Single complete dentureDR.HARSHA VARDHAN K.VSVSIDSThe prevalence of the condition where one edentulous arch opposesa natural or restored dentition is quite common. The primaryconsideration for a complete denture service is the preservation of thatwhich remains, a single conventional denture may be desirable when it isto oppose any one of the following:-11) Natural teeth that are sucient in number not to necessitate a !"edor removable partial denture.#) $partiallyedentulousarchinwhichthemissingteethhavebeenreplaced by a %&'.() $ partially edentulous arch in which the missing teeth will be) havebeen replaced by *&'.+) $n e"isting ,'.-ost commonly, it isthema"illaryarchwhichistheedentulousarch, the reason being that it is easier to retain, ma"illary, ,' in positionbecauseit is morestable, better toleratedbypatients andeasier tofabricate than mandibular dentures.DIAGNOSIS AND TREATMENT PLANNINGThe commonly cited long termgoal in prosthodontics is thepreservation of that which remains salient considerations are:-a) $cceptable interocclusal distanceb) .table/awrelationshipwith bilateral tooth contacts inretrudedclosure.c) .table tooth quadrant relationship, with a"ially directed forces.d) -ultidirectional toothcontactthroughoutasmall range0#mm)ofmandibular movements. These are the physiological characteristics seen mainly in dentatepatients but when only one arch is edentulous, unfavourable forcedistributions may cause adverse tissue changes. 1t is therefore, importantto identify some clinical changes and correct them.1. 2"tensivemorphologicchangesindenturefoundationresultinginarch relationship and occlusal plane discrepancies. #. 3aw relationship e"tremes. Thus, creating a hori4ontaldiscrepancybetweenarches anteriorlyandposteriorly. The forces of occlusion are resisted by the mucoperiosteum,2which allows some movement of denture base by virtue of its resiliency.7ut when movement of denture under load is great in one area than other dislodgement. These tissue changes ultimately lead to what is called as the singledenture syndrome in which patient compliances of loose or tiltingdentures and on e"amination damage to the mucosa and ridge resorptioncan be observed. -anagement includes treatment with reliners and tissueconditioners which temporarily cure the problem but the cycle of trauma, resorption and looseness continues.Thepatient becomesdissatis!edandedentulousridgecontinuesgetting resorbed.
