to complete an Occupational Therapy report after working through the Occupational Therapy process with an individual on her caseload. Having a caseload has meant Charlotte has made some clear professional relationships with those involved with the service user including; family members, carers and other professionals.’

Through gaining further clinical experience, I feel I will be able to develop this knowledge further. Being able to actively engage in team working, I believe I will then be able to exercise a critical understanding of team structures and effective team functioning through knowledge of group dynamics and professional roles across the community of practice.

During my final year practice placement, I began to interact and co-operate with others within and across organisations in providing person focused services, always working with the service user best interest in mind. An example from my practice placement final report provides evidence to support this capability:

‘Charlotte has settled in well across both teams and has made some effective relationships both within the immediate MDT and across external agencies for example, Charlotte has worked with health visitors sharing information to contribute to seamless effective care.’

To further develop this skill, I feel I need more clinical experience, so I am able to gain a fuller understanding of the wider inter-agency within my area of practice, but also to allow my confidence to grow so when appropriate to do so, participate and take lead in the delivery of services provided and reflect on the positive impact this will have on a service users experience of the care they receive.
