Instructions: Please indicate your choice of due date below. Remember to carefully read through the information on this form prior to submitting your request to SoFi.

I request all monthly payments on my SoFi Fixed Rate Personal Loan are set with a due date of __________ for each month.

Note: Available due dates 1-25th of the month. Due date changes may take 5 business days to reflect on the website.

SECTION 3: UNDERSTANDINGS AND AUTHORIZATIONS

I understand that:

• My loan must be current for the due date to be processed

• Due date changes can occur once per 6-month period

• Due date requests cannot be after the 25th of the month

• The new due date cannot be within 15 days of the date the request is submitted

• The new due date cannot be over 45 days from your previous due date

• If you are on auto-pay, your next payment will automatically be drafted on your next due date

• Due date changes may result in two payments being withdrawn in the same month

• Your loan will re-amortize once the due date request is processed, this may result in payment amountschanging
